Outreach• HFSS believes that each person has the right to access the local and wider community regardless of physical or

mental health status. However some of the people we support often feel marginalised by the wider society.• The Skills Centre intends to develop participation and confidence in accessing the community through the

following ways:• Employment• Recreation• Education• Social activities• The skills centre will provide opportunities to people by using their own choices, goals and aspirations as a way

forward to develop skills.• Where it is felt that an individual cannot make an initial decision about what support would be most

appropriate to meet their needs a full assessment will be undertaken that will attempt to give an informed choice towards a way forward.

• All community inclusion will be evaluated using the life skills awards, which will give a clear process to review achievements; this will also include achievements such as money handling, use of public transport and access to the local library etc.

• Once the initial assessment has taken place, a support package will be designed to enable the individual to be as independent as possible by developing their life skills in a supportive environment. Life Coaches will provide shared support, 1:1 support or 2:1 support to enable the individual to develop their skills to live a fulfilled life.
