Instructions for filling out the FORMAL ADMISSION application.

1. Print your Name, SUID, Major, Shippensburg University Email, Advisor Name, and Phone Number.

2. Using the Student Success Platform in the Myship portal, verify your total earned credits on the OVERVIEW tab. Write in the number of Total Credits

earned in the Block A row Criteria/Documentation column. Initial the corresponding Student Initial Box.

3. Using the Student Success Platform in the Myship portal, verify your Cumulative GPA on the OVERVIEW tab. Write in the Cumulative GPA in the Block B

row Criteria/Documentation column. Initial the corresponding Student Initial Box.

4. Using Degree Audit or the Student Success Platform in the Myship portal, verify your Basic Skills requirements status.

a. If you are using Degree Audit you will need to locate the Basic Skills requirement field. If you have met the requirements you will see a green

check box that indicates the Basic Skills requirement is satisfied. If you have not met the requirement it will show as an empty red box and say

Student must complete Basic Skills requirements at 60 credits.

b. If you are using the SSC Platform you will need to click on the CLASS INFO tab. Once the tab opens you will need to scroll to the bottom of the

page and in the High School/Pre-College section you will need to verify that the Basic Skills Met indicator is showing. You should see a score of

1 beside the indicator. If you do not see the Basic Skills Met indicator that means you have not fulfilled the requirement.

c. Check the appropriate line in the Block C row Criteria/Documentation column that applies to how you have fulfilled the Basic Skills

requirement. If you are unsure about how you have fulfilled the requirement, when you meet with your advisor you can confirm the method in

which you satisfied the requirement.

d. Initial the corresponding Student Initial Box.

5. Verify that your Clearance Packet is ENTIRELY updated and valid with the Office of Partnerships, Professional Experiences, and Outreach. Entirely means

that each clearance, TB Test, and Liability Insurance is valid. In the Block D row on the Criteria/Documentation column check each line once you have

verified that document is valid. Initial the corresponding Student Initial Box. Applicant will need to bring their copy of the completed clearance packet to

their meeting with the advisor.

6. Review the Gates and Status Levels Matrix to continue to familiarize yourself with the Status Level requirements moving forward.

7. Sign and Date the Student Signature fields.

8. Take your application to your Academic Advisor and review the application. You and your Advisor should again visit the SSC Platform and Degree Audit to

verify the program coursework and verification by the advisor should be made in regards to the required 6 credits for English (3 Writing, 3 Literature) & 6

credits for Mathematics. As you work through the verification together the Advisor will initial each Advisor Initial Box next to the applicant’s initials.

a. The Advisor will check the box recommending Formal Admission. The Advisor will fill in the Teacher Preparation program in which they

recommend you be admitted. The Advisor must Sign and Date the Advisor Signature fields.

b. If the Advisor does not recommend the Formal Admission, an immediate action plan must be formed by the Applicant and Advisor. The

Associate Dean’s office must be notified for approval.

9. Student will then hand deliver the completed application/or action plan to Shippen Hall Room 303 or Shippen Hall Room 356.

10. Application will be reviewed by the Teacher Education Council and a decision will be made.

11. Student & Dept. Chairs will receive an email confirming or denying Formal Admission. There will be another indicator used in Degree Audit and the SSC

Platform that will allow the applicant to see that they have been formally admitted into their program.
