UPMC HealthyU is a unique health insurance plan that rewardsyou for making healthy choices. As a member, you can earnreward dollars in a health incentive account (HIA) by completinghealthy activities throughout the year. UPMC HealthyU recommends healthy activities that are uniquely customized just for you, each with a reward dollar value to help you focus on what’s most important to understanding and improving your own health. The money you earn in your incentive account will automatically be applied to out-of-pocket medical costs, like your deductible,* coinsurance, and prescription copayments. Keep in mind, however, that there is a brief delay between when you complete your activity and when your reward dollar credit shows up in your incentive account.

Log in to MyHealth OnLine, the website that powers UPMC HealthyU. You can complete your MyHealth Questionnaire, a confidential health risk assessment. Your responses will help generate your personalized list of healthy activities. FEHB members can earn up to $250 for self-only coverage, or $500 for self plus one or family coverage. MyHealth OnLine is your confidential health resource where you can view your list of healthy activities, track the balance of your HIA, find doctors and pharmacies, and discover tools and programs that will keep you on track with your health goals. Use this guide to learn about some of the healthy activities you’ll find there, complete with brief descriptions and the reward dollars you’ll earn by completing them.

MyHealth Questionnaire – $50 This confidential health risk assessment, powered by WebMD®,is an online survey you take once a year. In as little as 20 minutes, the results help you understand your health status. After you complete it, you receive a customized roadmap to better health.

All enrolled employees and spouses can complete an assessment for reward dollars. Each assessment is worth $50 if completed in the first 90 days of the benefit year (or your effective date if you join the plan later) and $25 after that.

Health screening (routine blood work) – $30We call this important activity “Know your numbers!” And in this case, knowledge is power and better health. The screening will check your cholesterol and glucose levels. It may also check other important measures like blood pressure, BMI,

height, and weight. The screening can be done at your doctor’s office, a lab, or at certain convenience care clinics. You will receive $15 for each screening, up to a maximum of $30 for one year.

Personal Health Review – $15Have some questions regarding your MyHealth Questionnairerecommendations or the results of your biometric screening?A UPMC Health Plan health coach will speak with you for 5 to 10minutes to answer your questions. You’ll earn up to a maximum

of $30 per year (two sessions) for an individual, or $60 per year (four sessions) for a family. With a family plan, the subscriber or spouse can earn the full $60.

Beating the Blues Online Program – up to $130Are you feeling stressed or anxious? Take control of your mood with the Beating the Blues USTM online program. Throughout this eight-session, confidential program, you will learn step by step how your thoughts and feelings are connected. You will

set individual goals, follow scenarios of people with similar problems to your own, and practice new skills throughout the program.

Health coaching: Telephone-based lifestyle programs – up to $110 A health coach is your personal trainer for making lifestyleimprovements. Whether you want to lose weight, be morephysically active, improve nutrition, reduce stress, or stopsmoking, your health coach will help you achieve your goal.You’ll have weekly phone conversations with your health coach to help you stay accountable, plus a workbook that helps

keep you on task. The programs last from four to eight weeksdepending on the program you choose, with follow-up phonecalls from your health coach at 30, 90, and 180 days. You canenroll in only one program at a time, and you cannot enroll in the same program twice in one year.

Health coaching: Online/self-guided lifestyle programs – up to $110Each of our lifestyle improvement programs is designed to help you achieve a new health routine. Coach on Call – $15 eachIf you’re not ready to commit to an entire lifestyle or conditionmanagement health coaching program, you can participate in Coach on Call. You talk with a health coach over the phone for 5 to 10 minutes and then receive an email with a link to resources specifically designed to help you meet your health

goals. You can read through the resources at your leisure. You’ll earn up to a maximum of $60 per year (four sessions) for an individual, or $120 per year (eight sessions) for a family through Coach on Call. With a family plan, the subscriber or spouse can earn the full $120.

Healthwise Conversations – $15 eachHealthwise® Conversations use storytelling and animation to help you “get smart” about lifestyle topics like healthy weight and eating, positive thinking, and getting active. Healthwise Conversations can act as a convenient virtual health coach,

dynamically teaching you about a health topic and helping you develop a plan for improvement. You can earn $15 per session, for the first four videos you watch. There is a maximum of $60 per individual plan and $120 per family plan.

MyHealth Selections – $75 If you are ready to make a change in your life but your goal doesn’t fit into any of our program areas, a health coach can tailor a program for your specific needs. We can even tailor three or four of our lifestyle programs into one. As with the

other health coaching programs, you’ll talk regularly with a health coach over the phone to help you stay accountable, and you’ll receive customized material that complements your health coaching.

Emmi® Decide – $25 These 20-minute interactive videos can help you learn more about your health condition and treatment options. The videos explain your alternatives, discuss risks, and ask questions about your preferences so that you and your doctor choose the

treatment plan that is best for you. The Emmi Decide video programs allow for a $50 reward dollar maximum for individuals, and a $100 maximum for families. For a full list of programs, visit MyHealth OnLine.

Well-visits and immunizations – $50These activities are extremely important for keeping you healthy because they enable you to head off trouble before it starts. You receive a maximum of $50 per person per year

for all well-visits and immunizations. This includes routine physicals and gynecological visits. The exams must be coded by your physician as “preventive” exams to qualify.

Health screenings – $50 Regarding the screenings listed below: Receive reward dollars for one screening per year. Screenings can help lower your chronic disease risk. They also help you detect problems early, when they are far more treatable. Your doctor can assist you in

determining what screenings you need and when. Remember, however, that the screening must be coded by your doctor as a “preventive” service. Please consult with your doctor to determine which screenings are appropriate for you.

Flu shot – $25 Getting a flu shot is easy and convenient. Flu shots areavailable at your doctor’s office, many pharmacies, convenience

Dental and vision If your dental or vision benefits are provided by another insurance carrier, you will need to submit a health incentive account submission form to receive your reward credits. The form is available by logging in to MyHealth OnLine. Click on “Self-Service Tools” and then click on “I want to see all forms.” Remember that dental health is crucial to your overall health,

and getting regular cleanings can prevent painful and expensive repairs. Periodic eye and vision examinations are also an important part of health care. This form is available by logging in to MyHealth OnLine. Click on “Self-Service Tools” and then click on “I want to see all forms.”
