• Submit this form to Nanci Goldberg, ngoldberg@mines.edu, in AH 451 at least 2 weeksprior to the defense date. Submission of the form indicates that all Committee Membershave agreed to the date and time of your defense.

• Submit the final version of your thesis to your Committee Members no less than twoweeks prior to the defense date.

• Prior to your defense, your Admission to Candidacy form must have been submitted.You must be registered at the time of your defense.
