Lässt sich das Suchtgedächtnis löschen? Neue Wege in der Pharmakotherapie
Intervention
• Parallele Prozesse, gleiche Strukturen: NAc, Amygdala, PFC (Jentsch & Taylor 1999, Taylor et al. 2009)

Design: RCT; DCS vs placebo in combination with a exposure paradigm.Participants: 28 subjects with acrophobia acc. to DSM-IV.Interventions: 2 sessions of behavioral exposure therapy (virtual reality
exposure to heights within a virtual glass elevator), DCS or placebo were taken directly prior to each of the two sessions.
Main Outcome Measures: Symptoms were assessed by self-report 1 week and 3 month posttreatment. Presence and severity of acrophobia symptoms; electrodermal skin fluctuation was also included during the virtual exposure to heights.
Arch Gen Psychiatry 2004
Design: RCT augmentation trial examining the combination of DCS or placebo with exposure therapy for SAD.
Participants: 27 subjects with SAD acc. to DSM-IV with significant public speaking anxiety.
Interventions: 5 therapy-sessions, exposure to increasingly challenging public speech situation, 1h prior participants received DCS or placebo.
Main Outcome Measures: Standardized self-report before treatment, after treatment and 1 month after the last session.

insula, the amygdala, thalamus, the hippocampus/parahippocampus, mesocorticolimbic areas as the VS and the DS, the inferior, middle and superior prefrontal cortex (PFC), the anterior cingulate cortex (ACC) and the orbitofrontal cortex (OFC) ; p<0.001 uncorrected; cluster size >= 20 voxels

t2 (post-treatment): Striatal cue-induced activation (contrast “alcohol-neutral stimuli”)CET/Placebo (N=16) > CET/DCS (N=16)
stronger cue-induced fMRI activation compared to DCS patients in the VS [36 voxels, (x,y,z)=(12,10,-6), t=3.34] and DS [48 voxels, (x,y,z)=(-12,12,22), t=2.94]. There was no increased cue-induced activation in DCS compared to control patients in any brain region.p<0.05 uncorrected; cluster size >= 20 voxels; (x,y,z) = (12,10,-6)Interaction effect between VS activation on relapse behavior was observed in the whole group for post-treatment fMRI data (p = .033)
