Rehabilitation and Empowerment

Introduction:

Government of Andhra Pradesh issued certain guidelines for evaluation of various disabilities and

procedure for certification to the persons with disabilities for concessions and other benefits and for

taking action for producing of false certificates in respect of the claims for reservation etc.

It is being implemented across the state of Andhra Pradesh by Society for Elimination for Rural

Poverty (SERP) in close coordination with the Women, Child and Disabled Welfare Department.

Objective

The objective is to create a Dynamic Web enable system for comprehensive access, rehabilitation and

empowerment, through automation, capacity building, assessment of persons with disabilities (PWDs)

and maintaining Decision Support System (DSS).

SADAREM aim for elimination of rural poverty.

Benefits

Scientific assessment of degree of the disability is done on the basis of methods and formulas

prescribed in the Gazette 2001 issued by the Ministry of Social Justice and Empowerment, Govt. of

India.

A computer generated disability certificate with a unique ID and identity card is issued to the

concerned person.

As per the rules, the persons who are adjudged to be disabled above 40% will be issued the Sadarem

certificates, and only those will receive monthly pension of Rs 1500.

The SADAREM software will generate all the necessary documents such as note orders, beneficiary

list, reports, pension certificate and intimation document and sanction proceedings to the individuals.

SADAREM Software also generates the complete details including note orders, beneficiary list and all

necessary documents that the PWDs candidates are entitled.

The database hosted in the online portal to reach out to the PWDs candidates.

Objective of the SADAREM initiative is to create a Dynamic Web enable system for comprehensive

access, rehabilitation and empowerment, through automation, capacity building, assessment of persons

with disabilities (PWDs) and maintaining Decision Support System (DSS).

Government of Andhra Pradesh has taken this initiative of Identifying & Creating the Centralized

Database for Person with Disabilities by using scientific approach & disability guidelines.




SADAREM ICT solution is designed to cover the following features:

Generation of a computer based Disability Certificate with unique ID along with Identity Card.

Assessment of needs and maintaining the centralized data base. Software will also generate all the

details including the support services that the disabled persons are entitled, based on the need

assessment and a record of the services provided from time to time.

The database thus generated will be hosted in the Public Domain to enable service providers to

reach out to the disabled persons.

User (Disability person) Registration process

Firstly User(Disability person) needs to complete the registration and slot booking process in

Meeseva.

After successful registration, a 17 digit sadarem id will be generated which is based upon the

District, Mandal, Panchayat , village and Habitation.

In registration process the type of disability for the user will be identified in Meeseva.

The Types of disabilities are:-

1. Loco motor/OH

2. Visual Impairment

3. Hearing Impairment

4. Mental Retardation

5. Mental Illness

6. Multiple Disability

Based on the disability percentage the User can be categorised in to

1. Eligible

2. Rejected

3. Assessment Reject.

Process Flow

Depending upon the disability, Users (Disability persons ) need to fix the Appointment with

Doctors assigned to the Sadarem.

Then doctors will login through Sadarem website and check their Assigned users(Disability

persons) for the particular day.

Here 3 Doctors will be updating FascimileSignature and the process will start from Doctor 1,

Doctor 2 and end with Doctor3.




Meeseva Registration/Slot booking process.

Step 1

After opening the URL the below screen will be dispalyed.




Step 2

Enter the Login credentials and click on Login button as shown in below screen.




Step 3

After clicking the login button , the below screen will be displayed.




Step 4

Click on the Rural Development in List of Services and click on Request for SADAREM Certificate slot

booking link as shown in below screen.

After Clicking on Request for SADAREM Certificate slot booking link , the below screen will be

displayed. Here enter the Aadhaar number and click on submit button.

Step 6

After entering the Aadhaar number and clicking the submit button, the following page will be displayed

and fill all the details and click on Submit button as shown in below screen.




Step 7

After clicking the Submit button, a 17 digit sadarem id will be generated as shown in below screen and

click on Upload and Acknowledge buttons.




Step 8

After generating the sadarem id click on Acknowledgement, then the below screen will be displayed

After generating the sadarem id click on Upload photo , then the below screen will be displayed




Doctors Login

Doctor 1 Login

Step 1

Open the Sadarem website , the below screen will be displayed.

Clicking on Login button will display the following screen.




Step 3

Enter the Login credentials and click on Login button as shown in below screen .




Step 4

After clicking on Login button , the following page will be displayed, Click on Facsmile Signature link.

Step 5

After Clicking the Facssmile signature link the following page will be displayed, Click on Update button.




Step 6

After clicking on Submit button , the following page will be displayed, click on Sign button.

After Clicking the Sign button Doctor 1 can update his SDK signature as shown in below screen.

After updating the Doctor 1 SDK signature, Click On Assessment as First doctor as shown in below

screen.

Assigned Users to Doctor 1 can be selected as shown in below screen.

After the selecting the User then below screen will be displayed, their enter all fields and click on submit

button as shown in below screen.

After clicking the Submit button the following page will be displayed. Enter all the Fields and click

finally Update button as shown in below screens.

Click on Next button button as shown in below screen.

After clicking the next button the following page will displayed, click on update button.

After clicking on Update button the following screen will be displayed, click on next button.

After clicking on next button following page will be displayed, click on Update button.

Select all the fields and click on Update button button.

After Updating , Confirmation message will be displayed and click on Sign certificate as shown in below

screen.

The following page will be displayed after clicking on sign certificate and click on sign button.

After clicking on Sign button confirmation will displayed as Success for Doctor 1 DSK signature as

shown in below screen.




Doctor 2 Login

Step 1

Login with Doctor 2 login select select DSK signing as Doctor 2 as shown in below screen.




Step 2

Click on View button as shown in below screen.




Step 3

Select all fields and click on View button as shown in below screen.

Step 4

The following screen will be displayed after clicking the View button as shown in below screen.




Step 5

Click on Sign button as shown in below screens and Doctor 2 DSK signature will be updated

successfully.




Doctor 3 Login

Step 1

Login with Doctor 3 and click on DSK signing as third doctor as shown in below screen.

Click on View button as shown in below screen.




Step 3

Select all the fields and click on View button as shown in below screen.

Step 4

After clicking on View button the following screen will be displayed.




Step 5

Click on Sign button as shown in below screens , then Doctor 3 DSK signature will be updated

successfully as shown in below screen.




Print SADAREM Certificate

Step 1

Click on Print SADAREM Certificate in Rural development as shown in below screen.




Step 2

Enter the Sadarem id in the field and click on Get details button as shown in below screen.




Step 3

The following screen will be displayed after clicking on Get details button and click on confirm

payment button .




Step 4

The following confirmation message will be displayed after clicking the Confirm payment button

as shown in below screen and click Ok button.




Step 5

The following page will be displayed after clicking the Ok button.




Step 6

Enter all fields and click on print certificate button.




Step 7

Finally the Certificate will be downloaded after clicking on print certificate button as shown in below

screen.
