ELIGIBILITY

Medi-Cal eligible children

Under 21 years of age

Fee-for-service (FFS) Medi-Cal or belong to Medi-Cal Managed Care Plan

Income eligible children

Under 19 years of age

<200% Federal Income Guidelines (FIG)

No health care coverage for well-child exams

Must reside in California

CHDP GATEWAY

Process to enroll uninsured children into comprehensive health care services (Medi-Cal or Healthy Families)

Pre-enrolled into full-scope, FFS Medi-Cal at the time of the CHDP visit for up to 2 months

Parent/legal guardian can request Medi-Cal/Healthy Families application

Automatic enrollment of eligible infants under one year of age into Medi-Cal

Eligibility Criteria

Children and Youth under 19 years of age

Family income is at or below 200% of FIG

Must reside in the state of California

Includes children and youths eligible for

Limited scope Medi-Cal

Medi-Cal with Share of Cost

Infants under one year of age are automatically enrolled into Medi-Cal if mother had Medi-Cal at the time of delivery

Periodicity Schedule

Health exams are based on the following schedule:

1, 2, 4, 6, 9, 12, 15, 18 months

2, 3, and 4-5 years

6-8, 9-12, 13-16, and 17-20 years

Need to perform required screening component that wasn’t completed during the initial physical exam
