University Medical Center Employee Benefits Summary

Full-time, regular employee - scheduled to work 36-40 hours Part-time, regular employee - scheduled to work 20-32 hours

This benefit summary is for information only and is not meant to be all inclusive. It is not a contract, nor is it meant to impose any legal obligations either upon you or University Medical Center.

BENEFIT WHEN ELIGIBLE WHAT YOU RECEIVE

Retirement (Pension Program) Public Employee Retirement System (PERS).

Immediately (vested: 5 yrs)

UMC pays the full contribution (28%) of your base pay on your behalf (up to a cap of $265,000).

Consolidated Annual Leave (CAL)- Paid Time Off

Immediately Rate of accrual - based on 8-hour day: 0-4 yrs - 27 days/yr* 4-9 yrs - 32 days/yr* 9+yrs - 37 days/yr* *Based on 80 hrs/pay period. Part-time accrual is based on the number of paid benefit hours. CAL is used for personal leave, holidays, doctor appointments, vacation, and sick days up to 16 consecutive scheduled work hours (short term sick leave) etc. There are 11 scheduled holidays per year.

FICA Deduction Immediately You do NOT pay the 6.20% premium portion for Social Security. You DO pay the 1.45% Medicare deduction.

Extended Illness Bank (EIB) Immediately Up to six (6) days/yr on benefit hours paid. EIB is defined as any illness requiring hospitalization or an absence of more than sixteen (16) consecutive scheduled work hours.

EIB Conversion More than 720 EIB Hours

All accrued EIB hours in excess of 720 hours will automatically be placed in the employees CAL bank.

Medical Insurance First of the month following two (2)

full months of employment

Major medical, dental, life insurance, vision is part of the insurance plans. UMC pays the premiums for full-time employees (excluding Management positions).

Basic Life Insurance Same as Medical Insurance

Employee $20,000 + $20,000 AD&D Employee (Age 70-74 ) $13,000 + $ 13,000 AD&D Employee (Age 75 over) $ 9,000 + $ 9,000 AD&D Spouse of (employee under age 70 ) $5,000 Spouse of (employee over age 70) $1,000 Child (Age 6 months or more) $2,500 Child (Age 14 days to 6 months) $1,000 Note: Dependents are covered under the basic life insurance policy if the employee has covered the dependent under one of the health plans.

Optional Insurance Same as Medical Insurance

Additional life insurance, accidental death & dismemberment insurance, cancer/ intensive care, accident insurance, short term disability and tax savings plans.

Immediately MassMutual Financial Group provides additional supplemental retirement products and solutions.

Holiday Pay Immediately All hourly employees are paid time-and-a-half for hours worked on New Year’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day.

Employee Health Services Immediately Free pre-employment examination, Wellness Program, annual TB skin test and seasonal flu shots.

Workers’ Compensation Insurance

Immediately Medical care, compensation and rehabilitation for work-related injury or illness.

Bereavement Leave Immediately Three (3) paid days off for a death in the immediate family; as defined in the agreement between SEIU & UMC.

Jury Duty Immediately Regular pay for lost work time, less jury duty pay.

Outpatient Pharmacy Immediately Refer to Health Plan Benefits Plan Document for specific information on pharmacy discounts.

In-Service Education Immediately Voluntary - Paid for normally scheduled work hours. Required - Paid for all hours attending the in-service program.

Shift Differential Immediately Extra differential pay for work on evening shift and night shift.

Immediately Counseling services for the employee and immediate family members.

ESS is Your New Best Friend Forever (BFF)!

I’d like to introduce “Employee Self Service” (ESS). It is a new SAP module that is now available and it will soon become your new best friend forever (BFF)!

With ESS, you will be able to:

- Change your Address - View your emergency contact information - View your tax withholding status - View your direct deposit bank information - View the benefits that you’re enrolled in - View your salary statements (paycheck advices/stubs) from 04/04/08 to current

Your sign-on and password information will be given to you via a secured email. You will be able to access ESS on the UMC Intranet or on UMC’s website (www.umcsn.com) by clicking on the ESS icon: Since your salary statements will be available to you online, direct deposit advices will not be printed. The only exception to this will be for Environmental Services and Dietary Services employees. We are working on obtaining kiosks for those areas so sufficient computer access is provided before we discontinue printing their direct deposit advices. Upon termination, your access will be removed so please ensure you have downloaded and/or printed your information (paycheck advices, benefits information, etc) before your last day of employment.

Eligibility for Elective Salary Deferral

You are eligible to participate in the plan immediately upon employment.

Employee Elective Salary Deferral Contributions

Traditional Before-tax contributions

An eligible employee may contribute up to the maximum amount allowed by law ($18,000 for 2015).

A participant who turns age 50 any time during the calendar year or a participant who is over age 50 may make additional (catch-up) contributions above normal plan and legal limits (an additional $6,000 in 2015).

Any contributions and earnings grow tax-free until distribution.

Deducted automatically from employee’s paycheck.

Participants may change their contribution rate at any time.

Vesting You are always 100% vested in your own elective deferral contributions and earnings.

Loans You can have no more than one loan outstanding from the Plan at any given time.

You can borrow a minimum of $1,000 up to 50% of your vested account balance, with a maximum of $50,000, for any reason.

You pay interest on the loan at the current prime rate.

Distributions

Distributions are available from your account balance in the following situations: o Attainment of age 70½ o Retirement o Separation from service o Death of participant o Unforeseeable emergency (“hardship”) withdrawal (your plan may limit to a

portion of your account). Hardship circumstances include:

Expenses for medical care previously incurred by the employee, the employee’s spouse, or any dependents of the employee or which are necessary for these persons to obtain medical care.

Payments necessary to prevent the eviction of the employee from the employee’s principal residence or foreclosure on the mortgage on that residence.

Certain expenses relating to the repair of damage to the employee’s principal residence

Burial and Funeral expenses for the participant’s deceased parent, spouse, children, or dependents.

You will be eligible for a distribution at severance from employment at any age This Plan Highlight is intended to provide a brief description of certain plan features.
