Residualformaldehyde after low-temperature, steam, andformaldehyde sterilization

occurs in blankets treated with steam and formalde-hyde and then stored (Alder, 1968). The residuallevels in paper might be used in the storage ofgoods from central sterilization departments, forexample, to play a similar self-disinfecting role inpaper bags and wraps. A small injection of formal-dehyde vapour into the chamber during autoclavingmight be of value in this respect. The method used to recover the chemicals by

aqueous extraction does, we feel, give meaningfulresults but we cannot, except by a method such assteam distillation or reflux, claim to have extractedfrom any article all of such chemicals absorbed intoor combined with it. However, the results obtainedby aqueous extraction represent a major proportionof the total which could be extracted by refluxing. Inany case the level of recovered formaldehyde orethylene oxide obtained by aqueous extraction maywell bear a closer relation to the levels likely to befound in use in vivo when aqueous solutions arepassed through tubing into a patient, or the tubingor other article is inserted into a patient and sur-rounded by aqueous solutions. The solubility ofboth ethylene oxide and formaldehyde in water is

high compared with the levels obtained in the aqueousextractions and enables us to discount this factorin comparisons. We consider that in rubber and plastic articles,

residual levels of formaldehyde following the low-temperature steam and formaldehyde sterilizationprocess are not likely to be toxic to patients. Theyare certainly much lower than the ethylene oxidelevels we obtained following sterilization by thatgas. In fabric and paper the higher formaldehydelevels may in fact be of value as they may give acontinuing self-disinfecting effect during storage.
