Dear Oklahoma Hospice Provider, The Oklahoma Hospice Advisory Board is committed to promoting the availability and delivery of quality hospice care for all Oklahomans. We are writing to assist you and your hospice organization in preparing for quality measures reporting to the Centers for Medicare & Medicaid Services (CMS) in the very near future. What we know as of November, 2011: In accordance with the Affordable Care Act, hospice providers must report quality measurement data on selected quality indicators as determined by the Secretary of Health & Human Services no later than Fiscal Year (FY) 2014. If the hospice provider does not submit the required data, the provider will have their market basket rate reduced by 2% for that FY. The Secretary of Health & Human Services looks to the National Quality Forum (NQF) and other national entities for validated quality measures when selecting the quality indicators to be reported by hospice providers as part of this national public reporting initiative. Two measures have been identified for quality reporting in the near future: Outcome Measure (NQF-#0209)

1. Comfortable Dying – Percentage of patients who were uncomfortable because of pain on admission to hospice whose pain was brought under control within 48 hours. First year data collection timeframe: October 1, 2012 – December 31, 2012. Data submission will be due April 1, 2013. This first year data collection will be for the 3 months of calendar year 2012 only. Every year afterwards, the measurement data will be submitted annually.

Structural Measure 2. Each Hospice Provider will select 3 quality indicators from its own Quality Assurance &

Performance Improvement (QAPI) program and the quality indicators must be related to patient care.

First year data collection timeframe: October 1, 2012 – December 31, 2012. Data submission due January 31, 2013. This first year data collection will be for these 3 months only. Every year afterwards, the measurement data will be submitted annually.

The NQF Palliative and End-of-Life Care Steering Committee met over the summer of 2011 and recommended fourteen performance measures for evaluation of quality palliative and end-of-life care in the future. The recommended measures for palliative care and end-of-life care are now available for review on the NQF website:

Transcripts and relevant materials from the Steering Committee meetings are also available on the NQF website. The final decision on endorsement of the recommended measures will occur early in 2012.

NQF is a nonprofit organization based in Washington, D.C. that reviews and endorses standards for measuring and reporting on performance in healthcare. Most measures selected by CMS for quality reporting must have NQF endorsement.

It is important to note that the Board believes we can expect additional mandatory measures in FY 2013, 2014 and 2015. The Oklahoma Hospice Advisory Board also recommends each hospice provider begin selecting the three (3) patient-related quality measures of the provider’s QAPI program to be included in this public reporting initiative. Although there are only two measures selected for required quality reporting to CMS in the coming year, the Oklahoma Hospice Advisory Board recommends each hospice provider thoughtfully review the quality indicators currently being evaluated by the NQF as well as those quality measurements included in the PEACE Project to prepare for quality reporting and as tools to help improve the quality of hospice care provided to Oklahomans. THE OKLAHOMA HOSPICE ADVISORY BOARD:

Authorized by Section 1-860.13 of the Oklahoma Hospice Licensing Act {63 O.S. §§ 1-860 et seq.}.

Appointed by the Governor and functions as a professional advisory body to the Oklahoma State Department of Health (OSDH) for the development and improvement of services provided by hospices.

The Board:

• Reviews and approves in their advisory capacity administrative rules for hospice licensure;

• Evaluates practices and procedures of the Department regarding the administration and enforcement of the provisions of the Act; and

• Annually publishes a report of its activities and recommendations for improvement of hospice services and patient care to the Governor and the Commissioner of Health.

THE HOSPICE PHILOSOPHY:

• Hospice provides: o A continuum of home and in-patient palliative and supportive care; o Services are medically directed, nurse coordinated, and individualized; o Care is available twenty-four hours a day, seven days a week; o Care is provided on the basis of need, regardless of the ability to pay; o Bereavement services to the family continue after the patient's death.

• Hospice caregivers are: o Nurses; o Physicians; o Hospice aides; o Therapists; o Counselors, psychologists, and social workers; o Dietitians; o Homemakers; o Clergy; o Volunteers.

• Hospice services are: o Based on a plan of care; o Developed to meet the special needs of the patient & family; o Designed to support the best quality of life possible.

• Hospice patients and families:

o Make an informed election of the service; o Acknowledge curative care related to the terminal illness will no longer

be covered; o May revoke the election of the hospice benefit at any time.
