The following information will be necessary when you register at the
Welcome Center1. Proof of Residency (mortgage coupon/settlement
statement, rental agreement or electric/gas bill with address)
2. If not a current resident, purchase agreement or a letter from rental office is needed, and will be accepted on a 90 day waiver period.
3. Custody Papers (if applicable)4. Parent/Guardian Driver’s License or State Photo ID card5. Original or Certified copy of child’s Birth Certificate6. Current IEP or MFE (if applicable)• Your child is not registered until ALL
forms are returned to the Welcome Center.

• Call for an enrollment PIN at 614 920-6180• Go to website: www.pickerington.k12.oh.us• Click on “Enroll in the District”• Click on “Complete Online Registration”• Enter PIN• Complete forms online

Kindergarten Schedule• AM - 9:40 – 12:10• PM - 1:10 - 3:40• Mark AM/PM preference on registration form
(lottery if needed). Registrations must be processed prior to May 4 to be included in the lottery.
• First Day for Kindergarten – Thursday, August 30, 2012
• Class Placement, Supply list, Fee amount - You will receive a letter in August.

Kindergarten Admittance
• Legal Requirement: Child must be
5 years old on or before August 1, 2012
• Fees – A student fee will be charged for consumable supplies and may be paid at the Parent/Teacher orientation meeting in August.

How Can I Help My Child?
• Use the Parents’ Guide to Student Success• Build a relationship with your child’s teacher• Set high expectations and support your child
in meeting them• Carve out time everyday when your child can
concentrate on reading, writing, and math, uninterrupted by other distractions

Parent/Teacher Orientation Meeting
• Check the school website, local papers, and school calendar for dates and times for this event. You will also receive a letter informing you of the date.

ELL= English Language Learners
Shari Granados
• 7 different languages are represented in our school• 12 students are Limited English Proficient and
receive services• We are thrilled to have such a diverse group of
students, and we welcome all cultures, ethnicities and races

Home Language Survey
• Required by the State of Ohio to help determine students that may qualify for services
• If English is not the 1st language spoken at home, you will complete a survey as part of registration and will then be contacted by an ELL instructor for an assessment appointment

Bracken School Readiness Assessment
• Your child will be assessed on May 9, 10, or 11, at Toll Gate Middle School, 12089 Toll Gate Road
• You will set up an appointment for this assessment at your registration appointment at the Welcome Center
• The Bracken assesses six basic skills: colors, letters, numbers/counting, sizes, comparisons, and shapes
• The assessment is individually administered and the concepts are presented orally in complete sentences and visually in a multiple choice format. The child is not required to read or write.

• Only students identified by parents will be screened• If you suspect a speech problem, inform the Welcome
Center during registration• If requested, speech screenings will be held during the
Bracken assessment in May

Physician’s Report• DUE by August 8, 2012• REQUIRED FORM: must be on file at school
before your child can attend school• Will receive form from the school tonight• Schedule physician appointment NOW• Physical must be within the last year: 9/01/11 - 8/08/12• Must be signed and dated by the physician• Contact School Nurse if Physical is scheduled
after 8/08/12

• 4 or more DPT Diphtheria, Tetanus,Pertussis• 3 or more OPV Oral Polio Vaccine• 2 MMR Measles, Mumps, Rubella• 3 Hepatitis B• 2 Varicella (or documentation of
having the disease)• The first MMR and Varicella must have been
given after child’s 1st birthday

• When completing the Health History form online, please include special medical concerns
• Request a meeting with the school nurse if there are special medical concerns: Asthma, Diabetes, Seizure Disorders, Behavior Concerns, Bowel or Bladder Issues, Medications, Allergies

Prescription Medication Administration Forms
• Must be completed by the physician and parent or guardian
• Original container from the pharmacy
• Child’s name, medication name, dosage, route
• Side effects and special instructions

• Must be in the original container• Must be appropriate for age as written on
medication container • Must complete the form or send in a note with
the following:• Child’s name, medication name, dosage, time
to be given• Start and stop date
