Since2013,therehasbeenanopen-accessChestPainServicethatcanallowapatientwithstablechestpaintobeseen,assessedandinvestigatedwithanexercisestress test followed by a review with decision by the oncallconsultantCardiologist.Patientsareoftenseenwithin the same day of the call, and never more than
72hours.Acutechestpainswithsuspicionofacutecoronarysyndromes,quiterightly,aredealtwith in the usual way by the Emergency Department.
Thoserequiringinvasivediagnosticandinterventionalcardiacproceduresmaybefacilitatedby the Cardiac Catheterisation Laboratory (Cath Lab). The Cath Lab is the fulcrum of the cardiology service here at the Hermitage as with modern cardiology departments everywhere. TheHermitageCathLabhasahighlyskilledandexperiencedteamofnurses,techniciansandradiographers supporting the cardiologists in performing coronary angiograms, angioplasties,pacemakers and other procedures.
cardiac imaging tecHniqueS
TheCardiologyDepartmentattheHermitageMedicalClinicoffersarangeofadvancednon-invasive modern cardiac imaging modalities, ranging from CT coronary calcium scoring, CT coronary angiography to cardiac MRI with and without stress perfusion.
Cardiac MRI or its recommended term, cardiovascular magnetic resonance (CMR) has emerged as a specialised and powerful diagnostic test for cardiac diseases of the myocardium once the technicalpitfallsofimagingamovingorganhasbeenovercome.CMRisuniqueinitsabilityto image inmultiple planeswith excellent temporal and spatial resolution andhas severaladvantagesoverothercardiacimagingtechniqueswithouttheuseofiodinatedcontrastagentsand without the ionising radiation. It offers superb image clarity and tissue characterisation using different MRI techniques and sequences. This has revolutionised the assessmentof theRVaswell as thediagnosesofRVcardiomyopathieswhicharedifficult in traditionalechocardiography. In addition, CMR is extremely helpful in evaluating cardiac texture inpatientswithinheritedorpreviouslyundefinedcardiomyopathiestoaidinphenotyping.
calcium Score Scan
CoronarycalciumscansuseaspecialX-raytestcalledcomputedtomographyCTtocheckforthebuildupofcalciuminplaqueonthewallsofthearteriesoftheheart.Thistestisusedto
check for heart disease in an early stage and to determine how severe it is. Coronary calcium scansarealsocalledcardiaccalciumscoring.Theproceduretakesabout10–15minutesbutpatientsmayexpecttobeinthedepartmentforapproximately30minutesintotal.
The images are reported on by a Consultant Cardiologist and the scan report will be sent directly to the referring Doctor.ThiscardiacCTscanisavailabletooutpatientswithaGPorconsultantreferral.
cardiac ctThis scan gives detailed images of the heart and coronary arteries using an intravenous contrast injection.Italsoincludesacalciumscorescan.AcardiacCTscanforcoronarycalciumisanon-invasiveway of obtaining information about the presence, location and extent of calcifiedplaqueinthecoronaryarteries—thevesselsthatsupplyoxygen-containingbloodtotheheartmuscle.Calcifiedplaqueresultswhenthereisabuild-upoffatandothersubstancesundertheinner layer of the artery. This material can calcify which signals the presence of atherosclerosis, adiseaseofthevesselwall,alsocalledcoronaryarterydisease.Peoplewiththisdiseasehaveanincreasedriskforheartattacks.Inaddition,overtime,progressionofplaquebuildup(CAD)can narrow the arteries or even close off blood flow to the heart. The result may be chest pain, sometimes called “angina,” or a heart attack.
Theproceduretakesapproximatelytwoandahalfhoursintotalfromstarttofinish.Followingthe scan, a Consultant Radiologist and a Consultant Cardiologist will analyze the images produced and their report will go directly to the referring doctor.

treatment of Heart rHytHm
Oneoftheleadingdiagnosticmethodofheartdisturbancesandcardiacarrhythmia,EPstudy,is also performed in the Hermitage. Ablation is also carried out for patients with tachycardia or rapidheartbeatssuchasasupraventriculartachycardia(SVT).
