We would like to share some information with you about our policies and procedures below: Rent : Rent payments should be made payable to Kimberly P. Cappolla. For your convenience, you may directly deposit your rent into the owner's bank account at Regions Bank, Account #0135535105. You must request a hand-written deposit slip and write your address on it for your rent to be properly credited. You may set up automatic bill payment through your online banking, or you may mail rent, at your own risk, to P O Box 6091 Jacksonville, Florida 32236. Your rent is due on or before the first day of each month. Utility Shut-Off Locations : Electrical main: __________________________________________ Water main: _____________________________________ Trash Collection : Trash collection day is ________________________. Recycle collection day is ___________________________. Your trash cans should be placed in the proper pick up area no sooner than 5:00pm the evening before pick up and should be replaced to the proper storage area after pick up. If you need a recycle bin, these are provided by the city and you can contact them at 904-630-2489. Pest Control : All buildings are routinely treated for roaches, ants, and spiders. If you see any of these pests in your home, please contact Peninsular Pest Control at 904-389-3491 to set an appointment for an interior treatment. If you see signs of any other pests, such as rodents, please contact Marijo. If you have a pet in your home and have fleas, you are responsible for their immediate extermination. Lockout Procedure/Lost keys : In the event that you are locked out of your home, please contact a locksmith at your expense. You may call or text Marijo and if she is able, she will let you in for a fee of $30 that is due at the time of entry. If your keys are ever lost or stolen, please contact us immediately. Please do not change the locks at any time. Unit Inspections: We perform semi-annual inspections at all of our properties routinely, usually in April and October. You will be notified in writing prior to these inspections. Inspections are both to allow us to perform preventative maintenance in your home as well as to inspect that you are complying with the terms of your Rental Agreement. Maintenance and Repair Requests : If you have a maintenance or repair request, please notify Marijo Kitchen, preferably by text message, or phone call to 904-382-8768 Monday through Thursday between 9:00am and 4:00pm. At any time, should you have an emergency, you must call 911, then contact Marijo immediately. If you have a water/sewer or electrical emergency and are not able to reach Marijo, contact JEA at 904-665-6000. Please be reminded of your responsibilities for maintenance and repairs under our Rental Agreement: Report all repairs immediately and to be responsible for any further damages in the unit or building resulting from not reporting repairs in a timely manner; Contact Marijo immediately if any appliance is not working properly and be responsible for the preservation of any perishable items. In addition, once needed repair is reported, Tenant(s) must remove all items from in, on, and around the appliance; INSPECT BLINDS MONTHLY: Keep all blinds clean and free from dust/dander buildup and to replace any damaged blinds with like materials INSPECT FIRE EXTINGUISHER MONTHLY: If the fire extinguisher is used or found to be not in proper working order, Tenant(s) shall be responsible for replacing it immediately, and then notify Owner of same; TEST SMOKE DETECTORS MONTHLY: Tenant(s) shall be responsible for changing batteries as needed and if after replacing the battery, the smoke detector does not work, inform the owner immediately; INSPECT TRASH/TRASH CAN AND LID MONTHLY: Tenant(s) must keep trash can and lid clean and in good repair, and maintain trash; INSPECT HVAC FILTERS MONTHLY: Inspect the heat and air filter, at a minimum of once per month if there are no pets, and bi- weekly if there are pets or if the system return is in the floor, and if it is dirty: Central Heat & Air Systems- replace it with a new, pleated filter. Window units- turn the unit off, wash filter with mild soap and warm water, air dry, then replace the filter; REPLACE STOVE DRIP PANS AS NEEDED; REPAIR WALLS/PAINT AS NEEDED: Tenant(s) must maintain walls, woodwork, and paint in the same condition they were in on move-in day. Tenant(s) are not allowed to change paint without prior written permission from Owner; REPAIR WINDOW GLASS/SCREENS AS NEEDED: Tenant(s) must maintain all window glass and screens; REPLACE LIGHT BULBS AS NEEDED: Tenant(s) must replace light bulbs when they go out; KEYS/LOCKS: All keys and locks remain the property of the Owner. Tenant(s) shall not remove, replace, or add any locks to any door. If Tenant places additional locks on any door, the landlord shall remove same forthwith at Tenant’s expense. Tenants shall return all keys and copies made upon termination of this agreement; FIREPLACE: Tenant(s) must exercise safety measures when using the fireplace. This includes, but is not limited to, never leaving a fire or hot embers unattended by an adult, always using a fireplace screen, and for wood-burning fireplaces, always using a grate placed at the back of the fireplace, making sure the flu is open when in use, and only burning 2-3 small logs at a time. Pine logs may not be used at any time. Renters Insurance: It is your decision whether to purchase renters insurance. The Owner is not responsible for the Tenants personal property at any time. We highly recommend this inexpensive insurance to cover your personal belongings and/or liability. Please feel free to contact us if you have any questions.
