This is the DA Form 2408-12 which is located in the aircraft logbook. The highlighted portions must be completed after each flight. The code circled in red will change with each class. Consult your flight instructor to determine what your class code will be. The fuel section reflects fuel that was added during the flight and the total fuel in tanks after refueling was complete.
