Before answering the following questions, run the "GEAR UP ScholarshipEligible Seniors" Report in the database. That report is the source of information for these questions AND serves as the final list of scholarshipeligible students from your program for this year. If students appear as "not eligible" whom you think should be eligible, there could be two reasons: 1) The student WAS an active GU participant in all relevant years, but data entry errors are causing him to appear ineligible (e.g., student was entered in the database as College Bound instead of GEAR UP, or as "inactive" instead of "active"). In this case, correct the bad "participation" entries and rerun the report. The program must maintain backup documentation of student participation for any student marked "active." 2) The student missed a maximum of two years of active participation in the program due to extenuating circumstances that you, as program director, deem legitimate. In this case, leave the report as is and indicate when you answer the following questions if there are any students whose eligibility you wish to appeal. The director's appeal is described in the Scholarship Administration Procedures and are due by April 1st of each year. Appeals are not allowed for students who did not participate in NJ GEAR UP in 7th grade. Once the report accurately depicts the program's scholarshipeligible seniors, print it; use it to answer the questions below; have the program director sign and date the last page; and submit the signed original with the program's printed copy of the GEAR UP Annual Performance Report.
