Ventilator Request Guidelines

Please follow the listed steps to

request ventilator distribution

if you are in need of immediate

resource support.

It is the intent of Maricopa County Public Health to distribute ventilators in the most optimal, effective,

equitable, and efficient manner.

Please follow the steps below to submit your request:

1. Review CDC’s Strategies to Allocate Ventilators from Stockpiles to Facilities. This guidance details how ventila-tors will be strategically and ethically distributed among healthcare facilities in Arizona during COVID-19

2. Determine your need. Consider the number of and preferred type of ventilator. Please refer to the Ventilator Request Guidelines for a complete list of considerations.

3. Please submit a healthcare order form through the fol-

1. A ventilator request will be created on your behalf by MCDPH

2. MCDPH will assist you in procuring a ventilator from the following sources:

- Local Warehouse

- Hospital Partners
