PURPOSE OF POST: The purpose of this role is to work within the Integrated Teams that consist of staff from both Monmouthshire County Council and Aneurin Bevan Health Board. The role has been created to bridge Integrated Service support and Community Mental Health support to ensure that people in Monmouthshire are able to access the right person, at the right time, to receive the right support to enable them to continue to live their own life. We have two fulltime posts based within the Integrated Services that consist of both health and social care staff covering Chepstow and Abergavenny, and we believe this model of building services around the person delivers the best outcomes for people, organisations and staff. We are looking for enthusiastic and committed individuals to become part of one of these teams. Expectation and Outcomes of this Role:- We have two posts based within the Integrated Services that consist of both health and social care staff covering Chepstow and Abergavenny, and we believe this model of building services around the person delivers the best outcomes for people, organisations and staff. We are looking for enthusiastic and committed individuals to become part of one of these teams. We have made some real progress in Integrated Community Services in transforming the way we work, and are implementing the new Social Services and Well-being Act (Wales) 2014. We are looking for a dynamic individual who will work closely with the person and direct care staff to help the person achieve their identified personal outcomes and feel part of their local community. To support this way of working the successful person will need to be able to make connections across the Integrated Services, Direct Care and Older Adult Mental Health, and also local resources such as voluntary sector, community activities. The person will also be required to work with the Reablement team, based within the Integrated Services and support individuals achieve their goals. We need you to have a good understanding of the Social Services and WellBeing Act (Wales) 2014, and preventative support, be able to communicate effectively, both written and verbal, not only with Integrated Team, Direct Care and Older Adult Mental Health staff but also with the voluntary sector, private providers, and other agencies that are able to help a person realise their potential and outcomes, and to build links with these organisations. We need you to be a creative thinker and it is important that your values are driven from the need to ensure people are able to live their own lives and you are able to facilitate this through our care planning and working alongside the person.
