Yale’s “Game Changing” Stroke Treatment

This information comes from a recent Journal Inquirer

article by Ed Stannard of the New Haven Register.

Each year about 690,000 Americans experience an

ischemic stroke; that is, a stroke where a clot cuts off

blood flow to the brain. Stroke is the fifth leading

cause of death in the U.S. and “is a major cause of

adult disability,” according to the federal Centers of

Disease Control and Prevention. Last February it hap-

pened to Bill Yocum, a New London resident, who was

at a meeting in East Lyme. At Lawrence & Memorial

Hospital his was given IVTPA (intravenous tissue

plaminogen activator) the only treatment available for

large blood vessel acute strokes until last year. At first,

Yocum couldn’t talk, didn't know where he was,

dragged his right leg, and lost his concept of numbers.

He was flown from New London to Yale-New Haven

Hospital. There, Dr. Charles Matouk was able to re-

move the clot, using a device called a stent retriever.

Matouk, a cerebrovascular neurosurgeon describes it as

a wire threaded through a microcatheter with mesh on

the end. “It expands to about 4 millimeters in size, so

it’s tiny”, he says. The treatment involves threading the

stent retriever from the groin area into the large blood

vessels in the brain-known as the Circle of Willis.

Once the device reaches the blood clot, the stent is

forced onto it, sticks to it, and removes it. Within sec-

onds after the clots' removal-procedure is done under

local anesthetic-Yocum could immediately lift his

arms, start speaking, and understand more than when

he first arrived at Yale-New Haven. Dr. Martourk, who

has performed the procedure 70 times, says of the stent

retriever, “It’s really only in the last year that it’s

emerged as the new standard of care.” He says that for

those patients with the type of stroke that makes them

eligible for the stint retriever, “We’re able to open up

their blood vessels 90 percent of the time… What

we’re able to offer in Connecticut affords people with

these acute strokes the best outcome.”
