(A) Monitor the accumulations of timber debris at the piers, and if found to be increasing, removal during routine maintenance to prevent further build-up and to restrict scour influence may be required.
(B) Because the bridge has been evaluated to be scour critical, specifically monitor the footing exposure during future inspections and after periods of high flows. Consider placement of scour countermeasures if foundation exposure is found to be increasing.
(C) Reinspect the submerged substructure units at the normal maximum recommended (NBIS) interval of sixty (60) months.

REPORT SUMMARY
UNDERWATER INSPECTION
The substructure units inspected at Bridge No. 6610, Piers 1 and 2, were found to be in good condition with no defects of structural significance. The footing at Pier 2 was exposed at the middle column and was flush with the channel bottom and at the upstream column there was 3 inches of vertical exposure. An area of section loss was present at the downstream nose of Pier 1. A light accumulation of timber debris was observed at Pier 1, and a light to moderate accumulation of timber debris was observed at the upstream end of Pier 2. Steel piles exhibited loss of painted coating with light corrosion and no section loss. The channel bottom consisted of firm sand and cobbles and appeared to be stable with no appreciable changes or increased scour since the previous inspection.
(A) The footing was exposed along the west side of the middle column of Pier 2 and was flush with the channel bottom.
(B) The footing was exposed along the west side of the downstream column of Pier 2 with a maximum vertical exposure of 0.4 feet.
(C) A small area of section loss was observed 2 feet above the waterline at the downstream nose of Pier 1 measuring 3 square inches with 1 inch of penetration.
(D) A light accumulation of timber debris, consisting of 6 to 8 inch diameter branches, was observed at the upstream end of Pier 1.
(E) Timber debris, consisting of 3 foot diameter and smaller logs, was observed at the upstream nose and along the east face of Pier 2 and extended from the channel bottom to 3 feet above the waterline and to the shoreline.
(F) Heavy bank erosion resulting in vertical slopes upstream and downstream of the structure on the east and west banks. Banks were well protected at the structure with up to 2 foot diameter riprap and concrete revetment. The west embankment protection was covered with silty sand.
(G) Steel piles had 100% loss of paint coating with light corrosion and no loss of section from the channel bottom to 3 feet above the waterline. From 3 feet above waterline to the cap, steel piles had 20% loss of paint coating.
(H) The channel bottom material around Pier 1 consisted of silty sand with approximately 1 foot of probe rod penetration.
(I) The channel bottom material around Pier 2 consisted of cobbles, 6 inch diameter riprap, and sandy silt with 4 inches of probe rod penetration.
INSPECTION FINDINGS
6.6Maximum Water Depth at Substructure(s) Inspected (feet):
The original piers each consist of two hexagonal-shaped reinforced concrete columns supporting a rectangular reinforced concrete cap. The columns are each supported by a square concrete footing founded on steel H-piles. The extended portion of the piers added to the north end of the bridge each consist of a single H-pile encased in concrete and a steel pile shell. The steel shell and concrete encasement extend up into the rectangular reinforced concrete cap.

The superstructure consists of three spans of multiple steel beams supporting a reinforced concrete deck. The superstructure is supported by two reinforced concrete piers and two reinforced concrete abutments. The superstructure was widened in 1990 with the addition of two pile bents and an additional steel beam. The original substructure units are founded on square reinforced concrete footings founded on steel H-piles. The piers are numbered 1 and 2 starting from the west end of the bridge.

The routine underwater inspection of Bridge 6610 (CSAH No. 15 over the Minnesota River) was completed on June, 18, 2016. The underwater inspection was conducted from shore. The inspection was conducted by a team consisting of a PE-Diver with a valid MnDOT Team leader certification, a backup diver and a dive tender. The inspection utilized commercial dive equipment and techniques (SSA and/or SCUBA) in accordance with OSHA regulations. Profiles were taken along the upstream and downstream faces of the bridge and around the periphery of substructure units to determine the presence, location and area of scour.
The bridge elements inspected consisted of 4 reinforced concrete columns and 2 steel pilings. According to the bridge inventory or design drawings, Piers 1 and 2 were founded on steel H-piles. Inspection procedures followed FHWA guidance and the MnDOT Bridge and Structure Inspection Program Manual with channel bottom probing to search for bottom of foundations. The routine underwater inspection frequency is recommended to remain at 60 months based on those findings and risk factors. Also, inspection procedures should continue to follow the above approach and standard guidance with 100% Level I and 10% Level II intensity efforts.
