I. Call to Order

The meeting was called to order at 8:05am, Thursday, November 14, 2019.

Hill-Mekoba presided.

Hill-Mekoba welcomed members of the audience, explained the procedure for addressing the Board, and stated the

mission of the Board.

Pultz provided an update to the Board, that on 11/4/19, Board staff spoke telephonically with the Compliance officer

at Comprehensive Hospice, who corroborated the verbal and written statements of Complainant #2. Pultz also updated

the Board that on 11/13/2019, Respondent's mother came to the Board offices and provided Board staff with nine

letters of recommendation written on behalf of Respondent, along with medical information which was discussed

during a telephone interview with Respondent's mother on 10/03/2019.

Based upon the information presented at this meeting, Board members discussed whether a showing of extraordinary

circumstances or the possibility of irreparable harm has been made supporting the need for a Summary Suspension.

The proceeding began at 8:43am with the following Board members present: Quinn, Andersen, Berrigan, Boyer,

Atkinson was telephonically present and addressed the Board.

Chambers provided an update to the Board, and stated that Respondent was issued an interim order from the Board

Executive Director, but had received no response. Respondent stated she wanted to voluntarily surrender her license

but has not signed the voluntary surrender document yet.

Board member Quinn discussed if Respondent was in treatment. Respondent stated she is not.

After being called to the facility for assistance, you assessed the patient and recognized she needed to be transported

to a higher level of care for immediate treatment. You delayed emergent care by approximately one hour and 15

minutes while arranging for the purchase of supplies, removing previously applied ointment and dressing the burns,

causing the patient pain. You then chose in-house, private transportation to the hospital, instead of calling for an

emergency vehicle. Because of the severity and extent of the patient's burns, the standard of care dictated emergency

transport to a burn unit. During a telephonic interview with Board staff on August 29, 2019, and in your written

follow up on August 30, 2019, you admitted to treating the patient's burns before transport, estimating one hour and

Board meeting, upon meeting all licensure requirements and signing a Consent Agreement for a stayed revocation

probation not to exceed 12 months, followed by a minimum 24 month probation, grant licensure, immediately revoke

the license, and stay the revocation. The license will be initially limited for participation in an RN Board-approved

refresher or equivalent course only. Upon successful completion of this course, the limitation shall be lifted, and the

probation continued under applicable terms contained in the attached stipulation worksheet. If the refresher or

equivalent course is not completed within (12 months), or other violations of probation occur, the stay shall be lifted

and the license revoked. If not signed within 30 days, deny reissuance of licensure, as Applicant failed to submit

substantial evidence that the basis for the previous surrender has been removed and that the issuance of Applicant’s

license will not be a threat to public health or safety without the proposed additional monitoring and remediation.

IX. Dialogue with Nursing Students

Board processes, importance to report to the Board a criminal charge within 10 days, appointment to the Board of

Nursing, source of complaints, and most common complaints.

honesty regarding mistakes, asking questions, addiction issues and self-care.

reasonable cause, based upon the information in the investigative report, and material presented at this Board meeting,

at no cost to the Board, issue an Interim Order for a comprehensive psychological evaluation to include anger

management to be completed by a Board approved licensed psychologist, to be completed within 45 days and then

return to the Board. If the Interim Order is not completed, based upon the information in the investigative report, issue

a Notice of Charges and include the factual allegations and alleged violations for failure to comply with the Interim

Order.

Ridenour provided updates, Quinn and Preston will be retiring after this meeting.

Machesky has been reappointed for 5 years. Angela Fountain was appointed and will be oriented to the role prior to

the next Board meeting. Carolyn McCormies has been appointed. Berrigan and Guttierez will continue on until new

appointments are made to aid in transitions.
