2015 NATIONAL TEAM U.S. Paralympics will name a Cycling National Team(s) comprised of the following:

National A Team | National B Team | National Development Team Term #1 (January 1 – June 30, 2015) Each team will be nominated based on athlete performances from the following events:

• 2014 UCI Para-cycling Track World Championships o Individual Pursuit o 500m Time Trial (women) o 1000m Time Trial (men) o Team Sprint (if applicable)

• 2014 UCI Para-cycling Road World Championships o Individual Time Trial o Road Race o Handcycle Relay (if applicable)

• 2014 UCI Para-cycling Roald World Cups o Individual Time Trial o Road Race o Handcycle Relay (if applicable)

Each athlete will be given a ranking (Attachment J) based on points accumulated during the 2014 season. Points earned are based on the “Points Scale” table in Attachment I.

• Athletes who earned 50 points or higher will be nominated to the National A Team effective January 1, 2015.

• Athletes who earned between 30 and 49 points will be nominated to the National B Team effective January 1, 2015.

• Athletes who earned between 16 and 29 points will be nominated to the National Development Team effective January 1, 2015.

Term #2 (July 1 – December 31, 2015) Each team will be nominated based on athlete performances from the following events:

• 2015 UCI Para-cycling Track World Championships o Individual Pursuit o 500m Time Trial (women) o 1000m Time Trial (men)

• 2015 UCI Para-cycling Roald World Cups #1 and #2 o Individual Time Trial o Road Race o Handcycle Relay (if applicable)

Athletes who are high performers in races at the events listed above will be named to National team, as follows:

• Athletes with a 4th place finish in a race listed above with seven or more entrants, will be nominated to the National Development Team for Term #2.

• Athletes who win one medal in race listed above, will be nominated to the National B Team for Term #2.

• Athletes who win two medals in races listed above, will be nominated to the National A Team for Term #2, provided one (or more) of the medals was in an individual event.

2015 National A, B, and Development Team Minimum Performance Standards

Athletes nominated to the National Team are required to maintain minimum performance standards during Term #1 and Term #2 in order to maintain team status. Athletes who do not meet the minimum performance standards during Term #1 will be moved one level lower effective July 1, 2015, or – for athletes on the Development Team – will be removed from the team effective June 30, 2015. Minimum performance standards for each athlete will be outlined in their individual Athlete Performance Plan, which will be signed by the following parties:

DISCRETIONARY NOMINATION Discretionary nomination to the 2015 National Team(s) may be used if an athlete who has demonstrated international competitiveness during any of the qualification periods for the 2015 National Team(s) was ill, injured, or otherwise unable to compete or perform at a level to qualify him/her for the team; but it is expected that the athlete will return to internationally competitive form during the term to which they are named. Discretionary nomination may also be used if the athlete demonstrates a trend of improving performance in national or international competition, that when extended to a reasonable distance into the future, intersects the current international performance standards for a top three (podium) finish. 2015 NATIONAL TEAM STATUS U.S. Paralympics 2015 National Team status applies from the date the athlete is named (January 1, 2015 or July 1, 2015) through December 31, 2015, provided the athlete meets the term of the Athlete Performance Plan. National Team status and benefits are only conferred upon athletes who accept, sign and remain current with the U.S. Paralympics National Team Athlete Agreement. Athletes who meet National Team qualifying criteria during 2015, become eligible for National Team benefits beginning July 1, 2015 and will retain their National Team status through December 31, 2015. Athletes nominated to the 2015 National Team(s) will be added to the USADA Registered Testing Pool and will be responsible for remaining compliant with all forms, updates and tutorials as required by USADA. More information regarding USADA can be found in Attachment E. Athletes nominated to the National Team(s) must be internationally classified and hold a Paralympic-eligible sport class as per UCI master classification list and hold a current International Racing License for Cycling from USA Cycling. 2015 National Team Standards (Attachment A)

ROAD 2015 National Team Road standards are created using the following system: Men: Average of 3rd place finisher’s individual time trial result for each sport class at the following events:

• 2012 Paralympic Games • 2013 UCI Para-cycling Road World Championships • 2014 UCI Para-cycling Road World Championships

Women: Standards are calculated at 116% of the men’s standard for each respective sport class. This calculation is used to ensure equity across all women’s classes due to limited field sizes.

TRACK Men & Women: 2015 National Team Track standards are based on the average of the 3rd place finisher’s results for each sport class from the following events:

• 2012 UCI Para-cycling Track World Championships • 2012 Paralympic Games • 2014 UCI Para-cycling Track World Championships

Road and Track Standards may be utilized to evaluate athletes as part of their Athlete Performance Plan. Meeting a National Team standard does not imply National Team naming or nomination. 2016 NATIONAL TEAM U.S. Paralympics will nominate a National Team(s) for 2016, based on criteria outlined in the 2016 Sport & Program Plan. For the first term of 2016 the team will be named using a ranking list based upon the system used in Attachments I and J. Specific events will be included in this section of the plan when U.S. Paralympics receives confirmation from the UCI that the 2015 World Championships and World Cup calendar has been finalized. NATIONAL TEAM BENEFITS National Team athletes may be eligible for the following benefits:

1. USOC Direct Athlete Support (Attachment B) 2. Cycling Additional Athlete Support Programs (Attachment C) 2. USOC Elite Athlete Health Insurance program (Attachment D) 3. Air transportation (or ground equivalent), lodging, ground transportation, meal per diem,

and coaching support at designated U.S. Paralympics training and competition activities. 4. Hilton Hotel Discount: Hilton Hotel offers a discount for athletes staying at their

properties. Rooms can be booked by using the Hilton Promotion/Offer code 2637677 on www.hilton.com or by going to: www.hhonors.com/teamusastays.

5. Merchandise discounts: Athletes that stay at the Olympic Training Center will be given a discount of 15% off of all merchandise. Simply present the OTC ID to receive this discount.

6. Airline Discount: United Airlines offers a “friends and family” discount for USOC meetings and events. Athletes, Friends and Family paying their own way to travel to a USOC function can contact the United Airlines Olympic Travel Desk (1-800-841-0460), and request that the reservation agent apply the USOC discount when paying with their personal credit card.

12. U.S. Paralympics National Team uniform: National Team athletes will be provided cycling competition and casual apparel for training and competition. Guidelines for apparel are outlined in the U.S. Paralympics Cycling National Team Uniform Agreement (Attachment K) which all National Team athletes must sign and adhere to.

Athletes who meet the Emerging standard during a 2015 USA Cycling or UCI sanctioned event, may be invited to participate in selected U.S. Paralympics Cycling Program activities as outlined below. Invitation is at the discretion of the Director, Paralympic Cycling High Performance. 2015 Emerging standards (Attachment A) are based on 115% of the National Team standard. Athletes invited to official team events or activities must be internationally classified by the UCI OR nationally classified by U.S. Paralympics and hold a Paralympic-eligible sport class as per the UCI and/or U.S. Paralympics master list and hold a current season license for Cycling from USA Cycling. 2015 PROGRAM ACTIVITIES The 2015 program activities are outlined on Page 2. Athletes will be required to attend all 2015 activities included in their Athlete Performance Plan which will be developed by the U.S. Paralympics Cycling coaching staff in collaboration with the athlete’s personal coach. All athlete performance plans will be approved by the Director, Paralympic Cycling High Performance. Each athlete is responsible for individual arrangements (including expenses) to attend these activities (i.e. travel, lodging, accommodation, entries) unless otherwise indicated in the activity listing. National Team athletes are required to obtain a 2015 USA Cycling International Competition License. Camps Consideration for attendance at camps listed on page 2 will be determined by the Director, Paralympic Cycling High Performance in consultation with the U.S. Paralympics Cycling coaching staff. Competitions For UCI international competitions in 2015, athletes will earn Team USA roster nominations (regardless of National Team status) based on the published selection criteria for the event. Competitions that are Nations Events (open only to athletes nominated to the official Team USA roster by U.S. Paralympics), or where U.S. Paralympics will enter athletes as part of the National Team, are indicated by a Team USA annotation on the event schedule on page 2.

2015 OPERATION GOLD U.S. Paralympics will award Operation Gold funds in cycling based on the results at the 2015 UCI Para-cycling Track World Championships as outlined below AND the 2015 UCI Para-cycling Road World Championships.

In the Olympic/Paralympic year, multiple Operation Gold Awards will be paid to qualifying athletes. In non-Olympic/Paralympic years, Operation Gold Awards are limited to one award per athlete in a program year. Athletes who qualify in multiple events during a non-Olympic/Paralympic year will only receive the highest award amount for which he/she qualified at the specified competition. For Paralympic sports in non-Paralympic years, individuals must finish in one of the top six (6) places and among the top 50% of the athletes who started the event to qualify for Operation Gold Awards. Teams must finish in one of the top four (4) places to qualify for Operation Gold Awards.

Attachment B

2015 U.S. Paralympics Cycling Direct Athlete Support

1. DAS is processed monthly on the first Friday of each month. 2. Athletes are eligible for DAS at the beginning of the month following nomination to the

National Team(s). 3. DAS is provided to athletes that are in compliance with 2015 Athlete Agreement

obligations and personal performance plan obligations. If an athlete is not current with the agreement/plan obligations and/or USADA on the first day of the month in which the payment is being made, the athlete’s payment will be withheld until he/she is compliant.

4. U.S. Paralympics will provide DAS for a multi-sport athlete in ONLY one sport. In conjunction with the respective NGB/HPMO High Performance Director(s) the multi-sport athlete shall designate the primary (“A”) sport in which they wish to receive DAS.

5. Athletes sanctioned by USADA, WADA and/or the respective sport IF for a doping violation are not eligible for DAS, regardless of any National Team(s) status, during the period of such sanction.

6. Athletes sanctioned by the IPC and/or the respective sport IF for classification Intentional Misrepresentation are not eligible for DAS, regardless of any National Team(s) status, during the period of such sanction.

7. DAS will only be paid once the Athlete Support Designee Form has been completed and submitted (annual basis).

8. Athlete support is paid directly to the athlete. National Team athletes are responsible for understanding and maintaining their own eligibility status with other organizations where applicable (i.e. NCAA, NFHS, etc.), and may choose to decline direct athlete support payments in order to retain high school or collegiate eligibility.

9. Athlete support is contingent upon athletes adhering to the National Team agreement and their individual athlete performance plan.

2015 U.S. Paralympics Cycling Additional Athlete Support Programs

Coaching U.S. Paralympics Cycling will provide individual coaching support for National Team athletes in 2015. All athletes must communicate their preferred personal coach to the Director, High Performance Paralympic Cycling upon being named to the team. All coaches must hold a USA Cycling Level 1 or Level 2 certification, or hold a UCI Coaching Diploma. Agreements with all coaches (including payment terms) will be negotiated by the Director, High Performance Paralympic Cycling. Supplemental Athlete Support National Team Athletes will receive a stipend for the year, to support expenses such as: equipment, travel, training and supplemental coaching.

*Athletes who are named to the team, or who change team status level on July 1, 2015 will receive a pro-rated SAS allocation The 2015 procedure for Supplemental Athlete Support will be included in this section of the plan in early January, 2015.

Performance Incentive Support Additional performance incentives will be provided to athletes who attain results at UCI Para cycling C1 events and Road World Cup events – including results for team competitions – when competing as a member of Team USA*.

OTHER SUPPORT Equipment National Team athletes may have access to National Tteam equipment, including but not limited to:

• Bicycles/trikes/handcycles • Components and wheels • Power meters • Specialty helmets • Race-specific competition apparel • Trainers and rollers

All National Team equipment will be inventoried and distributed based on a) Team status, and b) medal potential. Equipment will be issued at the discretion of the Director, Paralympic Cycling High Performance in consultation with the U.S. Paralympics Cycling coaching staff. All National Team athletes using equipment owned by U.S. Paralympics will be required to sign the National Team Equipment Agreement Form (Attachment L) and will be responsible for any damage or loss to the equipment. Special Pricing National Team athletes may have access to special pricing from official partners and suppliers of U.S. Paralympics Cycling. Details and policies will be communicated to athletes on a case-by-case basis, by the Director, Paralympic Cycling High Performance throughout the 2015 season.

2015 U.S. Paralympics Cycling Elite Athlete Health Insurance (EAHI) The USOC agrees to provide up to 16 EAHI slots to athletes who meet the eligibility and criteria. Athletes who are approved for EAHI in January 2016 will retain their EAHI through 12/31/2015 provided they maintain National Team status. The approved EAHI eligibility and criteria for EAHI is as follows: Athletes must be currently training and competing to receive EAHI and they must sign the 2015 Athlete Agreement and Athlete Support Designee Form.

• Criteria 1: All 2015 National Team Cycling athletes are eligible for EAHI. • Criteria 2: The top 16 athletes per the ranking list in Attachment J will be offered EAHI

spots first. • Should there remaining slots available after Criteria 1 and/or Criteria 2, the decision to

award EAHI will be based on which athlete(s) have the best opportunity to medal at the Paralympic Games in 2016.

• Should there be any available slots after all eligible athletes who requested and have been awarded EAHI, those slots will remain unused.

MEDEX Insurance Every athlete enrolled in the Elite Athlete Health Insurance Program also receives MEDEX Plus as part of the benefits. MEDEX Plus provides:

• Medical Assistance Services (i.e., locate nearest doctors or hospitals, facilitate hospital payments, and monitor case) **

• Travel Assistance (i.e., replacement of lost/stolen travel documents, translation services, travel arrangements) **

• Destination Services (i.e., pre-travel information, travel and health information, real-time security intelligence) **

• Personal Security Services (i.e., political evacuation, security evacuation) ** • Medical Evacuation - In the event of a medical emergency, MEDEX will arrange & pay

for a medically supervised evacuation to the nearest medical facility or return to point of origin.

The United States Olympic Committee (USOC), National Governing Bodies (NGBs) and the World Anti-doping Agency (WADA) Code have authorized USADA to test any athlete who: • Is a member of a license holder of a NGB • Is participating at an event or competition sanctioned by the USOC or a NGB or participating

at an event or competition in the United States sanctioned by an International Federation (IF) • Is a foreign athlete who is present in the United States • Has given his/her consent to testing by USADA or who has submitted a Whereabouts Filing

to USADA or an IF within the previous 12 months and has not given his/her NGB written notice of retirement

• Has been named by the USOC or an NGB to an international team or who is included in the USADA Registered Testing Pool (USADA RTP) or is competing in a qualifying event to represent the USOC or NGB in international competition

• Is a U.S. athlete or foreign athlete present in the United States who is serving a period of ineligibility on account of an anti-doping rule violation and has not given prior written notice of retirement to the his/her NGB and USADA or the applicable foreign anti-doping agency or foreign sport association

• Is being tested by USADA under authorization from the USOC, an NGB, IF, any NADO, WADA, the International Olympic Committee (IOC), International Paralympic Committee, (IPC), or the organizing committee of any event or competition.

Some of these athletes may be added to the USADA RTP if they meet NGB, USOC, or USADA criteria. Athlete Career Services The Athlete Career Program was created by the USOC Career Services Department in partnership with Adecco, the USOC’s Official Career Management Services Sponsor. The Athlete Career Program provides a variety of services to elite athletes who are still training and competing as well as Olympians and Paralympians who have retired from sport. See attached brochures for more information. Programs and services include the following:

• Personal career assistance – work with your own career coach to assist you with everything from creating your personal career game plan, to creating a resume, to getting the job you want.

• Job placement assistance – whether you’re looking for a part-time, flexible scheduled job,

full-time employment, or an internship or job shadowing opportunity, Adecco can help you find what you need.

• Career management seminars – these seminars are broken into five topics that can be

delivered anywhere and anytime to fit athletes’ schedules. The Team USA Career Program Eligibility Criteria for Olympians, Paralympians and Hopefuls

Purpose The Team USA Career Program was created by the United States Olympic Committee (USOC) to assist Olympians, Paralympians and Olympic and Paralympic hopefuls with obtaining part-time, flexible career-oriented employment to offset training and competition expenses while also building work experience for their future. Team USA Career Program Opportunities The Team USA Career Program is in its early stages and is anticipated to grow over time. Currently, there are limited employment opportunities with USOC Sponsor, Hilton Hotels, Inc. Eligibility Criteria

1. Only athletes who are currently training and competing in Olympic and Paralympic events/disciplines will be considered for eligibility into the Team USA Career Program. 2. Athletes participating in Olympic/Paralympic events/disciplines must be a member of the U.S. National Team (senior level), or the equivalent, if the athlete’s sport does not have a National Team. *There may be cause for an exception to the Team USA Career Program eligibility criteria because of an extenuating circumstance where a deviation from the criteria best meets the needs of the athlete and NGB or the Team USA Career Program employer. These deviations will be handled on a case-by-case by the USOC’s Athlete Support Working Group, the athlete’s NGB and the Team USA Career Program employer. ** This eligibility criteria will be reviewed at least once per quadrennium by the USOC (with input from the Athlete’s Advisory Council (AAC) and NGB Council) to ensure fairness and consistency. Allocation of Team USA Career Program Opportunities By meeting the above eligibility criteria, athletes are qualified for consideration for an employment opportunity with Hilton or any other participating USOC Sponsor-Employer in the Team USA Career Program. Meeting the athletic eligibility criteria established by the USOC and U.S. Paralympics does not guarantee a position within the Team USA Career Program. Hiring decisions will be made independently by the Team USA Career Program Employer with information provided by the USOC and Adecco, the USOC’s Official Career Transition Management Sponsor and Administrator of the Team USA Career Program. Annual Eligibility Review Process

• Summer sports will be reviewed annually in the October/November timeframe to verify they still meet the Team USA Career Program Eligibility Criteria.

• Winter sport athletes will be reviewed annually in the March/April timeframe to verify they still meet the Team USA Career Program Eligibility Criteria.

Annual Eligibility Review Process (continued) Athletes no longer meeting the athletic eligibility criteria will be removed from the Team USA Career Program by the USOC. During these review periods and anytime throughout the athletes’ employment, the USOC sponsor-employer may terminate the athlete’s employment if he or she fails to meet mutually agreed upon and established work criteria. Athletes are expected to keep working in their Team USA Career Program position until they are notified by the USOC, Adecco and/or their employer that they no longer meet the eligibility requirements. Anti-Doping Refer to USOC Suspension of Benefits Policy. Code of Conduct Sign and abide by the code of conduct established by the USOC, when required. Sign and abide by the code of conduct established by the athlete’s National Governing Body and approved by the USOC.

Attachment H 2015 U.S. Paralympics Cycling Resident Program

U.S. Paralympics Cycling will conduct a Resident Program at the Colorado Springs (CSOTC) and Chula Vista (CVOTC) Olympic Training Centers. The core of the program will be at the CSOTC beginning January 2015, and continuing through the 2016 Paralympic Games. Athletes accepted as part of the program may reside in dormitories at the CSOTC and/or off campus at the CS or CSOTC. Specific training and competition will be geared to individual athlete needs, but generally will include 8-14 hours per week of training with other resident athletes, or independently.

Eligibility:

• Be at least 18 years of age (UCI Minimum racing age for Para-cycling). • Be a current U.S. citizen eligible to represent the USA in the Paralympic Games. • High school age applicants must be enrolled in high school and maintain a 2.7 GPA. Program

staff will strongly recommend online courses or enrolling in private school to best facilitate both school and training requirements.

• Applicants who take college courses must maintain a 2.0 GPA. • Be willing to submit to a complete health profile coordinated by USOC Sports Medicine

including thorough medical history and comprehensive physical examination. Requirements: (including but not limited to)

• Commitment to reside in Colorado Springs from January 1 – December 31, with a review every six months and renewal application each calendar year

• Strict adherence to the Athlete Performance Plan • Sport science testing • Sport psychology sessions • Nutrition counseling • Community outreach and involvement • Media training

Acceptance and Application Acceptance to the Resident Program will be at the sole discretion of U.S. Paralympics and all positions available in the Resident Program will not necessarily be filled. Evaluation of athletes for acceptance to the Resident Program will consider a wide range of factors including:

• Space availability. • Meeting and maintaining a minimum emerging standard (Attachment A) per the athlete’s specific

event/sport class. • Athlete’s current “home” training environment. • Potential for the athlete to medal at the World Championships/Paralympic Games. • Projected impact of the Cycling Resident Program on the athlete’s medal potential. • Athlete’s desire and willingness to commit fully to the Resident Program training. • Training compatibility with other resident athletes. • Relevant performance data from current quadrennium

The application and all necessary forms for the Resident Program are available at www.usparalympics.org. Any athlete who wishes to participate must apply and be accepted into the Resident Program. Coaching Staff U.S. Paralympics Cycling resident athletes will be coached by the U.S. Paralympics Cycling coaching staff.
