National Credentialing Committee (NCC)
• Makes recommendations regarding credentialing decisions using a peer-review process.
• Provides oversight, meaningful advice, and decisions regarding credentialing, recredentialing, and ongoing monitoring issues.
• Membership includes representation from a range of participating behavioral health practitioners.
• Meets bi-weekly to reviews credentials for any practitioners who do not meet established criteria.

Why Credential our Network?
• Federal, state and accrediting body requirements.– Our National Credentialing Program is designed
to meet all standards and requirements to assure the overall quality of ValueOptions provider network.
– Consistent with the credentialing standards established by NCQA, ValueOptions recredentials participating practitioners every three years, including primary source verification of licensure, education, work history, and professional malpractice insurance.

Notification Process• ProviderPulseSM phone message four to five months prior to
recredentialing due date.
• Follow-up notification with information regarding how to access your online recredentialing application sent via e-mail or fax one week later. E-mails sent from [email protected] with subject of
“ValueOptions Alert for [Provider Name]” Fax transmissions have coversheet with ValueOptions logo and
provider name• Reminder sent at day 15 via e-mail or fax and at day 30 via US
Postal Service.
• Failure to respond will result in disenrollment from the network.

Review of Recredentialing Information Needed• Recredentialing Application Information
Demographic Information Practice and referral information License/Certification information Professional Malpractice Insurance information
• Completed Taxpayer Identification Number and Certification (W-9 form)
• Copy of current Professional Liability Face Sheet
• Copy of current State License/Certification
• Copy of current DEA/CDS and proof of Board Certification (if applicable)

Preparing for Online Recredentialing
• Attend this Online Recredentialing Webinar √• Register for ProviderConnect
Go to www.ValueOptions.com, choose “Providers”. Under ProviderConnect Login click “Register”
• Have electronic copies of your credentialing paperwork saved to your computer
Once you complete the above steps, the online recredentialingprocess will be fast, easy and convenient!

To access your application: Log into ProviderConnect and click Provider Data Sheet on the side navigation bar.

5. Enter your License/Certification Information and upload or fax a copy of your current certificate(s).

6. Upload or fax a copy of your current Malpractice Insurance face sheet and enter your insurance carrier information.

7. Enter/update your Work History information. Click Save & Next after all the necessary information has been entered.

9. If applicable, enter your Disability Provider Network Information. Click Save & Next to continue.

10. If applicable, enter your Fitness for Duty Assessment Specialist Network Information (FFD Specialist). Click Next & Save to continue.

11. Answer all Provider Profile questions by selecting Yes, No, or N/A. If the ‘Yes’ radio button is selected, a space will be provided for an explanation to be entered.

Responses are required for all Provider Profile questions before you can continue. If you fail to answer one of the questions, you will be prompted with the following message.
12. Enter the necessary information for the Substitute for Form W-9 Request for Taxpayer Identification Number.

13. Read the Attestation/Participation Statement, then select electronic signature option and follow instructions on each page OR print attestation page and follow the instructions to submit via fax.

To submit attestation via fax:Select “Check here if you intend to fax the Attestation form” box
Select Print “Click Here” option and fax your signed and dated attestation within 5 days to 1-866-612-7795.

Additional Tips & Reminders:• Check “Spam” blocker on your computer and allow
ValueOptions emails to be received into your Inbox• Electronic reminders about your recredentialing application will
be sent from [email protected]• ValueOptions requires that you submit current copies of your
license(s), CDS, DEA and malpractice face sheet. Please update previously scanned documents into the Online PDS system when asked to do so or submit via fax within 5 business days.
• Missing or incomplete information will result in the need for outreach by the Credentialing Specialist and will delay your recredentialing process.
