For many families coming to hospital is a path to healing. For others it is a journey searching for a cure and for most it is a path to better health. The Children’s Hospital at Westmead takes a holistic approach to providing care for sick children and our journey is always one of creating a path to wellbeing.

Last year a new path was created at the Hospital in the form of a labyrinth and it proved to be a welcome addition to our positive and family friendly environment. Like so many features of the Hospital it was made possible through the generosity of donations, bequests and the commitment of our dedicated staff.

Walking a labyrinth helps foster mindfulness, relaxation and refreshment. It has one path only so you can enter secure in the knowledge you won’t get lost. Families of sick children may take many different paths to our Hospital but they arrive secure in the knowledge that we will offer them world-leading care and specialised services.

As the largest paediatric centre in NSW we care for more than 100,000 children each year. A focus on children first and foremost underpins our quality clinical care, community education, advocacy for improved child health and ground-breaking research into childhood illnesses.

The Children’s Hospital at Westmead is well regarded for achieving a number of firsts in healthcare and this tradition continues as we are the first hospital in Australia to install a labyrinth. These special types of mazes are considered to be beneficial in hospital settings and are beginning to play an important role in healthcare. More than 200 hospitals throughout the United States have a labyrinth in their grounds.

Children and young people understand labyrinths immediately. They recognise the sense of peace and often joy that can be found in this new feature of our gardens. For the many children and their families who have explored the labyrinth this year, they have found a new path to wellbeing at The Children’s Hospital at Westmead.

One of the more unusual and pleasant tasks I was asked to perform last year was to officially open the labyrinth in the grounds of The Children’s Hospital at Westmead. The labyrinth is positioned at the end of a tree lined promenade in a tranquil setting away from the bustle of the main building. It provides a safe haven for reflection and contemplation, and represents another example of our commitment to providing a total healing environment.

The labyrinth was made possible through the efforts of many staff. At its genesis was one of our oncologists, Dr Michael Stevens, who could see the benefit it would bring to his colleagues as well as the children and families in his care. Michael’s passion for the concept was shared by many others and their dedication and commitment was translated into action.

Dedication and commitment are attributes that are found in all staff at the Hospital. A willingness to support patients, families and clinical colleagues to make a difference is apparent in all aspects of work.

Some of our staff members have shared their stories in this Annual Review and it is a privilege for me to

work alongside such staff. The teams that you will read about in the following pages all played a role in changing the lives of young patients like Niamh, Belle, Jacob, Allie and Edward. I thank their families for letting us share the stories about their own journeys.

Like many of our young patients, these children and their families have been on different paths that have brought them to The Children’s Hospital at Westmead. Our new labyrinth encourages people to take different paths through the maze and this document has been designed with that in mind.

The continued support of our staff and the generosity of the community will see us continue our journey of providing the very best care and services for children. I hope that you can join us on this path to wellbeing.

Surgical SavioursDr Jonathan Karpelowsky is one of our leading paediatric surgeons specialising in minimally invasive surgery such as oesophageal repair on babies and the removal of tumours in the lungs of toddlers. Every day presents new challenges to improve patient outcomes with faster recovery times and less scarring. “We really push ourselves to work in new ways without compromising care,” he said. “We strive to make the image parents have of their healthy child into a reality.”

Advancements in keyhole surgery over the past decade have been nothing short of remarkable. What was once a new procedure for adults and unheard of for babies has become commonplace. “Scars are so small I often have to get parents to point them out to me,” Jonathan said. “In ten years we have gone from not doing it at all to designing our new operating theatre complex around it.” The Children’s Hospital at Westmead is now home to technologically advanced operating theatres equipped with video cameras as part of a multi-million dollar upgrade thanks to Sargents Pies Charitable Foundation.

The five state-of-the-art suites allow members of the surgical team to view on screens what the surgeon can see during delicate operations. This means nurses like Monica Hume can access medical imaging without having to move away from the operating table. “We can instantly take pictures on the touchscreen without disturbing the sterile field,” she said. It has taken teamwork and communication to a whole new level.”

Nurse Manager Jason Simpson oversees the coordination of 200 staff in the Operating Suites, Anaesthetics, Recovery, Day Stay and Short Stay Unit. It is his job to make sure when surgeons like Jonathan arrive in an operating theatre it is ready with the right staff and the right equipment for the right procedure. “Parents are literally handing their children’s lives over to us,” Jason said. “It is an enormous responsibility for all of us but there is so much satisfaction in the smiles on parents faces at the end of the day.”

iN a m hC DLittle Niamh is still several months shy of her second birthday but has had more milestones charted than most infants after she was diagnosed with a rare lung disorder when her mother was just 19 weeks pregnant.

A pre-natal scan showed cysts on Niamh’s right lung and mum Caroline had an anxious pregnancy with every stage under medical observation. After Niamh was born her condition was closely monitored with her right lung pushing her tiny heart out of place. Dr Jonathan Karpelowsky removed half of this lung the day before Niamh’s first birthday when she was old enough for her remaining lung to be able to expand to fill her chest.

“If the keyhole capability had not been available Niamh would have had a cut from the front of her body right across her back,” Caroline said. “The hospital recovery time would have been at least three weeks with increased post-op pain. Instead, we were home three days later with a normal busy one-year-old and down the beach, back to usual family life, on day four.”

Niamh will grow up with little scarring as a reminder of this start to life. Her mother knows how lucky she is to have been born full term and to go on to have a successful operation with such a quick recovery. “We appreciate every day how lucky we are to have her.”

LL The Children’s Hospital at Westmead has long

been an advocate of using creative processes like art therapy as a valuable tool for children needing distraction from pain and anxiety. This is only made possible because of the generosity of our supporters. We have also kept up to date with the latest technologies to ensure children had access to games and other screen based entertainment to support them through often difficult situations.

A groundbreaking collaboration between an innovative artist and our own clinicians has redefined ways of interpreting how art can be used to help sick children. One of our leading rehabilitation paediatricians has worked with an interactive artist to produce a new iPad app that offers a unique interpretation of art therapy helping sick children endure painful medical treatments.

Dr Angie Morrow in Kids Rehab and artist George Khut have created BrightHearts to show how biofeedback can be used in clinical settings to reduce pain and anxiety experienced by children undergoing painful procedures. Angie works with seriously ill children who often need to undergo painful treatments to manage cerebral palsy and other disabling illnesses. By controlling their breathing a patient can change colourful geometric images. The patterns change according to the patient’s changing pulse rate. Children learn to relax and slow down their heart rate. The app provides a distraction and helps children gain a greater sense of control over their own body.

BrightHearts took out a number of awards in the past year including the 2012 Arts and Health Foundation Award from the national business arts foundation AbaF as well as the National New Media Art Award from the Queensland Art Gallery, Gallery of Modern Art. It was also a feature of the Sydney 2013 Vivid Light Festival with an exhibition of interactive works.

As the Nurse Unit Manager in the Emergency Department Dominic Sertori trains new staff in understanding the importance of silence. He encourages them to be quiet so they can hear what parents are telling them. “A child might come in with what looks like a simple fever but the parents sense there is something else wrong,” he said. “Really listening to the parents, that is the key, and that’s at the heart of what makes this hospital so child and family focussed.”

More than 50,000 children are seen by our Emergency Department each year with many of them requiring surgery and admission. While fevers are common ailments seen by our Emergency Department staff, too often it is tragic accidents that bring children to the Hospital and this can challenge the trauma teams whose skills are called on to save lives every day.

Our family focus is crucial when children are involved in trauma and Dr Mary McCaskill, Medical Director of the Emergency Department, makes sure parents do not get lost in the action that surrounds their child. “We see families in acute distress and while the teams are busy assessing and treating their children we will focus on offering the family support as well,” she said. “It is important to see beyond the immediate.”

The ability of experts like trauma surgeon Prof. Danny Cass to look beyond the immediate has seen the Hospital’s health promotion work shaped by the children coming though Emergency. Danny will operate on children after major traumas and then advocate for change to prevent further injuries or deaths. He reflects on working in paediatric trauma in past decades when the outcome for children involved in traumas was in many cases death or severe injury. The picture is much brighter today from a combination of advancements in trauma treatment and prevention. “Kids have never been safer,” he said. “Our research has helped inform prevention campaigns and legislative change from child restraints and drowning to building codes and nursery furniture.” Many advocacy initiatives have been supported through generous donations.

A spinal cord injury didn’t stop four-year-old Belle from dancing up a storm in her hospital bed this year. The cheeky little girl loved to bop and swing her arms around to her favourite songs.

Belle suffered massive abdominal and spinal injuries when she was trapped under the wheel of a vehicle. Trauma surgeon Prof. Danny Cass operated on her tiny frame. She was initially expected to spend up to three months in intensive care and a year in hospital but surprised everyone with her speedy recovery. After just four months Belle was able to go home. She will be a frequent visitor to a number of specialties including the Burns Unit following skin grafts and Kids Rehab for ongoing rehabilitation.

Learning how to use a wheelchair has been another surprisingly easy challenge according to her mother Catherine.

“Belle has been unbelievably brave through all of this and I guess not being able to feel her injuries means it hasn’t been too overwhelming for her,” she said. “She’s such an inquisitive chatterbox asking millions of questions, she’s probably asked the medical staff more questions than I have.”

The Children’s Hospital at Westmead has been such a home away from home that when they were able to take special weekend leave for Belle’s birthday and it was time to return to Hospital Catherine said it was time to "go home". “We’ve dealt with so many wonderful staff here, from emergency to PICU [Paediatric Intensive Care], Clubbe Ward (burns unit) and Occupational Therapy, I can’t thank the staff enough for what they have done for Belle and for our family.”

Our Child Life Therapists and Social Workers help patients cope with medical treatments, injuries and illness. The Child Life Therapy programs minimise stress and anxiety for children and their families with positive experiences. They promote effective coping techniques through play, music, education and self-expression activities with the generous support of our local community.

The two teams regularly use therapeutic tools to help children understand their emotions, medical procedures and hospital experiences. A key collaboration this year involved Social Worker Judy Goodson and Child Life Therapist Christina Parkinson in our Burns and Plastics Treatment Centre. They created two puppets, Harvey and Harper, to help young children cope with burn related injury and procedures.

Bright blue Harvey, 4, reached for a hot cup of tea and suffered a scald burn on his chest and shoulder. His pink friend Harper, 5, was helping her mother bake cakes and touched the hot over door causing a contact burn. Many children with burns will be able to relate to the puppets' stories because they are based on the most common injuries that bring youngsters to our burns unit.

The puppets were made by Judy and feature in their own short film showing children how to take medicine, use a nitrous mask and how nurses change their bandages. Patients can watch the films on iPads and see Harvey and Harper attending the treatment centre and experiencing the same procedures as them.

Bear Cottage is the only children’s hospice in the world that is affiliated with a children’s hospital. It relies heavily on fundraising and community support. The home like environment has been designed to enhance the palliative care program of The Children’s Hospital at Westmead by not only providing excellence in paediatric medical care 24 hours a day but support, respite and end-of-life care for children with life-limiting illnesses and their families.

This is a special place where staff and volunteers are committed to helping families make memories every day. Music Therapist Phoebe Cormack helps families create a space for laughter, tears and nurturing. She touches many families in her role, as music is something we all turn to at different times. “A particular song can take you to a time and place and give you the opportunity to enjoy happy memories and process memories that may be difficult to recall,” she said.

Volunteers are the backbone to many services at Bear Cottage and at the Hospital. Tracey Smail volunteers to work alongside Phoebe in the music therapy team and loves bringing everyone together with song and dance. “The essence of Bear Cottage is the love and support offered to the families when grief and hardship is part of daily life,” she said. “To have a child with a life-limiting illness is overwhelming and Bear Cottage brings joy.”

CottageWith around-the-clock nursing and medical care provided, families can focus on important things such as spending quality time together and making every moment count. Parents can re-charge their batteries and enjoy time with one another. Nurse Emma Hole is involved in the day-to-day care of the children as she helps to give families much need respite. It is a role that is creating life-long memories for her. She remembers fondly a child struggling to communicate. “When I placed him on his bed his mum asked him to thank me and he did so by giving me a big smile and squeezing my hand,” she said. “I felt so touched by this simple gesture and it is a moment I will never forget.”

Jacob

Like many children these days, eight-year-old Jacob knows his way around an iPad better than his mum Jackie. For Jacob who has autism, epilepsy and intestinal failure, the electronic device is a calming place complete with Thomas the Tank Engine.

Finding calm is sometimes a challenge for Jackie having a son requiring constant care since failing to thrive as an infant. He requires a feeding tube and regular nappy changes. Jacob has an older sister who is also autistic and Bear Cottage has been a place of calm and for creating treasured new memories for this family.

“Jacob spends lots of time in hospital and he also takes a lot of time at home to do meds and feeds and it is very tiring,” Jackie said. “The respite care at Bear Cottage allows me to rest and just be a mum without being his nurse. And it’s wonderful to not have to worry about cooking or cleaning as well.” Jacob loves to cuddle up to other patients and to the Bear Cottage resident dog Frankie. “We’re all relaxed and enjoying seeing Jacob so happy, loving and affectionate.”

Staying at Bear Cottage is also special for Jackie because she gets to spend mother-daughter time with 11-year-old Isabella safe in the knowledge that Jacob is receiving around-the-clock care. Taking time out for a trip to a coffee shop for an afternoon hot chocolate with her daughter would not be possible at home.

More than 4,000 MRI (Magnetic Resonance Imaging) scans are taken at the Hospital each year. This technique uses a powerful magnet and radio waves to produce images of any part of the body including soft tissue such as organs and muscles that do not show up on x-ray examinations.

MRI scanning is an essential tool in paediatric care and unlike CT (computed tomography) scanning and x-rays, it does not use radiation. This year we benefited from the installation of a new MRI suite that will shorten waiting times and broaden the scanning scope of our medical imaging technicians.

The new state-of-the-art suite will help reduce waiting times for MRI procedures and important diagnoses. The new MRI service will provide improved access for young patients to contemporary imaging technology.

The addition of a second MRI suite will broaden our scanning scope as each machine has differing strengths and this will allow us to better cater for specific clinical needs of patients. The new MRI includes a built-in ambient experience with DVDs, lighting and projected images to help distract patients. In support of the scanning facilities and control room, both suites include an anaesthetic induction room, recovery areas, reporting rooms, waiting room and reception.

Chaplains

Jan Donohoo is one of the Hospital’s Anglican Chaplains and like her Chaplain colleagues is part of the trauma response team, alongside social workers and medical staff, when a child is rushed to our Emergency Department. Attending to a family’s needs as they unfold in those first few hours is not just spiritual but sometimes practical for the ‘on-call’ Chaplain. “We offer emergency relief and little things like tea and toast and toiletries,” Jan said. “Sometimes parents are alarmed thinking we have arrived because their child has been rushed to Hospital but it is their child who has arrived and we are already here.”

Care Pastoral

While the Hospital has Anglican, Catholic, Presbyterian and Uniting Church Chaplains as well as a Muslim Visitor the Chaplaincy Department can also call on 15 denominations or religious organisations at the request of families. The Hospital offers a multi-faith religious and contemplative space that is open 24 hours for prayer, reflection, meditation and services of worship. The quiet room has stained glass features from the original chapel of the old hospital. Daily prayers, weekly Mass and sacraments such as baptisms are available.

For some families with no religious affiliations, the pastoral care of the chaplaincy service can meet all manner of emotional needs.

Uniting Church Chaplain Trish Rooney sees their support as helping the Hospital to offer a truly holistic approach to healthcare. While many families may not seek out pastoral care they can offer a supportive ear and a shoulder to cry on. “Sharing in the highs and joys of healing are some of the special moments I have spent with sick children and their families,” she said.

Presbyterian Chaplain Ian Schoonwater understands crisis and illness often raise significant spiritual issues for people. He has recently joined the Hospital after working in adult hospitals and has been touched by the level of compassion in his new colleagues. “The hospitalisation of a child is a frightening time and parents feel totally helpless,” he said. “Parents need all the help they can get through this difficult time and we are simply another form of support.”

When Allie was just five days old she was airlifted from Canberra to the Grace Centre for Newborn Care in The Children’s Hospital at Westmead. At 15 days she underwent her first operation and at six months she was on the liver transplant waiting list.

For her mother Claire it was a frightening and sad time. “We anticipated that life would change with a new baby but we didn’t anticipate that change would involve a strict medication regime, nasogastric feeding and regular visits to Sydney,” she said.

After several months the family’s prayers were answered and Allie received a new liver just before her first birthday. She now has a new lease on life as an inquisitive and giggling two-year-old. “Her post-transplant immune suppression keeps us a little bit isolated from friends and playmates but it has given us the opportunity to spend valuable time together as a family.”

Claire fondly recalls Allie’s ‘shotgun christening’ through the Hospital’s Chaplains before her first operation and pastoral care during the many months the family were part of the Hospital community. “Regardless of religious beliefs, the Chaplains service offers genuine care and commitment to all patients and families. Their support can be invaluable.”

As allergies continue to skyrocket in the Western world our clinicians and researchers investigate different theories and treatment options. Australia has one of the highest rates of children with allergies and it is still not clear why. One theory relates to the lack of sun exposure and The Children’s Hospital at Westmead is investigating whether high rates of vitamin D deficiency are related to high rates of allergies.

Donations from the annual Woolworths Fresh Food Kids Appeal helped fund an Allergy and Immunology Fellowship involved in a number of important projects including those looking at vitamin D deficiency. The Fellow, Dr Peter Hsu, is helping in research and clinical work to better understand conditions where the immune system is out of control such as allergy, eczema and diabetes.

Food allergies are a particular focus for our clinicians with one in 20 Australian children now affected. In the past year we conducted more than 500 food allergy challenges under medical supervision in the Hospital with several youngsters having severe reactions resulting in anaphylaxis and the need for adrenaline. Through the NSW Statewide Anaphylaxis Education Program we have also trained 476 public schools as well as many Catholic and independent schools in the recognition, management and prevention of anaphylaxis in the school setting.

The Hospital’s dietitians helped produce a new cookbook with healthy and nutritious allergy free meals. The newly published ‘YUM!’ is the definitive cookbook for making healthy meals that the whole family can enjoy. The book contains basic advice on managing allergies, shopping tips, medical facts and practical tips for babies to teens. The 100 recipes exclude all the major allergens including wheat, cow’s milk/dairy, egg, peanut, tree nut and soy.

As the head of our Oncology Department, Dr Luciano Dalla-Pozza has overseen the ‘bench to bedside’ approach of the Hospital’s cancer research unit. The questions posed by clinicians are used to challenge researchers to search for novel answers, insights and approaches to developing treatments for children with cancer. “Having both the cancer research and clinical arms working together means that we can more easily ‘translate’ discoveries in the lab to improve the treatments at the bedside,” he said. “We are uniquely placed having clinicians and researchers working closely in partnership.”

Through our Children’s Cancer Research Unit the Hospital’s researchers and clinicians work on improvements in diagnosis, therapy and long-term survival so the unique needs and concerns of each patient and their family can be individually addressed. Supporters like the Kids Cancer Project can help make this a reality. Our aim is to understand why and how cancers develop in children and to use this knowledge to design more effective and less toxic therapies.

The Hospital is part of the Kids Cancer Alliance which funds research being undertaken by Haematology Fellow Dr Paddy Barbaro to identify children with leukaemia who are more likely to experience side effects from chemotherapy. This research is focused on telomeres – the ends of chromosomes – and whether their size and length predicts which children are likely to experience severe side effects. “We want to understand if children with shorter telomeres are more seriously affected,” he said. “Then we might be able to predict how best to change their treatment to reduce these potentially harmful side effects, without damaging a chance of cure.”

Telomeres are also the focus of a different study for Clinician Researcher Dr Loretta Lau. She has trained as a paediatric oncologist and knows first-hand how devastating cancers like neuroblastoma can be on young patients. “Cancer cells are very smart,” she said. “Telomeres can influence how they behave and whether they can survive intensive treatments with chemotherapy and radiotherapy. This research may one day help determine the types of treatments that may be suitable for individual patients so we can outsmart the cancer cells.”Bench to Bedside

Edward

Edward’s family describe him as a ‘little guy with a big journey’ and what a difficult journey it has already been for the three-year-old. Diagnosed at birth with a rare and life limiting degenerative neurological disease he is now also fighting cancer and severe side effects from chemotherapy.

Early infancy saw Edward in reasonably good health despite a low appetite and delay in walking from low muscle tone due to Niemann Pick Disease Type C. His mother Alison only needed to bring him to Hospital every six months for a check-up but that all changed at two years of age when he was hospitalised for pneumonia and over several months couldn’t bounce back to full health. Tests eventually revealed he had Acute Lymphoblastic Leukaemia (ALL), one of the most common types of childhood cancer.

“It’s an understatement to say that when the ALL diagnosis came all of us, including his doctors, were shocked as both diseases are totally separate and unrelated,” Alison said. “It was unbelievable that this poor little guy could be struck by lightning twice.”

Dr Luciano Dalla-Pozza is desperately working to find a cure for Edward including exploring a bone marrow transplant as an option for the little guy. “The road ahead is going to be rough,” Alison said. “But hopefully we will all be able to pull through this together.”

Operation ArtThe Children’s Hospital at Westmead has more

than 2,000 donated artworks that are displayed in all areas of the Hospital, generally at child-eye height. Our Honorary Art Curator, Joanna Capon OAM, helps manage the art collection including those from young artists-in-the making selected in Operation Art.

More than 800 artworks were entered in the 2012 Operation Art. This was the 18th year for the competition which is an initiative of the Hospital in association with the NSW Department of Education and Communities. Students are encouraged to create artworks for Hospital wards to aid the healing and recovery process of young patients.

Operation Art 2012 was proudly supported by Australian Nuclear Science and Technology Organisation (ANSTO). The 50 finalists were on exhibition at The Art Gallery of NSW and a tour of regional galleries was also curated before the works returned to become part of the permanent art collection at the Hospital.

Five-year-old Tatjana Lukanovic, from James Erskine Public School won the Junior ANSTO Award for her colourful depiction of an owl and Hussain Ali Ibrahimi, 17, from Maitland Grossman High School won the Senior ANSTO Award for his stunning portrayal of blue wrens. Hussain said his artwork reflected his circumstances: “I painted it in Leonora Detention Centre (Perth). It was like hospital. We had limited access to the world and wanted to be free like birds”.
