At an estimated cost of $550,000 (US), Phase 1 of the medical clinic will sit on 2.5 acres of land and will be approximately 35km from the Haitian border in Laguneta, DR. Rotating medical/social teams from all over the world including Residents/Students from the PUCMM Medical School will assist locally hired staff to deliver primary care with an initial focus on diabetes management and cervical cancer screening. There will be onsite sleeping quarters and showers for up to 40 volunteers. Phase 2 will address specialty care especially dental, ophthalmologic, audiologic and prenatal services. Power backed up by solar panels and batteries, the sick, and especially children will be cared for as the facility grows in size and equipment is funded and purchased. Phase 3 will be surgical and post-op care for minor surgery. Through a series of partnerships across multiple sectors, highlighted by the MOU signed between Rutgers-RWJMS Office of Global Health and PUCMM, the De Montesino CHC will become a Dominican operated patient centered medical home (PCMH) in a "medical neighborhood“ model. This clinical-community partnership includes the medical and social supports necessary to enhance health, with the local and global network of NGOs, academic partners, and RWJBarnabas Health all working in concert with the Dominican Ministry of Health to coordinate healthcare delivery. The primary focus will be on meeting the needs of the individual patient, but also incorporate aspects of population health and overall community health needs to build capacity. In addition, non-clinical partners such as local community board, faith-based organizations, schools, employers, public health agencies, Peace Corps and local volunteers can actively promote care coordination, fitness, healthy behaviors, proper nutrition, as well as healthy environments and workplaces. While always driving towards the overarching goal of a healthy, vibrant, socially just self-sustaining community, the De Montesino CHC will provide a multitude of culturally sensitive service, educational and research opportunities for both Dominican and international educators, students, professionals and humanitarians.
