REQUIREMENTS
Instructions for Earning Therapy Continuing Education and/or receiving a
Certificate of Attendance
This booklet is your ticket to earning therapy continuing education credit and/or receiving a certificate of attendance. Please read the instructions for this process and feel free to contact Abby at [email protected] or 573-634-7772 if you have questions.
STEPS YOU MUST FOLLOW IN ORDER TO RECEIVE THERAPY CE OR A CERTIFICATE OF ATTENDANCE:
1. The ONLY way you will receive a certificate is to email or mail the completed booklet
when you finish listening/watching your sessions, which includes the Verification of Attendance form and the individual session evaluations. You will receive your certificate by EMAIL after all documents are verified and received by MAHC. THERE ARE NO EXCEPTIONS to this process!
2. Complete the Verification of Attendance Form on the next page of this booklet. As a reminder - name, address and email MUST be completed.
3. As you attend each session of your choice, complete the corresponding evaluation for that
session located in this booklet.
4. As you complete a session evaluation: Record your attendance on the form called the Verification of Attendance. Repeat this process throughout the conference (evaluate each session and then record
on the Verification of Attendance form).
5. When it is time for you to turn your information in . . .
• Check to make certain you completed the Verification of Attendance Form by checking each session you attended.
• Check to assure that your name, address, email and signature appear on the Verification of Attendance Form. (This is important as this is the ONLY way we can VERIFY that you attended the sessions. Please legibly print your name and email!)
• Check to make certain you have completed evaluations for each session you attended Again, only completed verification of attendance & session evaluations will receive a certificate by email.
• Email your completed booklet to [email protected] or mail to: Missouri Alliance for Home Care, 2420 Hyde Park, Suite A, Jefferson City, MO 65101. A link to the overall evaluation can be found here. Please take a few minutes to complete the evaluation to assist MAHC in future planning.
