Dear Friends,

Warm greetings! This is my last editorial as Dr Kapil Zirpe, the incoming President-Elect will take over from the next issue. I had a pleasant 2 years

managing this job and I thank all my colleagues on the editorial board for their help. I am also grateful to all the contributors for sharing the information about the conferences oragnised by them and the authors of Journal Scan. Last few days are left for the upcoming Agra Conference, CRITICARE 2016 to be jointly held with the International Sepsis Forum. A lot to look forward to! By the time you get this issue the new Vice Chancellor and board members will have been elected as we will have elections between 15th-22nd January 2016. These elections will also be held electronically using the two-step system we used last year for the Executive Committee elections. As always I request you once again, to update your e-mail and mobile nos. so that you can vote in the next election and we can communicate with you electronically. This issue is the second special conference issue. Like last year’s issue you will find all that you need to know about the Agra conference: the venue, the organizers, the orators delivering the oration and past president’s oration, what to eat and where to go. The ISCCM is developing its own mobile app and you will see glimpses of the app in this issue. Dr Amin reminisces about the second CRITICARE held in 1995. In the end I wish everybody a happy new year!

Greetings and Warm Wishes to all

ISCCM members & Criticare

2016/ISF Delegates!

At Agra I hand over reins to Dr. Atul

Kulkarni heralding the beginning of a new era in the history of ISCCM. He will be the first ISCCM president to have a one year term. From my experience over the

last two years I know how difficult it is for the incoming president to get into the

groove and implement new ideas – it takes at least six months. With a one year term

half your time is already gone. Fortunately

we have enjoyed an excellent rapport and he has been my “Sakha” offering me unconditional support in the last two years in all my ventures.

ISCCM faces several new challenges in the years to come and all of us need to put our

best foot forward and help our incoming president in scaling new heights for ISCCM.

iSccM as an organization

Currently ISCCM has more than 8000

members and more than 80 branches all

over the country. The structure of the

organization and it’s working has changed in the last few years but a lot more needs to be done. Streamlining the processes and

working of the ISCCM office and college should be a top priority.

liaison with international organizations

We have established agreements with SCCM, ESICM, ANZICS and the

Neurocritical Care Society USA. We are also

members of international organizations like APACCM, ASAARCS and WFSICCM. We

now need to keep alive these collaborations and actively seek opportunities in research and education with these organizations.

the iSccM National conference

“CRITICARE” the ISCCM national conference is our flagship event and draws around 2500 delegates every year from

all corners of India. We also attract many

delegates from abroad mainly SAARC

countries and the Middle East. CRITICARE

2016 Agra is a joint meeting with the International Sepsis Forum and the latest

developments in sepsis will be discussed. The Agra organizing committee especially

Dr. Ranvir Tyagi, Dr. Rakesh Tyagi and Dr. Diptimala Agarwal have worked very hard to ensure a good conference. I am sure all

of you will enjoy the pleasant weather and the sites to see in Agra.

We need to rethink the way we do our national conference in order to improve

it further. The president is currently the

scientific chair of the national conference and I am happy to say that this arrangement

is working well, but it can certainly be improved. At present there is undue

strain on the local organizing committee

especially with regard to finances and how to attract delegates for both the workshops and the main conference. The ISCCM EC

needs to get more involved in all aspects of

the conference and work in tandem with the local organizing committee.

Workshops and Short courses

The ISCCM 4c course has progressed well under Dr. Chawla’s able stewardship and the 2nd edition will be launched at Agra. Our focus in the next few years should be to try and standardize all our workshops with uniform content and course manuals. The

conference organizing committee, the EC

and the college board need to work together for this.

iccM and education

The ISCCM college is now well established, the first 2 chancellors and vice chancellors have done a commendable job in streamlining the IDCCM and IFCCM

and nurturing the post MBBS certificate and nursing IDCCN. The challenges before

us are

1. Completely shifting to the online

website and streamlining the various processes involved in the working of the college

2. Improving the educational support from

educational modules in addition to the

current webinars

3. Getting the teachers to improve

teaching at the bedside and locally and

conduct competency based in-training

4. Improving the written and practical examination processes

From this year the president will also be the chancellor of the college further

This is the area we are the weakest. We have already done some epidemiological

studies (INDICAPS for example) and are

on the threshold of launching our very own research database and App “CHITRA”. The next steps are most important

1. Getting all our teaching institutes to

adopt CHITRA for data collection in

their units

2. Involving compulsory collection by our

IDCCM and other students

3. Providing support for data collection

locally

4. Providing electronic/online support for

using the CHITRA app

5. Providing timely reports and data

analysis for all individual units ensuring

confidentiality of data.

6. Generating an India specific severity of illness score

7. Doing further studies on the basis of this

CHITRA data base

Dr. Sriram Sampath has worked tirelessly on this project and has brought it to the point where it can be launched for further use by all our units/members. WE have planned a

one hour session for potential users during

CRITICARE 2016.

Like I said at the beginning of my message, I hand over reins to Dr. Atul Kulkarni and the new EC at Agra. I would like to once again thank all my ISCCM colleagues, members of the EC and college board and

all my friends for having supported me so

wholeheartedly in all my endeavours.

May ISCCM under Dr. Atul Kulkarni's guidance succeed in further promoting care

Padam Bhushan Awardee, guide and mentor to many of today’s intensivists, will deliver the ISCCM oration during the morning session of the 5th Feb 2016 at 9:50 -10:20 am on

first day of the conference.Dr. Brijendra Kumar Rao, born on 24th March 1954, is an alumnus of Maulana Azad Medical College, Delhi from where he did graduation in 1975 and post-graduation in Anaesthesiology in 1980.Dr. B.K. Rao started his professional career in 1983 as a Consultant Anaesthesiologist at Sir Ganga Ram Hospital, New Delhi. Right from the early years of his practice, Dr. Rao had special interest in Intensive Care Medicine. He had the vision to forsee critical care medicine emerging as a separate specialty and a key to high end care in all other disciplines. ICU’s in those years were manned by consultants from a single specialty. Deviating from this, Dr. Rao set up a Department of Critical care medicine in 1996 which was manned by full time multispeciality team of intensivists. Today, this has become the standard pattern in Intensive care units across the country. Dr Rao has been teaching critical care for over three decades to the residents of various specialities posted in the ICU. Dr. Rao is currently the chairman, Department of Critical care and Emergency Medicine, Sir Ganga Ram Hospital. He is a recognised teacher in Critical care medicine for the DNB and ISCCM diploma and fellowship programs, post graduate thesis co- guide and researcher.Dr. Rao served as the Chairman, Board of Management Sir Ganga Ram Hospital from 2006 to 2011. Dr. Rao was Post Graduate Committee and Ethics Committee member and advisor to the President, Medical Council of India. In 2013, Dr. Rao was nominated as a Member, Board of Governors, Medical Council of India by the Ministry of Health and Welfare, Government of India.Dr. Rao has been the Organizing Chairman and Coordinator of various national and international conferences & workshops which include “TraumaCare 2010” - 23rd international conference of International Trauma Care (ITC), “Trauma Criticare 2003,7th national conference of ITACCS (Indian Chapter)”, “Critical Care-Trauma” Chapter for the 51st & 52nd Annual Conference of Indian Society of Anaesthesiologists, “36th National Trauma Management Course” in association with Academy of Traumatology (India), International Conference on “A Healthcare Initiative - Public-Private Partnership Model” in collaboration with Ministry of Health & Family Welfare and Associated Chambers of Commerce & Industry of India (ASSOCHAM).Dr. Rao has participated in over 60 state and national conferences as invited faculty, speaker, guest speaker and keynote speaker. He has been an ‘Invited Faculty’ in many international conferences including American Association of Respiratory Care conference 1993, Tennessee, USA; ‘Trauma Care 2004’ international conference at Sydney, Australia; ‘Trauma Care 2007’ at Las Vegas, USA; ‘Trauma Care 2008’ at Yokohoma, Japan; and Oman Society of Anaesthesia & Critical Care conference 2009, Muscat, Oman.

Dr. Rajesh Chawla, MD, FCCM, is a well-known name in the Indian Pulmonary Medicine &Critical CareCommunity. He is currently the head of the department of Respiratory,

Critical Care and Sleep Medicine at Indraprastha Apollo Hospitals, New Delhi.He had been the past president of the Indian Society of CriticalCare Medicine and at present he is a Chancellor of Indian College of Critical CareMedicine.He has done some path breaking contribution to the Indian Society of Critical Care Medicine (ISCCM). He Conceptualized and started Critical Care Communication (bimonthly News Bulletin). He was the Editor for the same for five years. He was instrumental in starting Electronic election at ISCCM. He conceptualized & started Indian College of Critical Care Medicine. He was Vice Chancellor (2012-2014) and currently is the Chancellor of the College. He stared Comprehensive Critical Care Course (4C) and saw to it that this is successful. He has just launched an online Critical Care Infectious Diseases Course with the help of eminent Infectious

Dr. Rao has over 30 publications including journal articles, research papers and book chapters. He is a co-guide to many DNB theses from the department of Medicine and Pulmonology.Dr. Rao has held honorary positions in various professional bodies. He has been a Member - Editorial Board of Journal of ITACCS (Indian Chapter), Corresponding Editor- Asian Archives of Anaesthesiology and Resuscitation, North India Coordinator - International Trauma Anaesthesia & Critical Care Society (Indian Chapter),Ex-President - International Trauma Anaesthesia & Critical Care Society (Indian Chapter).Dr. Rao is a member of Society for Critical Care Medicine, European Society of Intensive Care Medicine, International Society for Study of Pain (Indian Chapter), Indian Society of Anaesthesiologists, Indian Society of Critical Care Medicine, International Trauma Anaesthesia & Critical Care Society (Indian Chapter), Research Society of Anaesthesia & Clinical Pharmacology.Dr. Rao is associated with various government, corporate and industrial bodies.He is a Board Member, National Accreditation Board for Hospitals and Health Care Providers (NABH); an external expert, Technical advisory committee for purchase of medical equipment, Govt, of NCT of Delhi; and Chairman - Health Committee, Associated Chambers of Commerces Industry of India (ASSOCHAM).Dr. Rao was awarded International Fellowship by American Respiratory Care Foundation and American Association for Respiratory Care, Dallas, USA in 1993 in recognition of the global efforts to promote the respiratory care practices. He has been felicitated at “Community Thanksgiving Ceremony” by Associated Chambers of Commerce & Industry of India (ASSOCHAM) in August 2006. He was bestowed with ‘Distinguished Service Award’ by Delhi Medical Association on Doctor’s Day (1st July 2006) in recognition of outstanding contributions to the medical profession and society at large,and “Indira Gandhi Excellence Award” at GPCON2006 held at Maulana Azad Medical College in November 2006. In recognition of contributions towards Anaesthesia and Critical Care, the Indian Society of Anaesthesiologists (Delhi Chapter) honored him with “Anaesthesia Award” in July 2007. Dr. Rao was felicitated with the “Achiever’s Award” on the Golden Jubilee of Maulana Azad Medical College on 26th February 2008.In recognition of his contribution to the field of medicine, Dr. Rao was awarded the third highest national award “Padma Bhushan”by His Excellency, the President of India, on 14th April 2009.

Abstract Title

Can cardiac dysfunction induced during weaning predict extubation success ?

Presenting Author : Amarja Ashok Havaldar, Bangalore

Introduction : Cardiac dysfunction induced by weaning is known. It can happen without underlying heart disease. Dysfunction is mainly caused by change in intrathoracic pressure from positive to negative leading to increased systemic vascular resistance, preload and afterload. It can be assessed noninvasively by echocardiography. Previous trials were done mainly in particular sets of patients such as COPD, without underlying heart disease, without arrhythmia. Aim of our study was to assess whether cardiac dysfunction induced during weaning by pressure support, predicts extubation

success. Inclusion criterias of our study were more pragmatic.

Methods : It was prospective observational study.After approval from institutional ethical committee and informed consent, patients >18 yrs of age, planned for extubation as per decision of treating physician, were included (n=100). Tracheostomised and pregnant patients were excluded. Primary outcome was extubation success or failure. Secondary outcome was ICU length of stay and ICU mortality.

Baseline characteristics, weaning and echocardiography parameters were collected pre and post extubation. LV systolic dysfunction was assessed by visual gestalt method. LV diastolic dysfunction was assessed by transmitral flow velocity and mitral annular flow velocity. TAPSE and IVC diameter was measured. E/A,E/e’ were calculated.

Results : Statistical analysis was done using STATA12. Baseline characteristics were comparable. Pre E/e’ was 7.92 vs 8.23 (P=0.6822) in extubation success and failure groups respectively.

Patients with moderate to severe LV dysfunction had higher E/e’ during weaning (7.36 vs 9.13P=0.0038).ICU length of stay and ICU mortality was higher in extubation failure group. Post extubation E/e’ was higher in extubation failure group (7.87 vs 9.38 P=0.0302)

Conclusion : E/e’ during weaning trial did not predict extubation success. Cardiac dysfunction induced during weaning may get masked during weaning by PS-PEEP and manifest post extubation. Patients with high E/e’ post extubation and with moderate to severe LV dysfunction may benefit from prophylactic non invasive ventilation.

The ISCCM national conference has evolved over the last 22 years into a highly respected annual event not only in the country but also in Asia and the rest of the world. We have

held joint meetings with the APACCM and this time we have a joint meeting with the International Sepsis Forum (ISF).

Workshops

This time we have taken a step forward in attempting standardized content and content for some of our workshops namely the mechanical ventilation, hemodynamic monitoring, ultrasound and ECHO, Neurocritical Care and EOLC and Communication workshops. Standardization of workshops is a work in progress and with time I hope all ISCCM workshops will be standardized like our 4C workshop. The revised 2nd edition of the 4C workshop will be unveiled for the first time at Agra. A new Critical Care Infectious Diseases Course will also be launched at the time of CRITICARE 2016. Registration for the workshops has been patchy and this may partly be due to the fact that there are now a lot of regional ISCCM conferences offering workshops.

International faculty, ISF and Main Scientific program

The preparation of the main scientific program for Criticare 2016 started with our agreement with the ISF. 14 international faculty from ISF will attend and there will be two halls devoted to sepsis sessions on day 1 and day 2 of the conference. This made it difficult for us to fit other topics and we were also not able to invite international faculty for certain themes. Fortunately for us we have Dr. Daniel De Backer (Hemodynamics), Dr. Massimo Antonelli (Mechanical Ventilation) and Dr. Romergryko Geocadin (Neurocritical Care) covering other areas. The ISF faculty too generously agreed to contribute to the non-sepsis scientific program. However with so many international faculty members, the Indian faculty may not have been too well represented.

The national scientific committee generously contributed topics and ideas for the various themes that have been included in the program. The pediatric program prepared by the ISCCM Pediatric section has been integrated with the adult program and has two sessions exclusively devoted to pediatric critical care on day 3. A

presidential session on “ICU culture around the world” is being done for the first time with the presidents of many international societies including SCCM, ESICM, ISF, and WFSICCM. There is also a panel on Critical Care in SAARC countries. The scientific sessions have been organized into various themes, interesting debates and informative master classes. The CritiQuiz has generated a lot of interest and the final quiz on day 2 promises to be as good if not better than the Quiz at Bengaluru. We have also introduced for the first time sessions along with our industry partners that will highlight the latest advances in critical care.

Research and showcasing young talent

More than 130 entries were received for free papers. There will be 36 oral presentations and over 80 E-poster presentations. A new format for E-poster presentations is being done as at the ESICM conference. There are also around 20 international ISF abstracts that will be presented.

Young talent awards have been given to 3 young presenters. As also we have introduced a new “Tips and Tricks by the Gurus” session where young intensivists from all over the country will present interesting cases and pose queries to gurus from India and abroad. We have also introduced a new “Best IJCCM papers 2015) session wherein the best 3 IJCCM papers will be presented.

Showcasing and highlighting our young talent along with offering the latest scientific knowledge through renowned national and international faculty has been a major challenge.

The jury is still out whether organizing the scientific content of a conference is a science or an art but there are always elements of expediency and “Jugaad”.
