Triggers in Asthma

Most people with asthma can think of several things that “trigger” their asthma or make it worse. Knowing what triggers your asthma means you can:

• trytoavoidthetrigger,or,

• takeextratreatmentbeforeoratthetimeofcontactwith your triggers.

Triggersusuallyshowthemselvesstraightawaybutsometimestakeeffectoverseveralhours.Keepingapeakflowrateorsymptomdiary will help you work out which triggers affect you.

The most common triggers are: colds and flu viruses, cigarette smoke, exercise and activity, allergies, chemicals and dusts at work, temperature and weather changes, emotions, fumes, household sprays and some medicines.

Often,it’snotjustonetriggerthatsetsoffanepisodeofasthmabutamixtureofseveraltriggersataroundthe same time.

Non Allergic Triggers

Colds and flu

Coldsandfluvirusesalongwiththroatandnoseinfectionscanbringonasthmaepisodes.Thefirstsignsareusuallyablockedorrunnynoseorasorethroat.Youcannotalwaysstopyourselfcatchingtheseinfectionssodealwith them as they appear. Everyone with asthma shouldhaveanannualfluvaccine.

• Assoonasthefirstsnuffleor blocked nose appears, follow your Self Management Plan instructions.

• Useyourpeakflowmeteror record your symptoms to check your progress.

• Seeyourdoctorifyourasthma gets worse.

Cigarette/marijuana smoke

Aroundthree-quartersofpeoplewithasthmabecomewheezyinasmokyroom.Ithasbeenshownthatchildrenwithasthmawhoseparentssmokehavemoreasthmasymptomsthanchildrenofnon-smokers.Also,exposureofyoungbabiestoexhaledorsecond-handsmokeincreasestheirriskofdevelopingasthmaandbronchialproblems.Inhalingmarijuanasmokeisjustasharmfultothelungsascigarettesmoke.

• If you smoke try to give up!! It isn’t always easy, but there are groups around to help. (For help with quitting, talk with your doctor or respiratory educator about local agencies that can help you, or call the Quitline on 0800 778 778). Remember, each cigarette you smoke is damaging your lungs. If you have managed to quit before – well done – it will be easier next time. Smoking may make your asthma more difficult to control and there is a very real risk of developing COPD (Chronic Obstructive Pulmonary Disease) – for example chronic bronchitis or emphysema, which would limit your life much more severely than asthma does.

• Iffriendsorfamilysmoke,explainhowitaffectsyourasthmaandencourage them to either give up or smoke elsewhere.

• Ifyouareoutavoidsittingnearpeoplewhoaresmoking.

exercise and activity

Wheezing,tightnessofthechestorcoughingduringorafterexerciseshowsthatthisistriggeringyourasthma.Ifyourasthmaiswellmanaged,youshouldbeabletoexercisewithoutthesesymptoms(usingthetipsbelow).Ifyoucan’texercisewithoutgettingasthma,seeyourdoctor.Rememberthatphysicalactivitywillhelpyourasthmaoverall–soit’sbettertomanageyourasthmaeffectively,allowingyoutoexerciseratherthanavoidingactivity.

• Do more asthma-friendly activities. Activities involving a lot of stopping and starting or a warm, moist environment are less likely to cause Exercise Induced Asthma (EIA). For example try swimming, walking, tramping, tennis, yoga, martial arts, tai chi, aerobics or team sports. Running and cycling are the hardest activities for most people with EIA.

• Remember to warm up. Stretching and a few minutes of running or skipping before exercising will:

- help prevent EIA

- protect you from sprains and strains

- get you in the mood to move!

• Check the conditions

- is your asthma worse due to a cold?

- is it a cold, dry day?

• Use your reliever inhaler before activity

- take1-2puffsofrelievermedicine(Bricanyl,SalamolorVentolin,…usually blue-coloured) just before you exercise.

- otherusefulmedicationsareIntal,TiladeorVicrom–theyareusually preventer medicines but can also be used 10 minutes before exercise.

• The role of symptom controllers. Symptom controllers (OxisorSerevent)areveryeffectiveatpreventingepisodesofEIA. These medications are normally taken twice a day and extra doses are not recommended immediately prior to exercise.

Temperature and weather changes

Changesoftemperatureandweathercanaffectpeople with asthma.

• Trytokeepyourhomeataneventemperature.

• Itmayhelptouseathermostatically-controlledheater in bedrooms at night to keep the temperature around 20°C.

• Wearingathin,warmscarflooselyaroundyourlower face can help warm the air you breathe.

• Ifyouknowthatcertainweatheraffectsyourasthma you may need to increase your medicine during that time.

Fumes

Triggersintheairincludenotonlyfactorysmokeandcarexhaust fumes,butalsocigarettesmoke,flysprays,strongperfumesand aerosolcleaningsprays.Somebuildingmaterialsandhomefurnishingsmaygiveofffumesthatmightmakeasthmaworse.Formaldehyde, inparticle-board,isoneofthese.

• Trytofindwhatthetriggeris.

• Alwayscarryyourrelievermedicationwithyou.

• Sealparticle-boardfloors,shelves,stairs,andhouseholdfittingswithpolyurethaneoran oil-based paint.

• Seeyourhomeiswell-ventilatedsothatfumesdo not accumulate.

Medicines

Thecommongroupsofmedicineswhichcantriggerasthmaare:

• Beta blockers Theseareused(intabletform)totreatanumberofillnesses–includinghighbloodpressure,heartconditionsandmigraine –aswellasineyedropsforglaucoma.Ifyouarediscussinganyoftheseconditionswithyourdoctorremembertotellhimorherthatyou also have asthma.

• Aspirin and non-steroidal anti-inflammatory pain relief Onein20peoplewithasthmaissensitivetoaspirinandothermedicinesforpainrelief.Ifaspirinupsetsyou,makesurewhenyoubuyanypainrelieftabletsthatyoureadthepacketcarefully.

Andremember,nochildren undertheageof12shouldbe given any aspirin. Paracetamol is thesafestmedicationforoccasionalusetotreatpainorcoldsandflu.However,researchhassuggestedthat the longer term use of high dosesofparacetamolcan aggravate asthma.

• Non steroidal anti-inflammatory pain relief (NSAID) NSAIDSmaytriggerasthmainsomepeople.Examplesofnonsteroidalanti-inflammatorypainreliefare:Ibruprofen(Brufen,Nurofen,Anafen,Motrin,Panafen),Naproxen(Naprosyn,Synflex,Naxen,Noflam)andMefenamicAcid(Ponstan).

• Natural products Itisimportanttorememberthat“natural”doesn’tnecessarilymeantheproductdoesnothaveharmfulsideeffects.Productscontainingbeepollen,royaljellyorechinaceahavebeenfoundtobeharmfultopeoplewithasthma.

Youmayfindthatyourchangingemotionssuchasbeingworried,up-tightorstressed,excitedor happy can make your asthma worse. The part youremotionsplayinyourasthmacanbedifficultto pinpoint. If you are aware that your feelings maybemakingyourasthmaworse,youmayneedprofessionalhelptohelpyoubalanceyouremotions.Yourdoctormaybeabletoassistyouorrefer you on to the appropriate person.

Allergic Triggers

Some people are particularly sensitive or “allergic” to certain things they come into contact with every day. Theseallergiesareimportantinhayfeverandeczemaaswellasasthma.Hayfevercausesanitchy,runnynose,sneezingandsometimesred,wateryeyes.Eczemacausesrednessanditchingoftheskin.

Themostcommonasthma-producingallergiesarerelatedto:housedustmites,animals,pollens,moulds,fungalspores,certainfoodanddrinks.

House dust mites

Housedustmitesaretoosmalltosee.Theyliveofftheflakesofskinweconstantlyshed.Weallhavetheminourhomes,insoftfurnitureandcarpetsandespeciallyinourmattressesandpillows.Theirwasteproducts,whichareverytinyandfloatintheair,canprovokeanallergicreactionwhenbreathedin.Allergiestodustmitesareverycommon.

Signs that you might have an allergy todustmitesincludewheezingwhenyouarevacuumingordustingorwhenyouenteradustyroom orhouse,orasthmasymptomsduringthenightorfirstthing in the morning.

• Getbeddingcoverswhichprovideabarrier.Mostmanufacturers of pillows, mattresses, bed bases and duvet inners use material which allows dust mite waste products out into the open air. However, you can buy special barrier covers for these items (ask at yourlocalAsthmaSocietyorcallAirflowProducts0800AIRFLOW or 0800 247 3569). If your asthma is triggered by house dust mite waste, and you take only one step towards reducing exposure to them,thisshouldbetheone–it’sthesinglemosteffectivemeasureyou can take.

• Vacuumingdoesnotgetridofdustmitesbutthoroughvacuuming of carpets will reduce the level of dust mite waste as long as the carpet isnotheavilyworn.Vacuumingcarpet,upholstery,mattressesandboth sides of pillows is recommended at least once a week. If your asthma is triggered by dust mite waste have someone else doitforyou.Usevacuumcleanerswithamicro-filter,an“S-class”filter,oranHEPA(“HighEfficiencyParticulateAir”)filtersystem.Machineswithoutthesesortsoffiltersaremorelikelytorecirculatethefinedustmitewastethroughtheirexhaustvent,backintotheair. IfyourvacuumcleanerdoesnothaveaHEPAfilteralwaysopenthewindows when you vacuum. Avoid using vacuum cleaners that expose you to the contents when you empty them. After vacuuming, leave the house and allow the dust to settle for a half hour or so before re-entering.

• Dampdustweekly(byusingadamp,notwetcloth).Avoidusingfeather dusters. The barrier covers on mattresses and bedding (seethefirstitemabove)shouldbedampdustedonceaweekbut not removed.

• Whenyoudothewashing,hangsheetsandpillowcasesinthesun to dry. Direct sunlight kills the dust mite. Alternatively, if you use a drier, run it an extra half hour on warm after the contents are dry.

• Airblanketsweekly,wherepossibleinnaturalsunlight.Dothesamewithanylooserugs.Washordrycleanblankets,pillowsandquilt(duvet) inners often – at least once every six to eight weeks.

• Don’tkeepthingsunderthebed,orontopofwardrobesbecause they gather dust.

• Inaddition,withanyyoungchildren:

- put soft toys in the deep freeze for at least 72 hours and then washregularly.Usewashablesofttoysandavoidfluffytoys where possible;

- avoid using sheepskins (especially avoid using them as infant bedding);

- do not put young children on a bottom bunk.

Catsarethesecondbiggestsourceofindoorallergen.AhighproportionofNewZealandfamilieshavecatsandtheallergenstheyproducetendtostayinthehouseforlongperiods.Theyarenotagoodchoiceofpetforfamilieswithpeoplewhohaveallergies.Otherpetsdonotseemtoproducesuchpotentallergens,butdogs,rabbits,guineapigsandmicecancauseproblemsinsomepeople.

• Neverallowpetsintothebedroomand, if possible, keep them outside.

• Ifyouaretovisitfriendsorfamilywhohave furry animals take extra medication beforehand, or ask them to visit you. People will understand if you explain why.

Food and drink

Thefoodsmostcommonlyassociatedwithfoodallergyarecow’smilk,wheat,seafood,eggs,soyandpeanuts.Themainsymptomsarehives,eczema,itching,vomiting,diarrhoea,abdominalpain,nasalcongestionandwheeze.

Mildallergyaffectingasthmaoccursinprobablyoneoutof50childrenundertheageoftwo.Mostchildhoodfoodallergiesareout-grownbytheageofthree.Inolderchildrenandadultsitislessfrequentaffectingaboutonein500people.Ifthecommonfoodscausingallergiesarenotintroducedtoachild’sdietuntiltheyare2theyarelesslikelytodeveloplife long allergies.

• Checklabelsonalltinnedandpackagedfoodtomakesuretheyarefree from the substances you are allergic to.

• Ifyouwishtotakesomethingoutofyourchild’sdiet,discussit firstwithyourdoctor,asitmaybeimportantandnecessaryforhealthy growth.

• Donotusecow’smilkformulas.

• Delayintroductionofsolidsforyourchilduntilsixmonthsofage.

Pollens, moulds and fungal spores

Pollenscancomefromgrasses,treesandshrubs.Privethasreceivedpublicityasatriggerforasthmabutnoteveryonewithasthmaissensitivetoit.Grassesandweedsaresowidespreadandhavesuch alongseasonthattheyarethemajorpollenprobleminNewZealand.

• Avoidmowinglawns.

• Replaceanytreeorshrubinyourgardenthatmakesyournoseandeyesitchormakesyousneezeandwheeze.Windpollinatedplantsshould be avoided because they have light pollen that travels easily.

• Takeextramedicineduringthemonthswhenyouknowpollensarelikely to make your asthma worse. If you also get hay fever, medicines may be required for this too.

• Keepthehouseaswarmandasdryaspossibletoavoidmoulds – do not use unflued gas heaters as they release moisture and particulates into the air.

• Removemouldormildewfromwalls,showercurtains,etc.withafungicide e.g. very-diluted household bleach.

• Airyourclothesandyourwardrobesregularly.

Workplace Triggers

Clues that something at work is making your asthma worse are that your asthma gets worse soon after starting a new job or while you are doing a particular part of your job, or your asthma improves when you are not at work, e.g. on holiday or at the weekend.

• Beforestartingajob,considerwhetheritinvolvesanythingthatmaymake your asthma worse.

• Identifythecauseandtakeadviceoncontrolmeasures,especiallyadequate ventilation.

• Avoidanytriggersthatyoucanidentify.Youmayneedto talk to your boss and see what can be done about avoiding them, or contact the Occupational Safety and Health Service. Yourdoctorwillbeabletohelpyouwiththis.

• Askifyoucanchangetoadifferentareaofyourworkandseeifthismakesadifference.

• Ifthetriggerscan’tbeavoided,discusswithyourdoctorwhichmedication you can increase. However, if you continue to be exposed to a trigger, then even with increasing medication your asthma is likely to worsen over time.

• Asafinalsolution,youmayhave to change your job.

• epoxyresinsandotherplasticsfromboatbuilders, mold manufacturers and plastic manufacturing processors.
