Forms and records applicable for use in performing prescribedoperations are listed below with brief explanations of each:(1) STANDARD FORM No. 26, DRIVER'S REPORTACCIDENT,MOTOR TRANSPORTATION. One copy of this form will be kept withthe vehicle at all times. In case of an accident resulting in injury orproperty damage, it will be filled out by the driver on the spot, or aspromptly as practical thereafter.(2) WAR DEPARTMENT FORM No. 48, DRIVER'S TRIP TICKETAND PREVENTIVE MAINTENANCE SERVICE RECORD. This form, properlyexecuted, will be furnished to the driver when his vehicle is dispatchedon nontactical missions. The driver and the official user of the vehiclewill complete in detail appropriate parts of this form. These formsneed not be issued for vehicles in convoy or on tactical missions. Thereverse side of this form contains the driver's daily and weekly pre-ventivemaintenance service reminder schedule.(3) W.D., A.G.O. FORM No. 478, MWO AND MAJOR UNIT ASSEM-BLYREPLACEMENT RECORD. This form will be used by all personnelcompleting a modification or major unit assembly (engine, transmis-sion,transfer case, and tracks) replacement to record clearly thedescription of work completed, data, vehicle hours, and/or mileage,and MWO number or nomenclature of unit assembly. Personnel per-formingthe operation will initial in the column provided. Minorrepairs, parts, and accessory replacements will not be recorded.(4) W.D., A.G.O. FORM No. 6, DUTY ROSTER. This form, slightlymodified, will be used for scheduling and maintaining a record of ve-hiclemaintenance operations. It may be used for lubrication records.(5) W.D., A.G.O. FORM No. 461, PREVENTIVE MAINTENANCESERVICE AND TECHNICAL INSPECTION WORK SHEET FOR WHEELEDAND HALF-TRACK VEHICLES. DESCRIPTION (figs. 1,2, and 3).a. General. The 45-ton Tank Transporter Truck-trailer M19is a combination of two units, the 12-ton, 6x4 truck M20 and the45-ton, 12-wheel trailer M9. The unit is designed to recover and trans-portdamaged tanks and materiel weighing up to 90,000 pounds.b. Truck. The 12-ton, 6x4 truck M20 is a tandem-rear-axle, or6-wheel, truck equipped with a rear pintle for towing the M9 trailer.The cab is a closed type on some models and open type on others.A ballast body and a winch are installed on the chassis behind thecab. The winch is operated through a power take-off installed onthe auxiliary transmission and is controlled from outside the cabby means of an air-operated clutch control and remote throttle con-trol.Recovery and loading is accomplished by means of the winch.Additional wheel traction can be obtained by filling the ballast bodywith any available materials such as gravel or sand. The spare wheeland tire are mounted in the forward end of the ballast body, and twostowage boxes are located inside the body at the rear end. A fuel tankis installed on each side of the truck immediately behind the cab.The storage batteries and fuel gate valves are carried in a compart-menton each side under the cab doors.
