    EXPLANATORY NOTES

    Vital statistics are derived from the Civil Registration System (CRS) and customarily tabulated

    according to the year of registration. Therefore, the number of occurrences of a vital event for a

    given year refers to the number of occurrences registered in that particular year. According to the

    law, registration of vital events is done at the place of occurrence. However, certain tabulations

    were prepared comply with the usual residence of mother in case of births and usual residence of

    deceased in case of deaths.

    Data on deaths registered in 2007 has been revised and all tables pertaining to deaths updated

    accordingly. Registration of births, deaths and marriages in Mullaitivu and Kilinochchi districts

    in 2008 have not been reported due to the situation prevailed in these districts. Therefore, the

    number of births and deaths registered in these two districts are excluded from the total births

    and deaths registered in Sri Lanka in 2008. Deaths of armed personnel that were registered at

    Head office of the Registrar General’s Department are included.

    Mid-year Population : Until 2011, the mid-year population estimates were prepared based on

    the Census of Population and Housing of 2001, which covered only 18 districts of the country

    and estimates for the rest of the 7 districts. District level estimates of mid-year population during

    that period were prepared on the basis of the place of occurrence.

    Estimates on mid-year population from 2012 onwards were based on the final results of the

    Census of Population and Housing conducted in 2012 which covered the entire Island and

    district level estimates of mid-year population were prepared on the basis of usual residence.

    Maternal Mortality Statistics: Information on maternal mortality extracted from the civil

    registration system is reviewed and supplemented with the maternal mortality data compiled by

    the Family Health Bureau of the Ministry of Health in order to improve the accuracy and

    completeness of maternal mortality statistics. This adjustment process was initiated in 2009.

    Crude Birth Rate (CBR) and Crude Death Rate (CDR): Until 2011, district level estimates of

    CBR and CDR are prepared using the number of births and deaths according to the place that

    they were registered. From 2012 onwards, the number of births by mother’s usual residence and

    deaths by usual residence of deceased is used to estimate district estimates of CBR and CDR

    respectively.

    Cause of Death: Causes of death (COD) information are extracted from the Death Declaration

    for hospital deaths, Verbal Autopsy for home death or Coroner Report for sudden deaths. The

    tenth revision of the International Classification of Diseases (ICD) is used to code the cause of

    death information.

    CONCEPTS AND DEFINITIONS

    CONCEPTS

    Live Birth refers to the complete expulsion or extraction from its mother of a product of

    conception, irrespective of the duration of the pregnancy, which, after such separation, breathes

    or shows any other evidence of life - e.g. beating of the heart, pulsation of the umbilical cord or

    definite movement of voluntary muscles - whether or not the umbilical cord has been cut or the

    placenta is attached. Each product of such a birth is considered live born.

    Marriage is defined as “an act, ceremony or process by which the legal relationship of husband

    and wife is constituted. The legality of the union may be established by civil, religious or other

    means as recognized by the laws of each country” (United Nations, 2001).

    Death is the permanent disappearance of all evidence of life at any time after live birth has taken

    place.

    Still birth is foetal death prior to the complete expulsion or extraction from its mother of a

    product of conception of 28 or more completed weeks of gestation .The death is indicated by the

    fact that after such separation the fetus does not breathe or show any other evidence of life, such

    as beating of the heart, pulsation of the umbilical cord, or definite movement of voluntary

    muscles.

    Maternal Death is the death of a woman while pregnant or within 42 days of termination of

    pregnancy, irrespective of the duration and site of the pregnancy, from any cause related to or

    aggravated by the pregnancy or its management but not from accidental or incidental causes.

    (WHO, 2004)

    Maternal death can be further sub divided into two groups; direct and indirect maternal deaths:

    direct maternal deaths are those due to obstetric complications of pregnancy (including delivery

    and 42 days postpartum) while indirect maternal deaths are those linked to other diseases or

    conditions when aggravated by the physiological effect of pregnancy.

    Age is the interval of time between the day, month and year of birth and the day and year of

    occurrence of the event expressed in the largest completed unit of solar time such as years for

    adults and children and months, weeks, days, hours or minutes of life, as appropriate, for infants

    under one year of age.

    Estate is any land of which ten acres or more in cultivation and situated in a district declared

    under section 3 of Medical Wants Ordinance to be an estate medical district.

    DEFINITIONS

    Mid-year Population is calculated using the Population Balancing Equation for which births,

    deaths and migration of the previous year is used. The number of births and deaths are the

    number of such events registered in that particular year. Net migration is derived from the data

    the Department of Immigration and Emigration.

    Population Growth Rate is the rate at which the number of individuals in a population

    increases in period from July 01st initial year to June 30th of the current year as a fraction of the

    initial mid-year population.

    Crude birth Rate is the number of births for a given year per 1,000 population in the same year.

    Crude Death Rate is the number of deaths for a given year per 1,000 population in the same year.

    Maternal Mortality Rate is the number of maternal deaths in a year per 100,000 live births in the same year.

    Infant mortality Rate is the number of deaths of children less than one year old in a given year per 1,000 live births in the same year.

    Neonatal Mortality Rate is the number of deaths before completing the first 28 days of life per 1000 live births in a given year or period.

    Perinatal Mortality Rate is the number of stillbirths and deaths in the first week of life per 1000 births (live births + stillbirths). Presently, perinatal mortality rate is not prepared due to unavailability of data.

    Age-specific Death Rate is the number of deaths of a specified age or age group in a given year per 1,000population of the same age or age group in the same period of time.

    Cause Specific Mortality Rate is the number of deaths due to a specific disease per 100,000 populations.

    Age-specific Fertility Rate is the number of births to women of a specified age group (15 – 49

    years) in a given year per 1,000 women in that age group in the same year.

    Total fertility Rate is the number of children that would be born per woman if she were to live to the end of her child-bearing years (age 50) and bear children at each age in accordance with prevailing age-specific fertility rates.

    General Fertility Rate is the total number of live births per 1,000 women of reproductive age (ages 15 to 49 years) in a population per year.

    Adolescent fertility rate (births per 1,000 women ages 15-19) is the number of children born alive to women aged15-19 per 1,000 women aged 15-19. Dependency Ratio is the number of children (0-14 years old) and older persons (65 years or over) to the working-age population (15-64 years old).

    Sex Ratio at Birth is the number of male births per 100 female births.

    INTRODUCTION

    Background

    Civil registration, as defined by the United Nations, is the continuous permanent and compulsory

    recording of the occurrence of vital events, like, live-births, deaths, foetal deaths, marriages etc.

    Civil Registration System (CRS) of a country is performed under the legislative provisions so as

    to provide a legal records and certificates on vital events to individuals and, to generate vital

    statistics on a continuous basis at the national and sub national levels.

    The superintendence and control of civil registration and vital statistics system (CRVS) in Sri

    Lanka is done by the Registrar General‟s Department, which was established in 1867, under the

    Birth and Death Registration Act, for the purpose of registering births and deaths. The general

    control and supervision of the department is vested in the Registrar- General.

    Legal Framework of Civil Registration System

    The earliest British enactment was the Regulation No. 7 of 1815 and this was followed by

    several amendments. The Ordinance No. 6 of 1847 was the first attempt to provide a general

    system of registration and was applicable to all births and deaths and to all marriages except

    those marriages of the Kandyan Sinhalese and the Muslims. This law was replaced by Ordinance

    No. 18 of 1867. The registration of births and deaths was made compulsory under an Ordinance

    passed in 1885 and came into effect in 1897. The present law for the registration of births and

    deaths was passed under Act No. 17 of 1951, which consolidated the several procedures

    contained in the previous laws and their amendments and became operational on 1 August

    1954.The present laws relating to civil registration in Sri Lanka were amended by the Births,

    Deaths and Marriages (amendment) Law No. 41 of 1975, that was provided to issue a free copy

    of the registration entry in the case of birth or death to the informant, and in the case of a

    marriage, to the female party.

    Administrative Structure

    For the purpose of registration of births, deaths and marriages, Divisional Secretariat Divisions

    are divided into smaller units, which are called as “Registration Division”. Each “births and

    deaths registration division” is consisted of one or more Grama Niladhari divisions. Registrars are called Medical Registrars when they are appointed to certain proclaimed areas like Municipal and Urban

    Council areas to register births, deaths and still births. A marriage registration division is generally

    co-existed with a Divisional Secretariat. For a marriage registration division there can be more

    than one Marriage Registrar.

    Under the decentralization policy of the Government, all administrative and statutory functions

    pertaining to civil registration have been transferred to the Divisional Secretaries that have been

    placed under the control of a Divisional Secretary and designated as Additional District Registrar

    for purposes of civil registration since 1992.

    Registration Procedure

    Registration of Birth: When a birth occurs, the father or mother, and in case the parents are unable to provide the information owing to some reasonable cause, the occupier of the house or building in which

    the child was born or each person present at the birth, or the person having charge of the child are bound

    by law to give information of the birth within 03 months of the date of birth to the respective Registrar.

    Registration of a birth is affected by entering the specified particulars in a birth register in triplicate.

    Registration of Death: Information of a death is required to be given to the respective Registrar within 5 days of the date of death. The law casts the obligation of giving information of a death to a Registrar, on

    the nearest relatives present at death, or in attendance at last illness of the deceased and in the absence of

    such relative every other relative of the deceased dwelling or being in the respective Registrar‟s division

    as the deceased and in the absence of such other relatives, each person present at death and occupier of

    the house in which the death took place.

    In respect of all births and deaths occurring in Estates, qualified informants should inform the

    Superintendents of Estates who are required to ensure that a report relating to each event is sent to the

    Additional District Registrar to enable him to register the event.

    Registration of Marriages: Registration of marriages in Sri Lanka is governed by three separate

    laws. The majority of the people in the country are subject to „the General Marriage Registration Ordinance‟. The Muslims are governed in matrimonial matters by „the Muslim Marriage and Divorce

    Act‟. The Kandyan Sinhalese have the option of getting married either under the „Kandyan Marriage and

    Divorce Act‟ or under „the (General) Marriage Registration Ordinance‟.

    Under the „e-civil programme‟ for computerization of registration process on life events – births and

    deaths, computerization of information have started at Divisional Secretariat level.

    Administrative Arrangement of Vital Statistics System

    Prior to independence, compilation of vital statistics was the responsibility of the Registrar

    General and the detailed vital statistics was published in the annual administration reports of the

    Registrar General‟s Department. In 1951 the Department of Census and Statistics (DCS) which

    was vested with the responsibility of supplying statistics required by the government took over

    the responsibility of compilation of vital statistics from the Registrar General. Since then,

    compilation, processing and tabulation of vital statistics are carried out by the Vital Statistical

    Unit, of the Registrar General‟s Department. The technical work of this unit is closely supervised

    by the Demography Division of the DCS.

    Reporting of vital statistics to Vital Statistics Unit of the Registrar General‟s Department is performed in

    two stages.

    1. Monthly summary on number of registered births and deaths are reported by Additional District

    Registrar of divisional secretariat by 20th of the following month.

    2. Computerized data set on detailed information on births and deaths are sent to Statistics Unit

    through e-mail. (Forms on Vital statistics of birth and death, Verbal Autopsy are attached.)

    Statistics Forms on birth, Death and Marriage which should be sent to the District Registrar before the

    checked for their accuracy and completeness. These forms are dispatched to the Land Registry Office for

    computerization. Land Registry Office is obliged to send computerized data each month.
