Why Choose the Geriatric
Assessment Program (GAP)?
The program provides a “no wrong door”
approach to senior healthcare. Clients are
assessed by a team of the finest Geriatricians
Social Workers, Occupational Therapists and
Nurses in Southern Nevada. GAP encompasses
all of the clients’ diseases, disabilities,
cognitive abilities, medications, self-care
behaviors, health-related lifestyle habits,
psychological conditions, environmental risks,
family and/or friend support systems.
Will I have to change my Primary
Care Physician?
No, the GAP program will send all reports to
your primary care doctors, as directed by the
patient. The complete GAP assessment is
approximately 2 hours. Appointments allow
patients and/or family/caregivers, 45 minutes
to discuss their health issues with the geriatric
physician. This allows adequate time to answer
questions and review the patient’s and
caregiver’s needs. GAP is an evaluation
program. GAP physicians make
recommendations; they do not order tests or
write prescriptions.
How Does GAP Help Caregivers?
At the GAP Program, we look at the “whole
picture”. This includes offering support,
information and services to benefit both the
patient and the caregiver.

community programming to support seniors and
adults with disabilities in their effort to remain

Nevada Aging & Disability Services

GAP is free for older adults 60+. Physicians’
fees are billed to medicare/insurance or
paid through a grant from NV ADSD. There
is no cost to the patient.
