L ET T E R F ROM OU R PR E S I DE N T

on behalf of the Hoag family, we are pleased to

provide this annual report to our community.

i wish to convey my personal thanks to the thousands of colleagues and supporters who have

made Hoag one of the finest hospitals and health systems in california.

this was a momentous year for Hoag. in 2010, we opened a new hospital in irvine and created

one of the largest orthopedic institutes in the united states. in addition, we were pleased to

celebrate 25 years of community presence in Huntington Beach. new technology was added

at many of our care delivery sites, with robotics and computer-based tools now becoming

commonplace throughout the organization. We created many new and innovative relationships

with our physicians, each of which promotes more effective ways to provide high-quality,

cost-effective care. our nursing program was also recertified as a magnet®-designated hospital

by the american nurses credentialing center. lastly, we redefined our brand from a hospital to

a healthcare system and created a new logo, all to ref lect our evolving position in the healthcare

delivery landscape of orange county.

While many new and important events have occurred this past year, we remain committed

to our mission, vision, and values – to provide the highest quality health care services to the

communities we serve, while we remain a trusted and nationally recognized health care leader.

in addition, we do all this while keeping our institutional values at the fore – excellence, integrity,

respect, patient centeredness, and community benefit.

We thank you for the opportunity to serve this outstanding, supportive community.

richard afable, m.d.

Trisha Mather’s storytrisha mather was diagnosed with stage one breast cancer just days

after completing the 2010 orange county marathon. nearly one year

after her mastectomy and reconstruction, trisha is back enj oying

running and her family.

Beating Breast Cancertrisha mather, a 38-year-old wife and mother of two, successfully

completed the 2010 orange county marathon, just a few days

before she was diagnosed with stage one breast cancer.

in early 2004, trisha discovered what turned out to be a benign lump, during a self-breast exam.

she became even more diligent with self-breast exams, which thankfully led to her detection of

two lumps in 2010. trisha immediately called her primary care physician, who sent her to Hoag

Breast care center in newport Beach, for a diagnostic mammogram and ultrasound, the later of

which showed she had stage one invasive ductal carcinoma.

“When i was told i had breast cancer, my thought was ‘i have to be here for my kids, i have to survive for

them’,” shares trisha. she credits the breast care team, including breast care nurse navigator Josephine

urbina, r.n., for talking her through the next steps of what she would experience. “the guidance and

assurance i received really set the tone for the upcoming year and i am forever grateful for that.”

after seeing a general surgeon and receiving a referral to Hoag Breast care surgeon lisa guerra, m.d.,

trisha felt confident she was in great hands. after undergoing a breast-conserving lumpectomy,

trisha underwent four rounds of chemotherapy. ultimately, to minimize her chance of future

lumps, trisha opted to do a bilateral nipple-sparing mastectomy and reconstruction.

“ken [her husband] and i really tried to maintain our quality of life with our work, kids and activities

and running was one of my releases during chemo,” shares trisha. “i ran to show my family i would

keep fighting and that we’d get through this and we did.” now nearly one year after trisha was

declared in remission, she is back running and planning her family’s summer 2011 vacation.

Conquering a Heart Attackas an active and competitive tennis player, daud ahmed prided

himself on being the epitome of health and fitne ss. that all

changed on february 7, 2010, when daud collapsed after

playing tennis at the Palisade s tennis club in newport Beach.

daud had no apparent warning signs of heart disease, only a family history of the disease, but had

always thought that his healthy lifestyle would conquer his genetics. if it had not been for the quick

reaction from his fellow tennis club members, the quick response by the paramedics and quick action

from Hoag’s experts, daud could have faced more excruciating challenges, and possibly death.

When daud arrived at Hoag, the cath lab team including cardiologist carey o’Bryan, m.d., took

him immediately from the ed to the catheterization lab where they proceeded with coronary

angioplasty and stent implantation to clear the blockage causing daud’s heart attack. daud’s

coronary artery disease was extensive and he had multiple blockages aside from the culprit

artery that caused his heart attack. during his hospitalization, Hoag’s team determined that

coronary bypass surgery would need to be scheduled at a later date allowing daud time to heal.

five and a half weeks after his heart attack, daud had successful coronary artery bypass surgery by

colin Joyo, m.d., one of Hoag’s exclusive cardiac surgeons.

“i truly believe that my active lifestyle saved me,” says daud. “However, even in the best of shape,

there could be health issues that are asymptomatic at first, then become apparent when it’s too

late, making it all the more important to be proactive when it comes to your health.”

now, just one year after his heart attack and surgery, daud is back playing tennis.

Daud Ahmed’s storydaud, a 64-year-old competitive tennis player, collapsed on his home tennis

court from a heart attack in february 2010. due to quick action from

fellow club members, newport Beach paramedics, and Hoag’s cath lab team,

daud survived and is back on the courts and enjoying every moment of life.

Claire Squier’s Storyclaire squier was 19 when she was diagnosed with type one diabetes. claire’s

endocrinologist has taught her to manage her diabetes through diet, exercise and

insulin regulation. claire, now 27 and pregnant with her first child, is a personal chef

and credits diabetes with helping her pursue her passion for food and helping people.

Diabetes Careclaire squier’s dream has always been to be a mother, so when

she was unexpectedly diagnosed with type one diabete s at

age 19 in Hoag’s e mergency department, her first thought was

‘can i still have children one day?’

“When you don’t know the signs of diabetes, you don’t think anything is really wrong,” shares

claire. “i was in denial when dr. iyer told me i had diabetes for the first time because i didn’t want

to accept the fact that i had to change my lifestyle.”

kris iyer, m.d., is claire’s endocrinologist and the medical director of the mary and dick allen

diabetes center at Hoag. “diabetes is a complex chronic disease. However, with better management

and monitoring in the early stages, we can increase positive outcomes and reduce the risk of

complications.” dr. iyer adds “Programs and education classes at the allen diabetes center can

help people like claire learn to live more successfully with diabetes.”

claire, now 27 and pregnant with her first child with husband casey, is a personal chef and credits

diabetes with helping her learn about food and how it affects the body. “By following my dream

and passion for food and helping people, i am also able to take better care of myself.”

claire is now a part of sweet success, a program within the allen diabetes center which provides

support and education for women who have or develop diabetes while they are pregnant.

“it ’s wonderful to have another resource to help guide me with diet during my pregnancy.”

Don Robert’s Storydon roberts was diagnosed with stage one lung cancer in June 2009. He was

referred to richard fischel, m.d., a Hoag-affiliated thoracic surgeon renowned for

his skill in minimally invasive lung surgery. Just one year following surgery, don

is back golfing at his favorite course and is enjoying time with his wife and family.

Minimally Invasive Lung Surgeryalthough don roberts quit smoking in 2000, his primary care

physician urged him to have a che st ct scan in June 2009

because he actively smoked for more than 40 years.

don trusted his physician and did the scan, which revealed an area of concern. after monitoring the

growing area for six months, don’s physician referred him to richard fischel, m.d., a Hoag-affiliated

thoracic surgeon renown for his skill in video-assisted thoracoscopic surgery (Vats), a minimally

invasive, leading-edge procedure that requires only four small incisions through which surgical

instruments and a thoracoscope are placed.

“a cancer diagnosis is not something you expect,” shares don. “yet, at Hoag cancer institute, and

under the care of the lung cancer program team, i felt confident that i would beat my lung cancer.”

dr. fischel explains that, “in cases like don’s, the minimally invasive Vats technique offers clear

advantages over the open-chest procedure because patients tend to recover more quickly,

experience less post-operative pain and there’s a lower risk of post-operative complications.”

and don was right. due to clear surgical margins and an overall positive assessment of his case,

don received the good news that no chemotherapy was needed; only a routine lung ct scan every

six months. now, nearly two years following surgery, don is back golfing at his favorite course, seacliff

in Huntington Beach, and enjoying time with his wife and large family of children and grandchildren.

“i can’t thank my doctor enough for letting me know about the importance of early detection,” says

don with a smile. “and i am equally thankful for the life-saving care that Hoag cancer institute’s

team, and e specially nurse navigator lilian reed, r.n. , provide s to patients like me each day.”

Lynn-Holly Johnson’s storythe 52-year-old actress, wife and mother of two, had a life-threatening stroke in

January 2010. With quick action from Hoag’s stroke team in the ed, a little luck from a

dissipating blood clot and one full year of rehabilitation and support from her family

and friends, she is back skiing, acting, writing and enjoying time with her family.

Surviving a strokeactre ss and newport Beach re sident lynn-Holly Johnson was

just three weeks past her 51 st birthday when she had a stroke.

after arriving at John Wayne airport after a flight with her parents, she describes what felt like an

out-of-body experience where she was watching herself stumble around the airport but couldn’t

communicate. When she couldn’t remember her address for the taxi driver, her parents recognized

this was an emergency.

once lynn-Holly’s husband, kelly, got her to Hoag’s ed, nursing staff immediately recognized that she

was having a stroke. Hoag’s stroke team opted not to give her the tPa clot-busting drug because the time

her symptoms started wasn’t definite. after careful observation, david m. Brown, m.d., neurologist and

Program director for Hoag’s stroke Program, noted that she showed signs of improvement. Because of

the continually-updated and evidence-based stroke protocols followed by Hoag’s state-of-the-art stroke

unit, she received an extensive and quick workup, which is especially important for unusual causes

of stroke, which can been seen in younger patients like lynn-Holly. after her examination, the team

discovered she had a patent foramen ovale (Pfo), a hole in her heart that didn’t close properly after

birth, which if left untreated, could cause further problems.

“my mission is to educate people on heart disease and stroke awareness and to empower women to be

proactive about their health,” shares lynn-Holly. after one year of cognitive therapy and successful

surgery to repair the hole in her heart, she is now a spokesperson for the american Heart association

and starting to act, write and get back to her favorite sport – skiing. “i’m lucky i have an awesome family

and i’m lucky i have such an amazing hospital within minutes from John Wayne airport.” a former

professional figure skater, lynn-Holly’s extensive acting career includes a golden-globe nominated role

in 1978’s Ice Castles and Bond girl Bibi dahl in the 1981 James Bond movie For Your Eyes Only.

Page 28: Who We Serve Who Serves - Hoag daud arrived at Hoag, the cath lab team including cardiologist carey ’Bryan, o m.d., took him immediately from the ed to the catheterization lab where

Patty G oethals’ Storythanks to hereditary cancer screening

and testing, three of Patty’s four siblings,

and possibly her two sons, have all

discovered their predisposition to lynch

syndrome.

“Hoag’s Hereditary cancer Program is such

an important life-saving resource. i now

feel empowered to protect myself and my

family and make sure they receive genetic

counseling and testing,” shares Patty.

While Patty’s greatest joy is her husband

and their three children, she enjoys

painting, a college hobby she picked up

again after her last cancer diagnosis.

Hereditary CancerPatty g oethals is a three-time cancer survivor.

the newport Beach resident and mother of three has survived melanoma skin cancer, uterine cancer

and most recently, breast cancer. Patty was concerned that based on her history of cancer and knowing

that breast cancer can be hereditary, her daughter may be at risk. thankfully, Patty’s breast cancer

surgeon referred her to the Hereditary cancer Program at Hoag.

during the genetic assessment of Patty’s family history, Hoag certified genetic counselor Jeanne

Homer, m.s., discovered that Patty’s mother had once battled uterine cancer and her brother had bile

duct cancer. Both of these cancers are linked to the hereditary cancer syndrome, lynch syndrome.

individuals with lynch syndrome have a 70 to 80 percent chance of developing colon cancer during

their lifetime, and are at greater risk for other cancers such as uterine, ovary, pancreas, renal and pelvis

cancer. “if Patty had not received genetic counseling prior to genetic testing, she probably would have

only received testing for hereditary breast cancer, and would have never discovered that she has

lynch syndrome, unrelated to breast cancer,” explains Homer.

Page 29: Who We Serve Who Serves - Hoag daud arrived at Hoag, the cath lab team including cardiologist carey ’Bryan, o m.d., took him immediately from the ed to the catheterization lab where

Angela Van Dorn’s Story“the most frightening thing a parent can hear

is ‘your baby’s heart just stopped beating.’ you

feel so helpless and there was nothing we could

do but trust the doctors and nurses when they

said, ‘everything will be ok’,” says angela. “We

are so grateful for the care and attention Hoag

nurses, doctors and the nicu team gave to

William and i during our time in the hospital. it

was a team effort to get through this and we’re

so thrilled William is doing so well today.”

Because of her and her son’s experience at Hoag,

angela, already a registered nurse on Hoag’s

pulmonary medicine unit, applied for a transfer

to Hoag’s nicu, where she currently works.

OB Hospitalist Program angela Van dorn was two trime sters into her pregnancy when

she felt something wasn’t right.

she came into Hoag’s emergency department after experiencing bleeding, and was admitted to the

antepartum unit. Her baby’s heart suddenly stopped beating, which meant an emergency c-section was

needed and thankfully one of Hoag’s board-certified oB hospitalists was already on the scene and ready to

respond with lifesaving intervention. thanks to their expert care, William Bradley Van dorn was delivered

successfully on march 9, 2009 at 28 weeks, almost three months premature and weighing two pounds.

William’s heart started on its own after he was born and he was immediately admitted to Hoag’s neonatal

intensive care unit (nicu). a huge blood clot was found in William’s umbilical cord, which had stopped his

heart. in approximately 99 percent of cases like William’s, the babies do not survive because the blood clots

are extremely hard to detect. William remained in Hoag’s nicu for 10.5 weeks, but thanks to Hoag staff expertise

and dedication to breastfeeding and developmental care, he was able to receive breast milk right away.

Page 30: Who We Serve Who Serves - Hoag daud arrived at Hoag, the cath lab team including cardiologist carey ’Bryan, o m.d., took him immediately from the ed to the catheterization lab where

Michael Blackman’s storyclosely affiliated with Hoag community

medicine, sos is one of the largest, volunteer-

based agencies providing safety net services

for homeless and low-income individuals and

families in orange county. michael did not have

insurance because the economy had affected

his full-time work laying tile and stone. “every-

one at sos was so caring and helpful and they

gave me all the tools i needed to get through this

experience,” shares michael. “i feel so blessed for

everyone at Hoag and sos that took care of me

and helped me get back on track.”

michael recently completed the cardiac rehabili-

tation program at Hoag and is working on his diet

and strength so he can get back to work soon.

Community Benefit michael Blackman never thought a heart attack could happen to him.

nearly 60 years old, the southern california native never anticipated that a long day of work would end at

Hoag’s emergency department fighting for his life. although michael did have a family history of heart dis-

ease – his father passed away at age 79 with heart disease – he didn’t recognize his symptoms. thankfully,

his brother was there to make sure he went to the ed after michael complained of dizziness and vomiting.

“at the ed, the physician told me ‘you’re having a heart attack’ and nurses kept telling me ‘help is on

the way, hang in there with us’.”

When michael came out of surgery, (he had several stents placed in blocked arteries to increase blood-flow

in his heart), he shares that he instantly felt better. He was also relieved when one of Hoag’s case managers

suggested he speak with share our selves (sos). sos, including medical director Patrick chen, m.d., helped

michael, who did not have insurance, with his discharge from Hoag, get the prescriptions he needed, rushed

his msi insurance application and found a primary care physician and cardiologist ray gandhi, m.d., for him.

Magnet Designation

Hoag’s nursing excellence is exemplified by our re-designation as a

magnet hospital.

american nurses credentialing center’s (ancc) magnet recognition Program® has once again designated

Hoag Hospital newport Beach as a magnet hospital. this designation has also been extended to Hoag

Hospital irvine since the commitment to nursing excellence is maintained at both hospitals.

the ancc magnet recognition Program® was developed to recognize health care organizations that provide

nursing excellence. magnet also provides a vehicle for communicating successful nursing practices and

strategies and is based on quality indicators and standards of nursing practices as defined in the newly

revised 3rd edition of the ana nursing administration: scope and standards of Practice (2009).

only six percent of all health care organizations in the united states have achieved magnet status. this

designation is an exciting achievement for Hoag and all nursing staff should be congratulated on their

outstanding commitment to quality care.

Magnet storyHoag’s mission as a not-for-profit faith-based

healthcare delivery network is to provide

the highest quality healthcare services

to the communities we serve, and this

is certainly represented by our nursing

staff. at Hoag, we highly value the patient

experience, and our nurses are one of the

most valuable resources we have to deliver

the care that patients and their families

need and have come to expect from Hoag.

A Continuum of Care:Hoag moves beyond hospital to health care network.

the opening of Hoag Hospital irvine represented the beginning of a new chapter in Hoag’s journey

to provide health care to a larger community. Hoag has grown beyond our coastal roots and

become more than a single hospital. With this shift, came a new look that reflects Hoag’s evolution.

the new look represents Hoag’s commitment to health and wellness for all orange county

residents. What will never change is Hoag’s promise to deliver the same world-class care and

patient-centered approach that has made us orange county’s highest ranked health care provider,

as shown through Hoag’s designation as the most preferred hospital by orange county residents

for the 15th consecutive year based on national research corporation (nrc) consumer study and

The Orange County Register’s reader’s poll.

“our new brand identity expresses Hoag’s vital connection with the entire community. it represents

the physical and emotional aspects of health and well being,” shares Hoag President and ceo richard

afable, m.d. “We have organized ourselves around the rich history of our familial name ‘Hoag’ – a

name that represents the extraordinary health care that this community has become accustomed

to and which will continue to evolve as we continue to grow as a world-class health care provider.”

Hoag is fast becoming the most trusted regional health care partner for orange county residents

with two acute-care hospitals, seven health care centers and a network of more than 1,400

physicians, 5,000 employees and 2,000 volunteers – all committed to providing the finest health

care services available. Hoag’s new brand is a contemporary representation of a familiar and

trusted partner in health. and while we have a fresh look, our mission remains the same – to

provide the highest quality health care services to the communitie s we serve.

inpatient and outpatient services, including: fully staffed emergency room, radiology/imaging,

laboratory, cardiology, neuro-diagnostic services and more.

from larger operating rooms featuring the latest technology and the most advanced imaging

equipment available, to enhanced aesthetics and interior design elements, the facility is retooled

to meet the needs of the community. equally impressive, the expansive $84 million renovation

effort was completed ahead of schedule and on budget and also created 900 new jobs in

orange county.

irvine and Hoag Hospital newport Beach have always shared a common bond. even before the

former irvine regional Hospital and medical center closed its doors in 2009, 24 percent of irvine

residents went to Hoag Hospital in newport Beach for care. in fact, the opening of the refurbished

154-bed hospital represents the culmination of a long-term effort by Hoag to deliver convenient

access to 21st century health care services to irvine and south county residents.

“We’ve been focused on our goal to deliver the hospital irvine deserves and we’re excited that

everyone can now see the results of our planning and hard work – a world-class health care facility

that will benefit the community for years,” says robert Braithwaite, Hoag senior Vice President and

chief operating officer. “now, we are doing what we do best, which is provide high-quality care to

the community.”

Personalized Orthopedic Carein november 2010, a unique partnership between Hoag and many

of orange county’s most respected orthopedic surgeons culminated

in the grand opening of Hoag orthopedic institute.

a state-of-the-art orthopedic hospital located within Hoag Hospital irvine, Hoag orthopedic

institute is poised to be one of the highest volume single-site orthopedic inpatient hospitals in the

Western united states. Hoag orthopedic institute focuses primarily on orthopedic reconstructive

surgery for adults, including: joint replacement (hip, knee and shoulder replacement), spine surgery,

trauma-related surgery that requires inpatient care and also outpatient services.

“Hoag orthopedic institute is a hybrid institution combining the best of the private practice world

with the best of a university research and education program,” shares James caillouette, m.d.,

surgeon-in-chief and chief-of-staff for Hoag orthopedic institute.

Hoag orthopedic institute is also setting the standard for superior care with personal clinical

nurse navigators for both inpatient and outpatient service s. from pre-hospitalization education,

scheduling and registration, to pre-operative evaluation and lab work, orthopedic-specific

clinical nurse navigators (like tanya sapze sian r.n. , pictured) guide patients through their

entire health care proce ss.

“clinical nurse navigators are wonderful re source s for patients and their familie s because their

assistance better prepare s patients for surgery which ultimately leads to better outcome s,” says

kanoe allen, msn, r.n. , ocn, executive director of nursing, Hoag orthopedic institute. clinical

nurse navigators are in regular contact with their patients and their families from the moment

they are scheduled for surgery – escorting them through each step of the process – then remaining

in contact with them, and their primary care physician for up to two years post surgery.

Hoag orthopedic institute, which include s a single specialty hospital, two advanced ambulatory

surgery centers and other support service s, allows the facility, including its physicians, staff,

and allied health profe ssionals, to focus on only one topic – providing the be st orthopedic care.

Orange County Vital Aging Program launches

california alzheimer’s disease data report published in 2009

estimated that in orange county, approximately 46,000 people

had alzheimer’s disease in 2008.

this number is expected to grow by 99 percent by 2030. Here’s the good news: methods of prevention

and treatment have been extensively studied in recent years and the knowledge we gain could

delay or even forestall the progression of alzheimer’s disease. early assessment may reassure

people of healthy aging or detect a problem in its earliest stages.

to help combat the community’s fear of alzheimer’s disease and related disorders (adrd), Hoag

neurosciences institute has started a comprehensive approach to these problems through

the orange county Vital aging Program* (ocVaP) launched in July 2010, under the direction of

William r. shankle, m.s. , m.d., program director, memory & cognitive disorders, Hoag

neurosciences institute (pictured left).

“the design of the program encourages the community, primary care physicians, and specialists

to work together to proactively address this growing concern, and to help counter the assumptions

that nothing can be done,” states michael Brant-Zawadzki, m.d., executive medical director, Hoag

neurosciences institute. “the ocVaP is being looked at by national leaders in the field as a

prototype of what a community-based adrd prevention program might look like.”

the self-assessment tools available on the program’s website allow users to gain education and

preliminarily assess their brain health by checking their memory, mood and risk factors of adrd.

the ocVaP encourages and offers more formal assessment and monitoring of memory in-person

either through participating physicians, or at Hoag Hospital newport Beach as a useful way to help

prevent and delay the onset of adrd. When seen at Hoag, participants will receive a free personalized

report that may reassure them, or suggest the need for further testing.

*note: the ocVaP is supported by a grant to Hoag neurosciences institute from uniHealth foundation, a non-profit philanthropic organization whose mission is to

support and facilitate activities that significantly improve the health and well-being of individuals and communities within its service area.

struggling with all types of neurological disorders

is important to the Voltmers. they directed

part of their gift to treat sleep disorders in the

community because of the research linking sleep

disorders to diabetes and many other conditions.

the Voltmer sleep center provides individualized

treatment programs for many sleep disorders.

it was also built using sustainable practices and

was constructed to reduce environmental impact

approximately one-third of all americans develop sleep disorders at some point in their lives, and the

need for treatment has continued to increase in orange county. to meet the increasing demand, Hoag’s

sleep center moved to a state-of-the-art facility with a beautiful, hotel-like atmosphere. the new and

improved center reopened its doors in august 2010 as the Judy & richard Voltmer sleep center, located

at 510 superior avenue, suite 280, within Hoag Health center-newport Beach.

the Voltmer sleep center is custom built with eight private bedrooms to conduct sleep studies.

each room has a private bathroom and is designed to feel like a high-class hotel with a comfortable,

soothing and sleep-conducive environment. in addition to the upscale design, the diagnostic

equipment has been completely upgraded to state-of-the-art technology from care fusion, which meets

and exceeds the american academy of sleep medicine guidelines. the technology upgrade includes a

database system that tracks patients and interfaces with the hospital information system to streamline

follow-up and aftercare, which provides an improved continuum of care.
