This Provider and Pharmacy directory is current as of May 2015. Some network providers may have been added or removed from our network after this directory was printed. We do not guarantee that each provider is still accepting new members. Section 1 IntroductionThis directory provides a list of IEHP Medicare DualChoices network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage.

You will have to choose one of our network providers who are listed in this directory to be your Primary Care Physician (PCP). The term PCP will be used throughout this directory. Generally, you must get your health care coverage from your PCP. You will usually see your PCP first for most of your routine health needs. Your PCP will also help you coordinate the rest of the covered services you get as a Member of your Plan. Coordinating your services includes checking or consulting with other Plan providers about your care and how it is going.

This includes: y Your x-rays; y Laboratory tests; y Therapies; y Care from doctors who are specialists; y Hospital admissions, and y Follow-up care.

The network providers listed in this directory have agreed to provide you with your health care/vision/dental coverage. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue going to that same provider. In some cases, you may get covered services from non-network providers. You will usually see your PCP first for most of your routine healthcare needs such as physical check-ups, immunizations, etc. When your PCP thinks that you need specialized treatment or supplies, your PCP will need to get prior authorization (i.e., prior approval) from your plan and/or medical group. This is called a referral. It is very important to get a referral (approval in advance) from your PCP before you see a plan specialist or certain providers. If you dont have a referral (approval in advance) before you get services from a specialist, you may have to pay for these services yourself.

In cases where non-contracting providers submit a bill directly to you, you should not pay the bill, but should submit it to us for processing and determination of your liability, if any. IEHP Member Services mailing address can be found on the outside back cover of this directory.

Emergency care can always be obtained in or out of the service area from the nearest available provider. If you have a medical emergency, get help as quickly as possible. Call 911 for help or go to the nearest emergency room, hospital, or urgent care clinic. In addition, when out of the services area, you can obtain dialysis treatment from any qualified dialysis provider.

After the emergency is over, you are entitled to follow-up care to be sure your condition continues to be stable. Your follow-up care will be covered by our plan. If your emergency care is provided by out-of-network providers, we will try to arrange for network providers to take over your care as soon as your medical condition and circumstances allow.

You must use plan providers except in emergency or urgent care situations. If you obtain routine care from out-of-network providers neither Medicare nor IEHP Medicare DualChoice will be responsible for the costs.

If you do not have the Provider and Pharmacy Directory, you can get a copy from IEHP Member Services at 1-877-273-IEHP (4347) or TTY at 1-800-718-4347 from 8am-8pm (PST), 7 days a week, including holidays. A complete list of network providers is also available on our website at www.iehp.org. You can ask IEHP Member Services for more information about plan providers, including their qualifications and experience. IEHP Member Services can also give you the most up-to-date information about changes in plan providers and about which ones are taking new patients.

Usted tendr que elegir como su Doctor de Cuidado Primario (Primary Care Physician, PCP) a uno de los proveedores de la red de servicios mdicos que se muestran en este directorio. La sigla PCP se utilizar a lo largo de este directorio. Por lo general, usted debe obtener su cobertura de atencin mdica por parte de su PCP. En casos en los que los proveedores que no tienen contrato le enven una factura directamente a usted, no debe pagarla, sino presentrnosla para que la procesemos y determinemos su responsabilidad, si es que la tiene. Siempre puede obtener atencin mdica de emergencia dentro o fuera del rea de servicio con el proveedor ms cercano disponible. Si tiene una emergencia mdica, obtenga ayuda lo ms pronto posible. Llame al 911 para pedir ayuda o dirjase a la sala de emergencias, la clnica de atencin urgente o al hospital ms cercano. Adems, cuando est fuera del rea de servicios, puede obtener tratamiento de dilisis por parte de cualquier proveedor de dilisis calificado.

The Primary Care Physicians (PCPs) in this directory are listed by county, then by city. The plan specialists are listed by specialty. When choosing a PCP, make sure that the PCP you select is located within 10 miles or 30 minutes of your home. All providers in this Provider and Pharmacy Directory accept both Medicare and Medi-Cal benefits.

When making your choice, you might also consider the following factors:

y If the Doctors office is open evenings and weekends

y What languages the Doctor speaks

y If the Doctor takes walk-ins or same-day appointments

Accessibility Indicator Description

B- Basic Access Demonstrates facility site access for the members with disabilities to parking, building, elevator, doctors office, exam room, and restroom.

L - Limited Access Demonstrates facility site access for the members with a disability is missing or is incomplete in one or more features for parking, building, elevator, doctors office, exam room, and restroom.

P - Parking Parking spaces, including van accessible space(s), are accessible. Pathways have curbed ramps between the parking lot, office, and at drop off locations.

EB - Exterior Building

Curbed ramps and other ramps to the building are wide enough for a wheelchair or scooter. Handrails are provided on both sides of the ramp. There is an accessible entrance to the building. Doors are wide enough to let a wheelchair or scooter user enter, and have handles that are easy to use.

IB - Interior Building

Doors open wide enough to let a wheelchair or scooter user enter, and have handles that are easy to use. Interior ramps are wide enough and have handrails. Stairs, if present, have handrails. If there is an elevator, it is available for public/patient use at all times the building is open. The elevator has easy to hear sounds and Braille buttons within reach. The elevator has enough room for a wheelchair or scooter to turn around. If there is a platform lift, it can be used without help.

R - Restroom The restroom is accessible and the doors are wide enough to accommodate a wheelchair or scooter and easy to open. The restroom has enough room for a wheelchair or scooter to turn around and close the door. There are grab bars which allow easy transfer from wheelchair to toilet. The sink is easy to get to and the faucets, soap, and toilet paper are easy to reach and use.

E - Exam Room The entrance to the exam room is accessible, with a clear path. The doors open wide enough to accommodate a wheelchair or scooter and are easy to open. The exam room has enough room for a wheelchair or scooter to turn around.

T - Exam Table/Scale

The exam table moves up and down and the scale is accessible with handrails to assist people using wheelchairs or scooters. The weight scale is able to accommodate a wheelchair.
