I understand that any misrepresentation of fact may result in Wags to Whiskers refusing adoption privileges to me. I authorize Wags to Whiskers of Texas, Inc. to contact all veterinarians listed on the application and the landlord if applicable. If my application is approved and later Wags to Whiskers discovers the above information is not true or correct, Wags to Whiskers reserves the right to remove the adopted animal from my home. I realize that permission to adopt comes completely at the discretion of Wags to Whiskers of Texas and that a post adoption home visit may be conducted if required at any time, for up to a year. If the animal is found to be in distress or not properly cared for it will be removed and become the property of Wags to Whiskers of Texas, Inc.

Adoption Contract

_, between Wags to Whiskers of Texas, Inc., a non-profit animal rescue, and This contract is made on

the adopter. By signing below the adopter acknowledges receipt from Wags to

Whiskers of Texas, Inc., a cat/dog described as # , a M/F _. In addition,

I acknowledge that I am 18 years or older and agree to be bound by and to comply with the following terms and conditions: PLEASE READ AND INITIAL EACH LINE

I agree that the animal is to be kept solely as a pet and provide proper and sufficient food, water, shelter and kind

treatment for the animal at all times. I agree to have kitten(s) vaccinated for rabies at 4 months of age at my expense

I will have the animal immunized and examined annually by a veterinarian at my expense.

I will not sell, abandon or allow the animal to be used for purposes of vivisection, experimentation, dog fighting or any

criminal activity that abuses, neglects or injures this animal.

I will not permit the animal to run at large or to become a public nuisance. I will keep a means of identification on the

animal at all times, and will immediately retrieve the animal from any public or private shelter when notified that the animal

is being held at any such location. In the event the animal becomes lost, I will make every reasonable effort and attempt

to locate and claim the animal. Microchipping is mandatory if not already done.

I agree that this animal will not be declawed. Wags to Whiskers of Texas, Inc. will impose a $5000 fine if at any time

during the lifetime of the cat it is declawed. Wags to Whiskers of Texas, Inc. will take the guardian to court to collect such

remedy. Wags to Whiskers of Texas, Inc. will also remove the cat from the home if it is felt that it is in the best interest of

the cat.

I am aware that Wags to Whiskers of Texas, Inc. reserves the right to have a home visit before and/or after adoption, to

assure that the animal is in a happy, safe environment. I am also aware that Wags to Whiskers of Texas, Inc. has the

right to remove this animal at any time that they are made aware of irresponsible treatment of this animal.

I agree to contact Wags to Whiskers of Texas, Inc. within one week of the adoption with a progress report on how the

animal is adapting. I also understand that Wags to Whiskers of Texas, Inc. is always available for answering questions or

concerns about my new pet.

If I return the animal to Wags to Whiskers of Texas, Inc. for any reason, I will at no time assert any claim, charge or

demand of any kind or nature against Wags to Whiskers of Texas, Inc. for any charges which may have been incurred by

me, including veterinarian fees, in connection with the animal. Wags to Whiskers of Texas, Inc. will accept back any

animal adopted from them at any time. No animal from Wags to Whiskers of Texas, Inc. will ever be turned in to a shelter.

Any and all adoption fees are not refundable. I understand that the information provided to me about the animal that I am adopting may have been received by Wags to

Whiskers of Texas, Inc. from third parties and that Wags to Whiskers of Texas, Inc. does not warrant the accuracy or

correctness of such information, including sex determination.

I understand that Wags to Whiskers of Texas, Inc. receives animals from various locations, and from various situations,

and I also understand that these animals come in varying degrees of health, and that Wags to Whisker s of Texas, Inc.

cannot and does not guarantee the health of this animal.

I understand that animals can be unpredictable and Wags to Whiskers of Texas, Inc. cannot anticipate or insure against

unexpected conduct of animals adopted from Wags to Whiskers of Texas, Inc. I acknowledge that Wags to Whiskers of Texas, Inc .

has not made through its agents, volunteers, or employees, any warranties regarding the future condition, temperament, or conduct of

this animal. I hereby accept the animal as is, assume all risks and responsibilities associated with ownership of the ani mal, including

bites and scratches, and I hereby fully and completely release indemnify and hold harmless Wags to Whiskers of Texas, Inc., its

directors, officers, volunteers, agents, servants and employees from any claim, cause of action or liability of any sort or nature, whether

known or unknown, directly arising out of our connection with the adoption, care, or ownership,
