Placement F1 - Cardiology

Activities also include clerking in elective admissions and seeing day-case patients on the medical investigation unit. Working in cardiology also means that the FY1 is placed on the on call rota which involves helping your senior colleagues to clerk in the acute medical admissions. You are also required to come in for the post-take ward round the following morning and present any patients you clerked in on the preceding day.

Placement F1 – Liaison Psychiatry

The department Liaison Psychiatry – Poole Hospital Rotation: Poole Hospital Foundation Programme The Liaison Psychiatry Service at Poole Hospital provides a psychiatric service to inpatients and attenders at the Emergency Department. It is part of a wider team which includes the Liaison Psychiatry Service at Royal Bournemouth and Dorset County Hospitals.

The type of work to expect and learning opportunities

The F1 doctor will be part of the Liaison Psychiatry Service and will be based at Poole. The service is for people between 18 years and the end of life. We receive approximately 900 referrals per year, reflecting the type of psychiatric conditions frequently seen by doctors not working in psychiatric settings. Commonly seen referrals include admission following self-harm, depression and anxiety complicating medical illness, medically unexplained symptoms and somatoform disorders, poor compliance with medical treatment and alcohol misuse. We also see patients with organic psychiatric disorders and where capacity for treatment is being questioned. This post will enable the post holder to gain skills, knowledge and attitudes needed to assess and manage common psychiatric presentations seen in non-psychiatric settings. There will also be experience in the crisis management and risk assessment of patients who self-harm. Also in the application of the Mental Health Act and the Mental Capacity Act. Learning opportunities:

To acquire skills in assessing people following an episode of self-harm, including risk assessment, and in planning their short term management

To become familiar with psychiatric/psychological presentations seen within a general hospital setting.

To acquire skills required to detect and diagnose depression in the medically ill.

Main duties of the placement

The post-holder will undertake assessments of patients following self-harm and those referred with a range of psychiatric/psychological problems from the general hospital wards. All work will be closely supervised by the Consultant and other members of the clinical team as appropriate, including at daily team meetings where all patients seen that day are discussed. Secretarial support will be provided.

Typical working pattern in this placement

The service operates Monday – Friday 0900 – 1700. A member of the team attends the Emergency Department and the acute admissions ward every morning and collects referrals. There is then a team meeting for approximately 30 minutes in which the work for the day is allotted. The team also responds to referrals as they come in, so that the intensity of work is unpredictable. We will ensure that during the course of the placement there will be time to undertake non-clinical work such as audit for the interested trainee. The trainee will be able to attend any academic events as required by the foundation rotation. The department Eating Disorders Service / Kimmeridge Court, St Ann’s Hospital The Eating Disorders Service offers a Community, Intensive Day Service and Inpatient Care to Adults suffering from Eating Disorders. The post holder’s responsibility would primarily be to manage patients on the 6 bedded Inpatient Unit.

The post holder will divide their time between Kimmeridge Court and Florence House. Kimmeridge Court Kimmeridge Court runs on 100% bed occupancy and accepts

referrals from the whole of the United Kingdom. All admissions are planned. Most patients will be suffering from a severe Eating Disorder associated frequently with significant psychiatric and physical co-morbidity. The post holder will be responsible for clerking admissions and managing the patient’s physical and mental health, with the support of a Core Trainee. Close liaison with the General Hospital is an important aspect of this work, should the patient require acute medical treatment. The post holder will also have the opportunity of attending the Consultant’s Outpatient Clinic and Mental Health Act assessments and tribunals. Learning Opportunities:-

1. To acquire skills in assessing and managing patients with Eating Disorders.

2. To acquire skills to detect and diagnosis common psychiatric co-morbidity in this group of patients.

A normal day of work comprises care of your ward patients, assisting in theatre and assessing patients pre-op. There will be days when you are on call and take referrals from A+E and GPs. This is the greatest opportunity for learning as you are the first doctor to assess the patient (ages ranging 5 years and older) and initiate a

management plan before senior review. If medical students are attached to surgery this is a good opportunity to teach by discussion around history, examination and management plan.

Main duties of the placement

Daily ward rounds with seniors and care for patients on the ward. Assist in theatres if needed. Manage on-calls and initiate management plans before senior input. Attend scheduled teaching sessions. Cover general surgery and ENT on call during night’s shifts.

Typical working pattern in this placement

Normal day - 08:00-17:00 ward work

On call day - 08:00-21:00 ward work and on call for acute admissions

Nights 7 days - 20:30-09:00 general surgery and ENT on call and ward cover.

2 scheduled teaching sessions per week.

The type of work to expect and learning opportunities

When based on a ward the FY1 will take responsibility for the daily medical management of ward based patients with supervision from a SHO. This will include optimising patients for surgery. Whilst on-call there will be many opportunities to clerk orthopaedic presentations and consent for the appropriate operation with supervision from the SHO. Learning Opportunities: Management of orthopaedic presentations Consent for basic orthopaedic operations Radiology of musculoskeletal injuries Medical management of pre and post-operative patients Assistance in theatre

B3, TAU, ED, Ansty (MAU), Paediatrics and other outlying wards at Poole Hospital

Main duties of the placement

The FY1 role is split between being on-call and post-take for one week and ward based the next week. Whilst on-call the main responsibility is to aid the SHO clerking, consenting and marking patients that are to be admitted. Ensuring that the patient is optimised for any surgery that may be required, together with their ongoing medical management. Post-take responsibilities are to attend the daily trauma meeting and complete the post-take ward round with a SHO and a consultant to ensure their management plan for the previous day/night admitted patients is completed. Together with assisting in theatre as required. Whilst on the wards for a week main responsibilities are to ensure completion of the surgeon’s post-operative plan. As well as continued medical management of patients and the optimisation of patients who are to have surgery. Attend daily trauma meetings. Also to aid in timely discharge of patients.
