CAPACITY BUILDING GRANT 2017 APPLICATION The mission of the Food Bank of Siouxland is leading Siouxland in the fight against hunger. PROGRAM OVERVIEW Food Bank of Siouxland is pleased to announce a grant opportunity made possible through the support of the Walmart Foundation. The Capacity Building Grant program is a competitive grant process that awards funds to partner agencies of the Food Bank that seek to make improvements to their program, allowing them to more effectively serve the individuals in their communities. Grant awards up to $2,500 may be requested. Itemized details need to be provided showing how the amount of money requested will be used if awarded. Eligible Applicants: Partner Agency must be active (ordering at least quarterly for the last year), partner for at least four months at the time of the grant deadline, and are reporting statistics regularly. Partner Agency must have met all Food Bank requirements for Food Safety and Civil Rights Training, reporting of retail pick-ups, and may not have an outstanding balance with the Food Bank over 30 days old at the time of the grant deadline. Partner Agency must have been found in compliance with safe food handling and storage of product at its last visit by any Food Bank staff person. DEADLINE: Friday, September 1, 2017. The Capacity Building Grant program is a competitive grant process that awards funds to partner agencies of the Food Bank that seek to make improvements to their program, allowing them to more effectively serve the individuals in their communities.

GRANT GUIDELINES

Evaluation Criteria: Grant applications will be evaluated based on: Strength of capacity building strategy/goals as detailed in the application. Likelihood of capacity building strategy/goals creating sustainable changes after

the funding period ends.

Expectations: If awarded a grant, partner agencies must submit a brief follow-up grant report by the designated due date detailing achievements resulting from the grant and how grant funds were expended. The report and due date will be provided at the time of grant award.

Timeline:

Wednesday, August 9, 2017 – Application posted online and emailed to Partner Agencies.

Friday, September 1, 2017 – Grant application deadline

September 5 through September 8, 2017 – Grant review period. You may be contacted during this time if the review committee has follow-up questions regarding your application or needs to schedule a site visit.

Monday, September 11, 2017 – Notification of decisions mailed and emailed.

September 18, 2017 – Deadline for the Grant Agreement to be returned to Food Bank of Siouxland. Ifthe Grant Agreement is not received, the award will be considered unclaimed. Unclaimed awards maybe given to other qualifying applicants.

September-October, 2017 – Distribution of grant awards.

Follow-up Grant Report - Due upon completion of your project or no later than May 31, 2018. The report should detail achievements resulting from the grant and how grant funds were expended. The follow-up grant report form will be mailed along with the Notification of Decision letter.

How to Apply: Typed applications are preferred. Handwritten applications will be accepted if they areneat and easy to read. Any proposal submitted after the deadline will not be considered.

4. If awarded a Capacity Building Grant to make improvements to your program, how will you measurethe effectiveness of your capacity building strategy? How will you know if the improvements weresuccessful at increasing your program’s effectiveness? (Example: “We will conduct a baseline survey prior to the implementation of a grocery shopping model distribution and within 60 days after instituting this model, we will conduct another brief survey with our clients and volunteers, asking about their overall experience, challenges, and suggestions for changing or modifying the model.
