Position Title: Community Health Representative Permanent Position Position Description: Fort William First Nation (FWFN) requires a CHR to be responsible for the

support, education and advocacy of health and health care within the community. The CHR will possess strong knowledge of various health prevention and treatment practices including, but not limited to, traditional and herbal approaches.

Major Responsibilities: • Participate in the research, development and maintenance of community health policies. • Provide health care and prevention education within the community including schools, community

centers and patient homes. • Liaise between patients, families and the community to ensure patients are informed of their

condition(s) and prescribed treatment(s), and are receiving quality care. • Liaise with health care providers in an effort to remain current with health issues. • Perform health care treatments as necessary and within capacity. • Perform frequent and accurate patient evaluations and respond appropriately. • Coordinate care with clinics, outside health care providers and hospitals. • Educate and guide patients in how to perform tasks they can do themselves. • Other duties, relevant to the position, shall be assigned as required. Qualifications: • CPR Certification required. • First Aid Certification required. • Post secondary diploma in health care required. • Secondary School Diploma required. • Criminal Records Check. • This position will be subject to pre-employment dug testing. Fort William First Nation is looking to hire an experienced MicrosoftSystems Administrator to manage and maintain its main office andbranch office network topology.

• Candidate must have minimum 5 years’ experience managingMicrosoft Active Directory, Exchange and SQL as well as possess recent MCSE, CCNA and A+ certification.

• The candidate will report on site once a week for a 7 hour period where they will address the issues affecting the computer network.Candidate must have experience in troubleshooting end userhardware and software issues.

• Other responsibilities will include server maintenance on theoffice’s business systems and servers, software updates, firmwareupdates, data backup testing, as well as antivirus and network security audits.

• Will be responsible for developing an organizational asset managementsystem and upgrade strategy for the ongoing replacement cycle of workstations and servers.

• Will develop and implement additional secure remote office connectivityvia wireless site to site technology as well as review and update remote access security procedures and policy.

• Required to be on call Monday to Friday 9:00 a.m.-5:00 p.m. for emergencies.

William First Nation (FWFN). Major Responsibilities: • Act as the point of contact and liaise between funders, partners, and various other community and

business organizations as well as the general public. • Provide coordination for all aspects of the planning, implementation, and evaluation of special

projects and programs. • Participate and collaborate with other offices. • Build and maintain positive relationships and contact with federal, provincial, and municipal

departments as well as personnel, public officials, public and separate school representatives, the general public and community and business leaders.

• Prepare and present annual summary reports of activities and results. • Coordinate meetings, conferences and public speaking engagements in an effort to educate the

community and raise awareness. • Work in coordination, via a series of meetings, with the Director of Economic Development, Economic

Development Manager, and FWFN Chief and Council as a means of consultation with FWFN. • Prepare business letters, newsletters, band council resolutions, work plans, budgets,

recommendations, applications and other related paperwork. • Other duties shall be assigned as required. Qualifications: • Minimum (3) years’ experience OR a Secondary School Diploma in a business related field, that would

JOB POSTING Position Title: Family Support Worker – Permanent Position Description: Under the general direction of the FWFN Chief and Council, with direct supervision

provided by the Health Services Manager, the Family Support Worker (FSW) gathers information on and assesses family functioning problems.

Responsibilities: • Gathers information relevant to the client’s problems, needs and risks by interviewing, observing behavior, meeting

with caregivers and service providers and using a variety of inventories, checklists and questionnaires. Assesses the information gathered to identify client problems, needs and risks. Develops and implements an intervention plan within program guidelines.

• Participates in the development, modification and evaluation of client service plans with the integrated case management team. Participates in integrated case management meetings on a regular basis to report clients’ activities and progress.

• Outlines services provided by the program and/or organization. Provides information on and referral to other community service providers, resources and professionals as required.

• Provides parenting skill building to clients on issues such as parent-child interaction, child development, discipline and guidance, and behavior management.

• Performs other related duties as required. • A full job description is available upon request. Qualifications: • A degree or diploma in the Social Service and Human Behaviour or related field and/or a minimum of one

year direct service experience or an equivalent combination of education and experience.
