Gut-acting therapies are common therapies for irritable bowelsyndrome (IBS). Most of these peripheral acting agents are primarilytargeted at individual symptoms. The evidence supporting the use ofthese agents in IBS is largely anecdotal. Serotonergic agents and thechloride channel activator lubiprostone have shown efficacy intreating symptoms of IBS. The clinical evidence supporting the use ofthese agents is based on data from high-quality clinical trials. The useof serotonergic agents for IBS in the United States is limited to the5-hydroxytryptamine-3 antagonist alosetron in the treatmentof women with severe IBS with diarrhea refractory to traditionaltherapy.
