Advertise the class in the local papers, online news, social media and email lists. e. Registration protocol and training of staff and volunteers f. Memory testing for those who request prior to class starting. g. Mail reminders to presenting professionals and staff and the class agenda to registered participants. f. Survey class participants at final class meeting to assess effectiveness and needs of participants. 2. $2400.00 to fund a Wellness Calling System for one year to make daily calls to approximately 30 women who live alone with dementia or who care for a demented partner with no family locally to check on them. a. Create training program for staff and volunteers who will make the telephone and email contacts. b. Create assessment packet that includes contact information, times and protocol in case of emergency. c. Train staff and volunteers on calling protocol and emergency protocol for clients. d. Survey wellness participants to assess effectiveness and needs of participants. 3. $1000.00 to fund 10 rural outreach trips providing education and support group consultations to underserved areas in Southern Utah and SE Nevada helping female caregivers and women with dementia. a. Survey rural Area Agency on Aging director to assess need for outreach, consultation and education. b. Identify contact information and locations for rural outreach services. c. Develop a follow up plan for continuation of support. c. Create rural outreach packet and educational supply cart.

CAPACITY

Our organization has been expanding the Early Stage Memory Loss education series since 2006 and have partnered with Intermountain LiveWell, area neurologists and neuropsychologists, doctors, nurses, social workers and lawyers from various community agencies.

Rural outreach has begun in Mesquite Nevada, Kanab and New Harmony. Funding to create systems, packets and relationships in these areas will help us have capacity and sustainability as we move forward.

The Wellness calls have been a desire for many years, but until this year we lacked the office space for volunteers and staff to do this service. Funding will help us develop a system, protocal and the training to continue this desperately needed service in the future.
