About this guideWelcome to the 2018-2019 issue of LiveWell, a resource guide designed specifically with older adults, people with disabilities, and caregivers in mind. The landscape of issues for the population we serve is continually changing, therefore the Ventura County Area Agency on Aging (VCAAA) is continually creating innovative methods and programs to enhance quality of life and provide options to safely, securely, and optimally age in place.

VCAAA’s commitment to evolve to meet the needs of seniors, individuals with disabilities, and their caregivers extends to this publication. LiveWell made its debut last year to replace what was Eldercare. This year’s guide has been taken to a new level to better serve Ventura County and is now a fully integrated publication that offers stories and listings in both English and Spanish. VCAAA is proud to be able to offer a far more accessible guide to the communities we serve.

The articles and listings in this guide are to help you and your loved ones navigate through some of the challenges we all face in our quest to maintain our independence and to provide you with tools to advocate for the services you need to LIVE well and age optimally.

This guide can be a valuable tool, but remember, it is just that, a guide to help you find resources. If you have specific questions or need further assistance, don’t hesitate to contact one of our Information & Assistance Specialists to assist you further. They can be reached at (805) 477-7300, 8:00 a.m. to 5:00 p.m., Monday through Friday and are trained to connect you to the multitude of resources available in Ventura County and can arm you with the information you need to make those important life decisions.

LiveWell is available at a variety of locations throughout Ventura County as well as directly through the VCAAA office in Ventura. An online version of the guide is also available at www.vcaaa.org.

Acerca de esta GuíaBienvenidos a la edición de 2018-2019 de LiveWell, una guía de recursos diseñada específicamente pensando en adultos mayores, personas con discapacidades y proveedores de cuidadores. El paisaje de problemas para la población a la que atendemos está cambiando continuamente; por ello, la Agencia Sobre el Envejecimiento del Área del Condado de Ventura (VCAAA, por sus siglas en inglés) está creando continuamente métodos y programas innovadores para mejorar la calidad de vida y proporcionar opciones para que las personas puedan envejecer en su lugar de residencia de forma segura, protegida y óptima.

El compromiso de la VCAAA para evolucionar y así cubrir las necesidades de personas mayores, personas con discapacidades y sus cuidadores, se extiende a esta publicación. LiveWell debutó el año pasado, sustituyendo a la antigua lo quelra el Eldercare. La guía de este año ha pasado a un nuevo nivel, para servir mejor al Condado de Ventura, y actualmente es una publicación plenamente integrada que ofrece artículos y anuncios tanto en inglés como en español. during a visit to your doctor, nurse, or therapist, you will be advised to exercise on a regular basis. What are the benefits of exercise? How does regular exercise improve your health? And how much and how intense does someone need to exercise to benefit from regular, structured physical activity?

Regardless of your age, regular physical exercise is essential to maintaining functionality. We often decrease the amount of physical activity we do daily as we age, resulting in a loss of strength, flexibility, resilience, and function. Adding exercise to a daily routine provides a positive impact in many specific ways, including:

• Slows the effects of aging

• Increases lean muscle mass and builds strength

• Maintains flexibility of the joints and muscles

• Improves circulation, heart and lung function

• Works to maintain a normal weight

• Promotes improved sleep and restfulness

• Helps to manage stress, anxiety and tension

• Decreases the risk of ground level falls at home and in the community

ENTER: The Exercise PrescriptionThe Exercise Prescription refers to the specific plan of fitness- related activities that are prescribed by a health professional such as a doctor, exercise physiologist, or physical therapist to target specific conditions as well as address general health. Currently there is an initiative called Exercise is Medicine, to promote the idea that physical activity has the same importance as diet, medications, and other medical interventions.

The exercise prescription includes various types of exercise including aerobic (heart-specific) activities, resistance training (strength specific), balance activities, and flexibility exercises. A well-designed prescription will include all of these different types of exercises and provide guidelines for intensity, duration,

frequency, and precautions to prevent injury or damage. The American College of Sports Medicine (ACSM) recom-

mends moderate physical activity for everyone, regardless of age. According to Exercise and Sport Science Australia, a min-imum amount of 210 minutes of moderate intensity exercise or 125 minutes of vigorous intensity exercise should be per-formed per week. Exercise should include both aerobic and resistance training. For greater health benefits, exercise should be performed regularly with no more than a two-day gap between training sessions.

Often it can be difficult to get insurance to pay for a clinical visit with a professional for the specific purpose of receiving an exercise prescription. A general or family physician can screen for contraindications, recommend shoes, provide ad-vice, and offer a referral to a therapist who can provide an exercise prescription as part of a care plan – an important step in a crucial form of medicine that extends and saves lives.

COALITION AND PROGRAM

Most falls occur in the victims’ primary residence (68 percent).

In the aim to raise awareness, the EFPC conducts multiple presentations locally and nationally. Since 2013, the Coalition has hosted nine local awareness forums.

The Coalition created the Elderly Fall Prevention Program (EFPP) that begins the falls awareness process in the field. In 2014, a pilot program began in west Ven-tura County. Initially, EMS providers deter-mine whether the individual’s call is truly fall related when responding to a 911 call. A quick home assessment and mitigation is conducted, including recommendations for less hallway clutter, improved lighting, or proper rug placement, etc. Important literature is imparted. Evidence-based tests are performed when the patient arrives in the hospital.

With the patient’s consent, data gath-ered from the fall prevention registry is used to determine the next steps; i.e. home assessment versus further medical needs from public health. If home modi-fication is required, services are provided by CBOs involved in the coalition. The

Ventura County’s Elderly Fall Preven-tion Coalition (EFPC) has been in

existence since 2013. The coalition was formed by a multidisciplinary group head-ed by the Ventura County Medical Center after Coalition members noticed a signifi-cant increase in injuries throughout Ven-tura County that were directly related to falls. The goal of the Coalition is to aim for prevention of primary falls.

This Coalition is a union of more than 30 Ventura County-based organizations; including, but not limited to: both trau-ma centers, Emergency Medical Services (EMS), Ventura County Area Agency on Aging (VCAAA), Ventura County Board of Supervisors (VC-BOS), Public Health, all other non-trauma hospitals, and nu-merous community-based organizations (CBOs), highly vested in improving care of older adults.

Falls are the leading cause of injury deaths and doctor visits among people over 65. They are also the leading cause of hospital admissions and emergency de-partment visits. It is striking that an older adult dies from a fall every 29 minutes.

fall prevention co-ordinator (FPC) en-courages participa-tion in balance and strengthening evi-dence-based classes (EBC), such as Tai Ji Quan: Moving for Better Balance, A Matter of Balance, Stepping On, and Walk with Ease. Year-to-date, the number of EBC participants has increased from 6,300 in 2014/2015, to more than 13,000 in 2016/2017.

Data shows that older adults who fall and decline have close to a five-fold chance of falling again, versus those that fall and participate in EBCs.

The EFPC has been awarded yearly proclamations by the VC-BOS since 2013, the highest award by the National Asso-ciation of Area Agencies on Aging (n4a) in 2015 for a successful program that im-proves lives of older adults and caregiv-ers, and given the health care champion award for 2017 by the Partnership for Healthy Ventura County.

Further funding will be required to ex-pand the program to the entire county. The coalition will not be successful with-out multidisciplinary efforts.

Evidence shows that the world’s population is getting older, and the population of individuals 60 and older will increase from 11 percent to 20 percent by 2020.


L as evidencias muestran que la po-blación mundial está envejeciendo, y

la población de personas de 60 años de edad y mayores aumentará desde el 11 por ciento al 20 por ciento para el 2020.

La Coalición para la Prevención de Caídas de Personas Mayores del Condado de Ventura (EFPC por sus siglas en inglés) se fundó en 2013, formada por un grupo multidisciplinar encabezado por Ventura County Medicalio Center, los miembros de la Coalición observaron un aumento significativo en lesiones en todo el Condado de Ventura directamente relacionadas con caídas. Con la finalidad de incrementar la con-ciencia del problema, la EFPC realiza múl-tiples presentaciones a nivel local y nacio-nal. Desde 2013, la Coalición ha celebrado nueve foros locales de concientización.

La Coalición creó el Programa de Pre-vención de Caídas para Personas Mayores (EFPP), con el cual comienza el proceso de concienciación sobre las caídas en este campo. En 2014 se puso en marcha un programa piloto en el oeste del Condado de Ventura. Inicialmente, los proveedores de EMS determinan, cuando responden a una llamada al 911, si la llamada de la persona verdaderamente está relacionada con una caída. Se realizan una evaluación y mitigación rápidas en el hogar, incluyen-do recomendaciones para que haya menos desorden en los pasillos, mejor iluminación, o una colocación adecuada de alfombras, etc. Se entregan publicaciones importantes y se realizan pruebas basadas en evidencias cuando el paciente llega al hospital.

Con el consentimiento del paciente, se utilizan datos obtenidos del registro de pre-vención de caídas para determinar los sigui-entes pasos, es decir, evaluación del hogar en vez de necesidades médicas adicionales por parte de salud pública. Si es necesaria una modificación del hogar, los servicios los prestan las CBO que participan en la coali-ción. La coordinadora de prevención de caí-

TIPS FOR

HEALTHY LIVING

The importance of medication management cannot be overstated. While we’ve quickly become used to the idea of taking more and more vitamins and pills each day, this has a number of potential health hazards. What can be done to help you take medications safely?

1. Make sure the pharmacy label says why you are taking the prescription. 2. Create and maintain an up-to-date medication list (including over-the-counter

drugs and herbal supplements) and bring the list to the doctor with you. 3. Ask your provider if the dosage is age-appropriate. 4. Talk to the pharmacist and ask questions. Learn about the

potential side effects, dosage, proper storage, and anything else that will help you take medications correctly.

5. Get a second opinion if you are uncertain. 6. Be aware of medications deemed unsafe for the elderly. 7. Know the side effect profile of your medications. If you notice health

changes, contact your physician. 8. Tell your provider about any previous adverse drug effects. 9. Closely monitor medication compliance in the cognitively impaired. 10. Minimize the number of providers and pharmacists you use.

3rdIN VENTURA COUNTY, Alzheimer’s Disease serves as the third-leading cause of death. More than 14,000 people in Ventura County are living with Alzheimer’s, and more than 40,000 individuals are serving as caretakers for those living with the disease.

Medicaid costs in California related to Alzheimer’s are currently $3.776 billion, and will jump by 36.4 percent by 2025. Per capita, $30,072 is spent on Medicare for people with dementia.

The percentage change in Alzheimer’s disease deaths has also risen to 71 percent between 2000 and 2013, where-as the percentage change in breast cancer, prostate cancer, heart disease, stroke and HIV deaths have all decreased.

In Ventura County, more than 70 percent of those with mild cognitive impairment (MCI) have at least one other chronic condition.

Every 65 seconds, someone in the United States develops Alzheimer’s dementia. By mid-century, someone living in the U.S. will develop the disease every 33 seconds.

In Ventura County, only 45 percent of people living with Alzheimer’s, or their caregivers, reported being told of their diagnosis. Given the long duration of this disease, the phys-ical, emotional and financial strain on Alzheimer’s caregivers can last several years. In 2017, the lifetime cost of care for a person living with dementia was $341,840.

New findings from the 2018 Alzheimer’s Association Facts & Figures Report show the growing burden of Alzheimer’s on people living with the disease, their families, and caregiv-ers. In California in 2017 alone, there were more than 1.5 million caregivers providing more than 1.8 billion hours of unpaid care, valued at a total of $23 billion.

Tips for staying ahead of the disease include:• Take care of your

mental health.

• BUDDY UP: staying socially engaged may support brain health.

• STUMP YOURSELF: challenge your mind with a strategy game like bridge or build furniture.

Breaking BarriersTO PROVIDE BETTER SERVICE

AS AN ADULT WITH A MOBILITY DISABILITY, it would be easy for me to spout off the things not to do: stare, over-help, pat me on the head! But, the most impactful way for service providers to improve is to consistently educate themselves about the attitudinal barriers we each have in us. We all tell ourselves we are accepting and inclusive of individuals with disabilities, but believe it or not, we still contain some pity, stereotype, or fear – especially when it comes to communication.

So, how can service providers, volunteers, and peers combat these ingrained barriers?

Learn about leaders in the disability community: Ed Roberts, Justin Dart, Richard Devylder, Cather-ine Campisi. Learn about the people who scaled the steps of the U.S. Capitol Building to protest in-accessibility, organized around transportation until public buses were accessible, advocated for closed captioning and ASL interpreting for communica-tion access, or tactile domes on sidewalks. Look at the disability as the co-culture it is, not as the curse or unnatural thing many believe it to be.

As an Independent Living Center, we follow and fully believe in the Social Model of Disabili-ty, which says disability is natural, and ALL of us are going to experience it at some point in our lives. We may be born with a disability, have a temporary one after an accident, or experience it through aging. The Social Model is a frame-work for an accessible world, architecturally, attitudinally, and culturally. Individuals with dis-abilities are not “the problem”; an inaccessible world is. Our organization, the Independent Liv-ing Resource Center (ILRC), has the privilege of being a core partner with the Ventura County Area Agency on Aging (VCAAA) in the Ventu-ra County Aging and Disability Resource Center

(ADRC), and our organizations have become a state model for bridging the gap in services for individuals with disabilities and older adults. We recognize that these two populations often do not want to face the fact that their services can be the same.

As I sit here writing this article, I can’t help but reflect on the requested subject: how can we better serve adults with disabilities?

The VCAAA already does an excellent job of serving adults with disabilities. They have sought out training and relationships that lend to a cohesive and inclusive environment. ILRC is seen as an ex-pert in cross-disability, independence-based services and feels very lucky to partner with the VCAAA in this ever changing and improv-ing world of service provision to individuals with disabilities and older adults.

This vision is just around the corner.Every day, with donor support, the Ventura County Area Agency on Aging Foundation (VCAAA Foundation), formerly known as the Senior Nutrition Action Council (SNAC), is helping to conquer hunger and isolation by providing funding for nutritious meals and other life-changing services to individuals aged 60 years and older. We are making even greater strides to engage with the community so that we will better understand the gaps in services for older adults in our local neighborhoods and identify creative ways to drive change, so that we can together do the most good. But we need your support.

As baby boomers like myself become seniors, our community faces mounting health and economic challenges associated with an aging population. We started seeing more waitlists and greater service gaps when the first of the boomers turned 60 in 2006, and growing needs persist as the youngest of this generation will earn their older adult distinction by 2024.

Our aim is to keep up with this growth and bridge these gaps to dramatically improve the quality of life for older adults, adults with disabilities, and caregivers. We are the VCAAA Foundation, and for more than 25 years we’ve been working as a nonprofit charitable organization to help target and serve this diverse population countywide. We recently expanded our organization’s mission to provide support beyond nutrition services to include all Ventura County Area Agency on Aging (VCAAA) programs and activities, such as housing, care management, legal services, in-home and caregiver services, and many other services in demand. We also build partnerships that bring together resources, so we can together shape the future of senior services.

Imagine una comunidad en la que todas las personas mayores y adultos con discapacidades en cada vecindario están felices, sanos, conectados, productivos, y viven con independencia donde quieren vivir. Y los que necesitan ayuda tienen cuidado con los recursos para ofrecer atención de calidad a lo largo del camino. Imagine un Condado de Ventura con equi-dad geográfica y una comunidad entera comprometida en el esfuerzo para hacer que esto sea una realidad continua.

Things have changed for the LGBT community in recent years. Marriage

equality brought greater awareness of LGBT issues. It has also served to bring greater acceptance of LGBT relationships.

However, older LGBT people may still be reluctant to identify as LGBT when entering an assisted living facility, nursing home or accepting home health care into their home. Their historical discrimination and growing up in a time when being gay was a crime cause many LGBT older adults to be fearful in new situations, especially when they are in a vulnerable physical state.

Enter the “LGBT Aging Coalition of Ven-tura County,” an organization that evolved out of the nonprofit Senior Concerns in Thousand Oaks. Senior Concerns organized community leaders and organizations inter- ested in changing the condition of LGBT elders' lives here in Ventura County. The mission is to provide a network of resourc-es and education in support of LGBT older adults living in Ventura County.

Since 2017, the LGBT Aging Coalition has been offering quarterly presentations to the public to educate about LGBT aging issues. Recent topics presented by the Co-alition have included economic inequalities faced by LGBT seniors, impacts of demen-tia with a focus on access to care for older LGBT adults and a panel presentation of personal experiences. These presentations are open to the public but also provide a focus on service provision for agencies serving older adults. Practical information for agencies serving older adults is provid-ed with an opportunity to ask questions and gain feedback.

“Our goal is to create a community in Ventura County where every older adult, including every LGBT older adult, will feel comfortable and safe accessing the services they need to age in place with dignity,” said Martha Shapiro, one of the leading forces behind the success of the LGBT Aging Coalition.

The LGBT Aging Coalition looks forward to expanding the education and trainings

in 2019 to include topics involving legal issues, housing rights, financial insecuri-ty, social isolation and access to services. These presentations will both serve to edu-cate and empower those in our community who are providing the services as well as those receiving the services. Together we can create the community we wish for, one where every older adult can access the needed services and age with the very best quality of life.

Aging in Ventura County as a Lesbian, Gay, Bisexual, or Trans-gender individual (LGBT) can create new challenges and con-cerns. There are unique barriers to housing, healthcare, homecare and other long-term care services that older LGBT adults need support navigating. Approaching these new services can feel daunting, but avoiding them can lead to a decline in health and functioning. The LGBT Aging Coalition of Ventura County was formed to help make Ventura County a safe and welcoming place that provides accessible services to all older adults, with a special focus on the needs of the LGBT older adult population.

PET PANTRY: Members of our community sometimes fall on hard times and have to make the difficult decision whether to buy food for themselves or pet food for their four-legged friends. VCAS is fortunate to have community supporters and local businesses who donate enough pet food to keep our Pet Pantry stocked! Our Pet Pantry is open daily during office hours at both shelters in Simi Valley and Camarillo. To apply for Pet Pantry services, visit www.vcas.us/PetPantry.

ADOPTION DISCOUNT: VCAS offers adoption discounts to seniors who wish to adopt a loving pet. Anyone 55 years or older can adopt a dog or cat for half the price of a regular pet adoption! An additional pet license fee may be included.

VOLUNTEERING: Ventura County Animal Services has a wonderful workforce of dedi-cated volunteers who give their time and energy to care for, enrich and love the more than 12,000 animals who cross our threshold annually. In 2017, our volunteers donated an astonishing 79,316 hours of service. Many of our volunteers are recent retirees and have called VCAS their “home away from home.” Our 400 active volunteers range in age from 14-84 and many lent their hearts and souls to our rescue efforts during the recent Thomas Fire. VISIT: Guests are welcome to visit both shelters Tuesday through Sunday. Grey Law is a private, nonprofit organization that provides free legal services for seniors in Ventura County. Our mission is to fill the gaps often left by traditional legal firms.

Grey Law primarily provides legal services that used to fall under the category of “Poverty Law.” In truth, Poverty Law, such as qualifying for Medi-Cal, can now ap-ply to many older adults who never considered it in years past. The services that fall under what was Poverty Law become a vital resource when faced with the possibility of needing a skilled nursing facility.

The work conducted at Grey Law also provides older adults with answers for ques-tions about their rights, Medi-Cal, Medicare, landlord-tenant issues, skilled nursing care issues, powers of attorney, advanced health care directives, and whether or not it is a good idea for an individual to author a will for free.

When a client calls, they will find that Grey Law is a one-on-one, face-to-face firm that provides the opportunity for clients to seek answers to all troubling legal questions. These one-on-one sessions are conveniently handled at Senior Centers throughout Ventura County by appointments scheduled through Grey Law’s main office.

In addition to one-on-one sessions with clients, Grey Law provides a great deal of public education on a vast spectrum of legal issues. For example, the Grey Law legal team provides a class called L.I.F.E. (Legal Information For Elders) that covers most of the areas of law with which we can assist. These classes are scheduled through the Ventura County Area Agency on Aging. Grey Law also has very specific classes on new laws, Medi-Cal, Medicare, elder abuse, and other important information topics.

Because of budget limitations, Grey Law cannot help clients with every area of law. For example, Grey Law cannot help with civil litigation matters, family law matters, and cannot provide defense for criminal matters. The private bar is the correct choice for litigation, most civil matters, and family law matters, and the Public Defender’s office is available for criminal matters.

Since its inception, the BEC has saved Ventura County residents millions of dol-lars. In the first half of 2018 alone, the BEC was able to save clients nearly $550,000.

It has been five years since the Ven-tura County Area Agency on Aging

(VCAAA) opened the doors to the Ben-efits Enrollment Center (BEC), providing a crucial resource for Ventura County resi-dents looking to bridge the gap between financial hardships and some of life’s most basic needs.

The vision was clear from the start: offer a one-stop shop and no wrong door ap-proach to individuals 18 and over living in Ventura County.

VCAAA staff and volunteers alike un-derwent extensive training to understand every element of the public benefits avail-able to any client who might seek guidance and service from the BEC. The doors then opened to a comprehensive program that provides everything from an initial screen-ing and counseling sessions, to assistance with applications and continued support as financial and medical needs change.

The list of benefits that can be accessed through the BEC is extensive and includes food and nutritional support through CalFresh; home utility bill support through Low Income Home Energy As-sistance Program (LIHEAP); Medi-Cal coverage; prescription drug coverage; unemployment benefits, and the Medi-care Savings Programs.

Services offered through the BEC are not “a one size fits all,” and counselors carefully assess the needs of every individual to de-termine the benefits for which they qualify.

Case in point:A retired Ventura County resident was struggling to make ends meet. On a fixed income, and against rising cost of living ex-

penses, utility and grocery bills were quickly mounting. A visit to the BEC led to enroll-ment in CalFresh, which helped decrease monthly food expenses, and to LIHEAP, which got the client caught up on existing home utility bills and on a regular program that will assist with future utility bills.

A second client, just 30 years old, was facing financial hardships after the devel-opment of a physical disability. The client’s connection to the BEC led to Social Security and disability benefits. Counselors carefully reviewed assets to ensure that the client’s benefits would not be jeopardized by the limited monthly income the client was still able to secure through light work.

Medicare/Health Insurance FraudEvery U.S. citizen or permanent resident over age 65 qualifies for Medicare, so there is rarely any need for a scam artist to research what private health insurance company older people have in order to scam them out of some money. In these types of scams, perpetrators may pose as a Medicare representative to get older people to give them their personal information, or they will pro-vide bogus services for elderly people at makeshift mobile clinics, then use the personal information they provide to bill Medicare and pocket the money.

Scammers like to take advantage of the fact that many people above a certain age own their homes, a valuable asset that increases the potential dollar value of a certain scam. A particularly elaborate property tax scam in San Diego saw fraudsters sending personalized letters to different properties on behalf of the County Assessor’s Office. The letter, made to look official but displaying only public information, would identify the property’s assessed value and offer the homeowner, for a fee, of course, to arrange for a reassessment of the property’s value and, therefore, the tax burden associated with it.
