• Spouse or Domestic Partner and their children
under 26 years of age who are not self-
supporting and who live with the Insured or who
are a full-time or part-time student.
• The Named Insured may also cover a
dependent child to the end of the year in which
the Dependent reaches age 30 under certain
circumstances.

Effective and termination dates
• The student insurance plan is effective on August 18, 2010.
Your specific coverage is effective on the first day of the period for
which premium is paid or the date the enrollment form and full
premium are received by UnitedHealthcare, whichever is later.
• Coverage ends on August 17, 2011
Coverage terminates on that date or at the end of the period
through which premium is paid, whichever is earlier.
• Dependent coverage will not be effective prior to that of the
Insured student or extend beyond that of the Insured student.
• Refunds of premiums are allowed only upon entry into the
armed forces.

Plan 1 Benefits Overview
• Up to $250,000 benefit maximum, per insured, per policy year
• In network :
deductible $50 for each injury or sickness, up to $250 max
deductible per policy year
co-insurance of 90% until the insured has paid $10,000. Payment
will be made for 100% of additional covered medical expenses not
to exceed the $250,000 maximum benefit.
• Out of network : deductible $50 for each injury or sickness, up to $550 max
deductible per policy year
co-insurance of 70% until the insured has paid $20,000. Payment
will be made for 100% of additional covered medical expenses not
to exceed the $250,000 maximum benefit
• PPO network is UnitedHealthcare Choice Plus

Plan 1 Benefits Overview
• Inpatient benefits including hospital, newborn care,
physiotherapy, surgeon and assistant surgeon fees
• Outpatient benefits including day surgery,
anesthetist, medical emergency expenses,
diagnostic x-ray and lab services
• Other benefits including ambulance, alcholism/drug
abuse treatment, urgent care and walk in and adult
wellness.

• USF student insurance includes access to
UnitedHealthcare’s Choice Plus network
You can search for network providers at
www.uhcsr.com/usf
• Physicians and facilities in the network have agreed
to provide services to University of South Florida
students for a discounted rate
• Using a network provider is recommended since
service is provided at the discounted rate and the
student insurance plan will pay for a larger
percentage of the eligible service received
If you use out of network providers, the cost of your service
will be greater, and the student insurance plan percentage
paid will be less

provider of your choice. To
maximize your benefits and
minimize out-of-pocket cost, go
to the USF Student Health
Services (SHS)
• SHS Hours: Monday-Friday 8:00
a.m. to 5:30 p.m.
• After hours, students are referred
to in-network local after care
offices.
• Dependents are not eligible to
seek care at the SHS
• Always show your student
insurance I.D. card when getting
care
