1. Name of Nominee/Property: For staff awards, list the nominees first and last

name as they would like to appear on the MADACS award if they win, followed by the name of the site.

Ex: Jon Doe/Sunshine ApartmentsAll other categories – please list the site and/or corporate name only.

2. Address of Nominee/Property: This line is for the actual site where the

property/nominee works. This is the address the nominee will be judged at. Judging schedules are based on location, so please verify all information is correct before submitting your final entry.

3. Phone/E-mail: This should be the contact phone and e-mail for the actual nominee. We need this information for judging purposes.

Each entry must have a contact person listed or submission will be returned. A valid phone and e-mail are required. The contact person is the person submitting the entry on behalf of the nominee. For staff awards, the person submitting the entry must be a supervisor or employer.

Please note: The only way to have more than one company listed on/or receive an award is through a joint entry.

For each entry you will need to supply one judge for another category. Entries submitted without a judge will be returned and considered incomplete.
