1.PURPOSE OF POST:

1.1 To support patients (18 years and over) attending Eastbourne District General Hospital Urgent & Emergency Department (EDGH UED) to access primary care services, providing information and guidance to use more appropriate health care rather than attending the Urgent & Emergency Department (ED)

1.2 To support unregistered patients attending EDGH UED to understand the value of being registered with a GP for their ongoing primary healthcare needs

1.3 To have a positive impact by lowering the numbers of repeat unregistered attendances at EDGH UED and offer alternative solutions to support people’s needs.

2.DUTIES AND RESPONSIBILITIES

2.1 To work as part of the Urgent &Emergency department team to effectively implement the Non-clinical Navigator service

2.2 Ensure the service is accessible to all people

2.3 Meet and greet patients at reception, facilitate and administrate the Non-clinical Navigator service and ensure accurate record keeping

2.4 Work closely with clinical and reception staff to identify patients whose presenting condition would make them more suitable to access services provided in primary care or in the local community

2.5 Ensure patients are aware of all Primary Care health services available and how to access them, providing appropriate information to patients e.g. service leaflets, written information, health apps

2.6 Be responsible for identifying those patients who are not currently registered with a local GP and assist these patients to register

2.7 Identify unregistered patients from other areas assisting and encouraging them to register with a GP in a similar way, providing information on how to register with a GP and directing them to practices close to their home address via use of nhs.uk

2.8 Ensure there is a system in place to contact the GP practice once a patient has agreed to registering, to follow up patients who have been given GP registration details and record relevant data

2.9 Forge close working links and foster positive working relationships within the Urgent and Emergency department and all GP practices within ESBT

2.13 Have access to a comprehensive, accurate directory of services (dos) available locally from statutory, voluntary and the community sectors, ensuring this is always kept-up-to-date

2.10 Offer patients signposting and/or direct support to appropriate local voluntary organisations and community groups

2.11 Identify when there is a need for urgent action or for a step-up in care and communicate with the relevant professional

2.12 Ensure any carers are identified and signposted to appropriate support services2.14 Work collaboratively with other NCNs and the Team leader so that the service is co-ordinated, monitored and developed effectively

2.15 Attend regular supervisions, appraisal, case management, debriefings and group meetings

2.16 Ensure that accurate records, data, client evaluations and client case studies from the NCN service are collected and analysed and to provide monitoring reports required by East Sussex Better Together, ESHT and Age UK East Sussex, plus any other reports as required

2.17 Keep up to date with service developments and recommend improvements to the NCN service on the basis of new innovation and good practice

3.Other

3.1 To work within the terms of the policies and procedures adopted by Age UK East Sussex, ESHT and ESBT Standard Operating Procedure, with particular attention to Equality and diversity, confidentiality, safeguarding children and young people, safeguarding adults and health and safety requirements

3.2 To be prepared to alter agreed weekly working arrangements on occasions to be able to attend meetings and events and/or to cover other team members’ sickness or annual leave

3.3 To understand and act within relevant quality frameworks

3.4 To undertake other tasks and duties as may be reasonably required by the Non-clinical Navigator Team leader or the Urgent & Emergency Department Senior Sister

TrainingAll new service Non-clinical Navigators will be expected to take part in the following training: First aid, manual handling, infection control, health & safety, safeguarding children and young people, safeguarding adults, equality and diversity, confidentiality, data protection, lone working, dementia awareness, and any other training as requested for the role

A minimum of 2 years active and successful experience of working with adults within a health/ social care or information and advice setting

Experience of working with the public ✓

Demonstrable experience of working to contract specifications

Demonstrable experience of constructive partnership working with voluntary, statutory and community organisations
