Benefit Amount: 500% of the initial benefit amount elected
Health Screening Benefit reimbursement: $50 per year per covered person
You can choose coverage for yourself and eligible family members:

• Increments of $ 5,000 Spouse and dependent coverage is 50% of the employee benefit, in increments of $5,000
