กรมธรรมประกนภยคมครองการเดนทางในประเทศแบบกลม

โดยการเช�อถอขอแถลงในใบคาขอเอาประกนภยซ� งถอเปนสวนหน�งของกรมธรรมประกนภยน( และเพ�อเปนการตอบแทนเบ(ยประกนภยท�ผถอกรมธรรมประกนภย หรอผเอาประกนภยตองชาระ ภายใตขอบงคบ เง�อนไขและขอกาหนดท�วไป ขอตกลงคมครอง ขอยกเวน และเอกสารแนบทายแหงกรมธรรมประกนภยน( บรษทใหสญญากบผเอาประกนภยดงตอไปน( หมวดท" 1 คาจากดความ

ถอยคาและคาบรรยายซ�งมความหมายเฉพาะท�ไดใหไวในสวนใดกตามของกรมธรรมประกนภย จะถอเปนความหมายเดยวกนท(งหมด ไมวาจะปรากฏในสวนใดกตาม เวนแตจะไดกาหนดไวเปนอยางอ�น ในกรมธรรมประกนภย 1.1 บรษท หมายถง บรษท ชบบสามคคประกนภย จากด (มหาชน) 1.2 กรมธรรมประกนภย หมายถง ตารางกรมธรรมประกนภย ตารางผลประโยชน เง�อนไข ขอกาหนดท�วไป ขอตกลงคมครอง ขอยกเวน เอกสารแนบทาย ขอระบพเศษ ขอรบรอง และใบสลกหลงกรมธรรมประกนภย ซ� งถอเปนสวนหน�งแหงสญญาประกนภย 1.3 ผเอาประกนภย หมายถง บคคลท�ระบช�อเปนผเอาประกนภยในตารางกรมธรรมประกนภยน(และ/หรอเอกสารแนบซ�งเปนบคคลท�ไดรบความคมครองตามกรมธรรมประกนภยน( 1.4 ผถอกรมธรรมประกนภย หมายถง บคคลหรอองคกร ท�ระบช�อเปนผถอกรมธรรมประกนภยในตาราง กรมธรรมประกนภยน( ซ� งเปนผจดใหมการประกนภยเพ�อประโยชนของผเอาประกนภย 1.5 อบตเหต หมายถง เหตการณท�เกดข(นอยางฉบพลนจากปจจยภายนอกรางกายและทาใหเกดผลท�ผเอาประกนภยมไดเจตนาหรอมงหวง 1.6 การบาดเจบ หมายถง การบาดเจบทางรางกาย อนเปนผลโดยตรงจากอบตเหตซ� งเกดข(นโดยเอกเทศและโดยอสระจากสาเหตอ�น 1.7 การเจบปวย หมายถง อาการ ความผดปกต การปวยไขหรอการตดโรคท�เกดข(นกบผเอาประกนภย 1.8 ความรบผดสวนแรก หมายถง ความเสยหายสวนแรกท�ผเอาประกนภยจะตองรบผดชอบเองตออบตเหตแตละคร( ง

1.9 แพทย หมายถง ผท�สาเรจการศกษาไดรบปรญญาแพทยศาสตรบณฑตไดข(นทะเบยนอยางถกตองจากแพทยสภา และไดรบอนญาตใหประกอบวชาชพสาขาเวชกรรมในทองถ�นท�ใหบรการทางการแพทย หรอทางดานศลยกรรม 1.10 พยาบาล หมายถง ผท�ไดรบใบอนญาตประกอบวชาชพพยาบาลตามกฎหมาย 1.11 โรงพยาบาล หมายถง สถานพยาบาลใดๆ ซ� งจดใหบรการทางการแพทยโดยสามารถรบผปวยไวคางคนและมองคประกอบทางดานสถานท�ท�มจานวนบคลากรทางการแพทยท�เพยงพอตลอดจนการจดการใหบรการท�ครบถวน โดยเฉพาะอยางย�งมหองสาหรบการผาตดใหญและไดรบอนญาตใหจดทะเบยนดาเนนการเปนโรงพยาบาลตามกฎหมายสถานพยาบาลของอาณาเขตน(นๆ 1.12 สถานพยาบาลเวชกรรม หมายถง สถานพยาบาลใดๆ ซ� งจดใหบรการทางการแพทยโดยสามารถรบผปวยไวคางคนและไดรบอนญาตใหจดทะเบยนดาเนนการเปนสถานพยาบาลเวชกรรมตามกฎหมายของอาณาเขตน(นๆ 1.13 คลนก หมายถง สถานพยาบาลแผนปจจบนท�ไดรบอนญาตตามกฎหมายดาเนนการโดยแพทยทาการรกษาพยาบาล ตรวจวนจฉยโรคและไมสามารถรบผปวยไวคางคนได 1.14 ผขนสง หมายถง สายการบนพาณชย รถไฟ เรอเดนสมทร เรอเฟอรร� รถขนสงผโดยสาร รถโดยสารปรบอากาศประเภทรถโคชท�ผเอาประกนภยโดยสารในการเดนทาง 1.15 เอดส หมายถง ภมคมกนบกพรอง (Acquired Immune Deficiency Syndrome) ซ� งเกดจากการตดเช(อไวรสเอดสและใหหมายความรวมถงการตดเช(อจลชพฉวยโอกาส เน(องอกรายแรง (Malignant Neoplasm) หรอการตดโรค หรอการเจบปวยใดๆ ซ� งโดยผลการตรวจเลอดแสดงเปนเลอดบวกของไวรส HIV (Human Immuno Deficiency Virus) การตดเช(อจลชพฉวยโอกาส ใหรวมถง แตไมจากดเฉพาะเช(อท�ทาใหเกดโรคปอดบวมหรอปอดอกเสบ (Pneumocystis Carinii Pneumonia) เช(อท�ทาใหเกดโรคลาไสอกเสบหรอเร(อรง (Organism Or Chronic Enteritis) เช(อไวรส (Virus) และ/หรอ เช(อราท�แพรกระจายอยท �วไป (Disseminated Fungi Infection) เน(องอกรายแรง (Malignant Neoplasm) ใหรวมถงแตไมจากดเฉพาะเน(องอก Kaposi’s Sarcoma เน(องอกเซลน(าเหลองท�ระบบศนยประสาทสวนกลาง (Central Nervous System Lymphoma) และ/หรอโรครายแรงอ�นๆ ซ� งเปนท�รจกในปจจบนน(วาเปนอาการของภมคมกนบกพรอง(Acquired Immune Deficiency Syndrome) หรอซ�งเปนสาเหตท�ทาใหคนท�เปนเสยชวตอยางกระทนหน เจบปวย หรอทพพลภาพ โรคภมคมกนบกพรอง (AIDS) ใหรวมถงเช(อไวรส HIV (Human Immuno Deficiency Virus) โรคท�ทาใหเย�อสมองเส�อม (Encephalopathy Dementia) และการระบาดของเช(อไวรส

หมวดท" 2 เง"อนไขและขอกาหนดท"วไป 2.1 สญญาประกนภย

สญญาประกนภยน( เกดข(นจากการท�บรษทเช�อถอขอแถลงของผถอกรมธรรมประกนภย หรอผเอาประกนภยในใบคาขอเอาประกนภยและขอแถลงเพ�มเตม (ถาม) ท�ผถอกรมธรรมประกนภย หรอผเอาประกนภยลงลายมอช�อใหไวเปนหลกฐานในการตกลงรบประกนภยตามสญญา บรษทจงไดออกกรมธรรมประกนภยน(ไว ให

ในกรณท�ผถอกรมธรรมประกนภย หรอผเอาประกนภยรอยแลว แตแถลงขอความอนเปนเทจในขอแถลงตามวรรคหน�ง หรอรอยแลวในขอความจรงใดแตปกปดขอความจรงน(นไวโดยไมแจงใหบรษททราบ ซ� งถาบรษททราบขอความจรงน(นๆ อาจจะไดจงใจใหบรษทเรยกเบ(ยประกนภยสงข(นหรอบอกปดไมยอมทาสญญา สญญาประกนภยน(จะตกเปนโมฆยะตามมาตรา 865 แหงประมวลกฎหมายแพงและพาณชย บรษทมสทธบอกลางสญญาได บรษทจะไมปฏเสธความรบผดโดยอาศยขอแถลงนอกเหนอจากท�ผเอาประกนภยไดแถลงไวในเอกสารตามวรรคหน�ง 2.2 ความสมบรณแหงสญญาประกนภยและการเปล"ยนแปลงขอความในสญญาประกนภย

กรมธรรมประกนภยน( รวมท(งขอตกลงคมครองและเอกสารแนบทาย ประกอบกนเปนสญญาประกนภย การเปล�ยนแปลงขอความใดๆ ในสญญาประกนภยจะตองไดรบความยนยอมจากบรษทและไดบนทกไวในกรมธรรมประกนภยน( หรอในเอกสารแนบทายแลวจงจะสมบรณ 2.3 ระยะเวลาเดนทางและระยะเวลาคมครอง

2.3.1 กรมธรรมประกนภยน( จะใหความคมครองความสญเสยหรอความเสยหายของผเอาประกนภยซ� งเกดข(นภายในระยะเวลาการเดนทาง ท(งน(จะจากดระยะเวลาการเดนทางแตละคร( งไมเกน 30 วนตดตอกน กรณมเหตฉกเฉนทางการแพทย การจ( เคร�องบน การลาชาหรอขดของของเคร�องบนท�ผเอาประกนภยใชโดยสาร ซ� งทาใหผเอาประกนภยไมสามารถเดนทางกลบไดภายในกาหนดระยะเวลาเอาประกนภย กรมธรรมประกนภยน( จะขยายความคมครองออกไปจนกระท�งผเอาประกนภยเดนทางกลบสจดหมายปลายทางตามความคมครองภายใตกรมธรรมประกนภย

2.3.2 ระยะเวลาเดนทางแตละคร( งของผเอาประกนภยจะเร�มตนและส(นสดภายในระยะเวลาเอาประกนภย 2.3.3 ในกรณท�เปนความคมครองแบบ One Way ตามท�ระบอยในตารางกรมธรรมประกนภย และ/หรอ

หนงสอรบรองการประกนภย ความคมครองใหเร�มตนเม�อผเอาประกนภยเชคอน ณ เคานเตอรสายการบน หรอ

สถานขนสง หรอทาเรอ (แลวแตกรณ) และดาเนนตอเน�องกนไปจนกระท�งผเอาประกนภยเดนทางถงอาคารผโดยสารขาเขาของสนามบนหรอสถานขนสง หรอทาเรอ ท�จดหมายปลายทาง(แลวแตกรณ)

2.3.4 ในกรณท�เปนความคมครองแบบ Round Trip ตามท�ระบอยในตารางกรมธรรมประกนภย และ/หรอหนงสอรบรองการประกนภย ความคมครองใหเร�มตนเม�อผเอาประกนภยเชคอน ณ เคานเตอรสายการบน หรอสถานขนสง หรอทาเรอ (แลวแตกรณ) และดาเนนตอเน�องกนไปจนกระท�งผเอาประกนภยเดนทางถงอาคารผโดยสารขาเขาของสนามบนหรอสถานขนสง หรอทาเรอ จดหมายปลายทาง (แลวแตกรณ)ในเท�ยวขากลบของการเดนทาง 2.4 การเปล"ยนแปลงยานพาหนะในการเดนทาง

ถาผเอาประกนภยมความจาเปนตองเปล�ยนยานพาหนะในการเดนทางในสวนหน�งของการเดนทางท�คมครองเน�องจากสาเหตท�อยนอกเหนอความควบคมของผเอาประกนภย ความคมครองภายใตกรมธรรมประกนภยน(จะยงคงมผลบงคบใชอยเสมอนกบไมไดเปล�ยนยานพาหนะ 2.5 การแจงและการเรยกรอง

ผถอกรมธรรมประกนภย ผเอาประกนภย ผรบประโยชนหรอตวแทนของบคคลดงกลาวแลวแตกรณ จะตองแจงใหบรษททราบถงการบาดเจบ หรอการเจบปวยโดยไมชกชา ในกรณท�มการเสยชวตตองแจงใหบรษททราบทนท เวนแตจะพสจนไดวามเหตจาเปนอนสมควรจงไมอาจแจงใหบรษททราบในทนทได แตไดแจงโดยเรวท�สดเทาท�จะกระทาไดแลว 2.6 หนาท"ในการรกษาสทธของบรษทเพ"อการรบชวงสทธ

โดยคาใชจายของบรษท ผถอกรมธรรมประกนภย หรอผเอาประกนภยตองกระทาทกอยางเทาท�จาเปนหรอเทาท�บรษทรองขอใหทาตามสมควรไมวากอนหรอหลงการรบคาสนไหมทดแทนจากบรษท เพ�อรกษาสทธของบรษทในการเรยกรองคาเสยหายจากบคคลภายนอกแทนผเอาประกนภย 2.7 หนาท"ในการรกษาซากทรพยท"ไดรบความเสยหาย

เม�อเกดความเสยหายกบทรพยสนท�เอาประกนภย ผเอาประกนภยตองไมละท(งทรพยสนน(น และตองสงมอบตอบรษท หากบรษทมความจาเปนตองใชซากทรพยสนน(นเพ�อการพจารณาคาสนไหมทดแทน 2.8 การจายคาสนไหมทดแทน

บรษทจะจายคาสนไหมทดแทนสาหรบกรณการเสยชวตใหแกผรบประโยชน สวนคาสนไหมทดแทนอยางอ�นจะจายใหแกผเอาประกนภย โดยจะจายคาทดแทนภายใน 15 วนนบแตวนท�บรษทไดรบหลกฐานแสดงความสญเสยหรอความเสยหายท�ครบถวนและถกตองแลว

ในกรณมเหตอนควรสงสยวา การเรยกรองคาสนไหมทดแทนเพ�อใหบรษทชดใชตามกรมธรรมประกนภยดงกลาวขางตนไมเปนไปตามขอตกลงคมครองในกรมธรรมประกนภย ระยะเวลาเอาประกนภยท�กาหนดไวอาจขยายออกไปอกได ตามความจาเปนแตท(งน( จะไมเกน 90 วน นบแตวนท�บรษทไดรบเอกสารครบถวนแลว

หากบรษทไมอาจจายคาสนไหมทดแทนใหแลวเสรจภายในกาหนดระยะเวลาเอาประกนภยขางตน บรษทจะรบผดชดใชดอกเบ(ยใหอกในอตรารอยละ 15 ตอป ของจานวนเงนเอาประกนภยท�ตองจายท(งน(นบแตวนท�ครบกาหนดระยะเวลาตามวรรคหน�งหรอวรรคสองแลวแตกรณ 2.9 การระงบขอพพาทโดยอนญาโตตลาการ

ในกรณท�มขอพพาท ขอขดแยง หรอขอเรยกรองใดๆ ภายใตกรมธรรมประกนภยฉบบน(ระหวางผมสทธเรยกรองตามกรมธรรมประกนภยกบบรษท และหากผมสทธเรยกรองประสงค และเหนควรยตขอพพาทน(นโดยวธการอนญาโตตลาการ บรษทตกลงยนยอมและใหทาการวนจฉยช(ขาดโดยอนญาโตตลาการตามระเบยบสานกงานคณะกรรมการกากบและสงเสรมการประกอบธรกจประกนภยวาดวยอนญาโตตลาการ 2.10 การส>นสดความคมครองโดยอตโนมต

ความคมครองตามกรมธรรมประกนภยฉบบน(จะส(นสดโดยอตโนมตสาหรบผเอาประกนภยท�กออาชญากรรมหรอขณะถกจบกม หรอหลบหนการจบกม 2.11 เง"อนไขบงคบกอน

บรษทจะรบผดชดใชคาสนไหมทดแทนตามกรมธรรมประกนภยน( หากผถอกรมธรรมประกนภย ผเอาประกนภย ผรบประโยชน หรอตวแทนของบคคลดงกลาวแลวแตกรณ ไดปฏบตถกตองครบถวนตามสญญาประกนภยและเง�อนไขแหงกรมธรรมประกนภย 2.12 การชาระเบ>ยประกนภย

เบ(ยประกนภยจะถงกาหนดชาระทนท หรอกอนความคมครองจะเร�มตน 2.14 การบอกเลกกรมธรรมประกนภย

ท>งบรษทและผถอกรมธรรมประกนภย หรอผเอาประกนภย สามารถบอกเลกกรมธรรมประกนภยได 2.14.1 การบอกเลกดงกลาวจะตองแจงใหทราบกอนการเดนทางลวงหนา 48 ช�วโมง โดยบรษทฯ จะคนเบ(ยประกนภยใหกบผถอกรมธรรมประกนภย หรอผเอาประกนภยโดยหกคาใชจายในการดาเนนงาน

2.14.2 การบอกเลกกรมธรรมประกนภยนอยกวา 48 ช�วโมงกอนการเดนทาง บรษทฯ จะไมคนเบ(ยประกนภย ยกเวนเปนการบอกเลกเน�องจากผขนสงเปนผยกเลกตzวเดนทางของผไดรบความคมครอง บรษทฯ จะคนเบ(ยประกนภยใหกบผถอกรมธรรมประกนภย หรอผเอาประกนภยโดยหกคาใชจายในการดาเนนงาน หมวดท" 3 ขอยกเวนท"วไป การประกนภยตามกรมธรรมประกนภยน>ไมคมครอง 1. ความรบผดสวนแรกท"ผเอาประกนภยตองรบผดเองตามท"ระบไวในตารางกรมธรรมประกนภย (ถาม) 2. ความสญเสยหรอความเสยหายใดๆอนเกดจากหรอสบเน"องจากสาเหตดงตอไปน>

2.1 สงคราม การรกราน การกระทาท"มงรายของศตรตางชาต หรอการกระทาท"มงรายคลายสงคราม ไมวาจะไดมการประกาศสงครามหรอไมกตาม หรอสงครามกลางเมองซ"งหมายถงสงครามระหวางชนท"อาศยอยในประเทศเดยวกน การแขงขอ การกบฏ การกอความวนวายใดๆ การปฏวต การรฐประหาร การประกาศกฎอยการศก หรอเหตการณใดๆซ"งจะเปนเหตใหมการประกาศหรอคงไวซ"งกฎอยการศก

2.2 อาวธนวเคลยร การแผรงส หรอกมมนตภาพรงสจากเช>อเพลงนวเคลยร หรอจากกากนวเคลยรใดๆ อนเน"องจากการเผาไหมของเช>อเพลงนวเคลยร และกรรมวธใดๆ แหงการแตกแยกตวทางนวเคลยร ซ"งดาเนนการตดตอกนไปโดยตวของมนเอง 3. การกอการราย (ไมใชกบขอตกลงคมครองผลประโยชนการจ>เคร"องบน) 4. ขณะท"ผเอาประกนภยกาลงข>น หรอกาลงลง หรอขณะโดยสารอยในอากาศยานท"มไดจดทะเบยนเพ"อบรรทกผโดยสาร และมไดประกอบการโดยสายการบนพาณชย 5. ขณะท"ผเอาประกนภยขบข"หรอปฏบตหนาท"เปนพนกงานประจาอากาศยานใดๆ 6. ขณะท"ผเอาประกนภยกออาชญากรรม หรอขณะท"ถกจบกม หรอหลบหนการจบกม 7. ขณะท"ผเอาประกนภยปฏบตหนาท"เปนทหาร ตารวจ หรออาสาสมคร และเขาปฏบตการในสงครามหรอปราบปราม หมวดท" 4 ขอตกลงคมครอง ภายใตขอบงคบ ขอตกลงคมครอง ขอยกเวน เง�อนไขและขอกาหนดท�วไป และเอกสารแนบทายแหงกรมธรรมประกนภย และเพ�อเปนการตอบแทนเบ(ยประกนภยท�ผถอกรมธรรมประกนภย หรอผเอาประกนภยตองชาระ บรษทตกลงจะใหความคมครอง ดงตอไปน(

DT 1 ขอตกลงคมครอง

ผลประโยชนการเสยชวต การสญเสยอวยวะ สายตา หรอทพพลภาพถาวรส>นเชงเน"องจากอบตเหต คาจากดความ การสญเสยอวยวะ หมายความถง การถกตดออกจากรางกายต(งแตขอมอ หรอขอเทาและใหหมายความรวมถงการสญเสยสมรรถภาพในการใชงานของอวยวะดงกลาวขางตน โดยมขอบงช(ทางการแพทยชดเจนวาไมสามารถ กลบมาใชงานไดอกตลอดไป การสญเสยสายตา หมายความถง ตาบอดสนท และไมมทางรกษาใหหายไดตลอดไป ทพพลภาพถาวรส>นเชง หมายความถง ทพพลภาพถงขนาดไมสามารถประกอบหนาท�การงานใด ๆในอาชพประจา และอาชพอ�น ๆ ไดโดยส(นเชงตลอดไป ความคมครอง การประกนภยน( คมครองความสญเสย หรอความเสยหายอนเกดจากการบาดเจบทางรางกายของผเอาประกนภย โดยอบตเหตและทาใหผเอาประกนภยเสยชวต สญเสยอวยวะ สายตา หรอทพพลภาพถาวรส(นเชง ภายใน 180 วน นบแตวนท�เกดอบตเหต หรอการบาดเจบท�ไดรบทาใหผเอาประกนภยตองรกษาตวตดตอกนในฐานะ ผปวยในในโรงพยาบาล และเสยชวตเพราะการบาดเจบน(นเม�อใดกด บรษทจะจายคาสนไหมทดแทนใหดงน( 1 100% ของจานวนเงนเอาประกนภย สาหรบการเสยชวต 2 100% ของจานวนเงนเอาประกนภย สาหรบการตกเปนบคคลทพพลภาพถาวรส(นเชงและการทพพลภาพถาวรส(นเชงน(น ไดเปนไปตดตอกนไมนอยกวา 12 เดอน นบแตวนท�เกดอบตเหต หรอมขอบงช(ทางการแพทยชดเจนวาผเอาประกนภยตกเปนบคคลทพพลภาพถาวรส(นเชง 3 100% ของจานวนเงนเอาประกนภย สาหรบมอสองขางต(งแตขอมอ หรอเทาสองขางต(งแตขอเทา หรอสายตาสองขาง 4 100% ของจานวนเงนเอาประกนภย สาหรบมอหน�งขางต(งแตขอมอ และเทาหน�งขางต(งแตขอเทา 5 100% ของจานวนเงนเอาประกนภย สาหรบมอหน�งขางต(งแตขอมอ และสายตาหน�งขาง 6 100% ของจานวนเงนเอาประกนภย สาหรบเทาหน�งขางต(งแตขอเทา และสายตาหน�งขาง 7 60% ของจานวนเงนเอาประกนภย สาหรบมอหน�งขางต(งแตขอมอ

ภยธรรมชาต หมายถง ภยอนตรายท�เกดข(นตามธรรมชาต และนามาซ� งการสญเสยหรอความเสยหายท(งชวตและทรพยสน อาทเชนไฟไหมปา น(าทวม พาย แผนดนไหว เปนตน ความคมครอง

การประกนภยน( ใหความคมครองถงคาใชจายของผเอาประกนภยท�ไดชาระไปสาหรบคาใชจายในการเดนทางและคาท�พกผเอาประกนภยในระหวางการเดนทาง และ/หรอคาปรบหรอคาใชจายท�เกดข(นตามมาจากการท�ผเอาประกนภยตองเดนทางกลบภมลาเนากอนกาหนดเน�องจาก

ผลประโยชนการลาชาของการเดนทาง ความคมครอง บรษทจะจายคาทดแทนคาใชจายใหในกรณท�การเดนทางของผเอาประกนภยถกเล�อนหรอลาชาออกไปต(งแต 6 ช�วโมงตดตอกนจากกาหนดการ โดยบรษทจะจายคาทดแทนใหไมเกนจานวนเงนจากดความรบผดท�ระบในตารางกรมธรรมประกนภยแกผเอาประกนภย สาหรบทก 6 ช�วโมงเตมตอเน�องของการลาชา ซ� งมสาเหตมาจาก 1. สภาพอากาศเลวรายทาใหผขนสงตองเล�อนการเดนทางออกไป 2. การชารดของเคร�องจกรกลของผขนสง 3. การประทวงหรอนดหยดงาน ซ� งเร�มข(นหลงจากท�บรษทไดตกลงรบประกนภยตามกรมธรรมประกนภยน( การเรยกรองผลประโยชนการลาชาของการเดนทาง ผถอกรมธรรมประกนภย ผเอาประกนภยหรอตวแทนของบคคลดงกลาวแลวแตกรณ จะตองแจงสงเอกสารหรอหลกฐานใหแกบรษทภายใน 30 วนนบแตวนท�เกดเหต โดยคาใชจายของตนเอง 1. แบบฟอรมการเรยกรองคาสนไหมทดแทนท�กาหนดโดยบรษท 2. สาเนาหนงสอเดนทางและ/หรอหลกฐานการเดนทางของผเอาประกนภย 3. สาเนาบตรประจาตวประชาชนของผเอาประกนภยพรอมรบรองสาเนาถกตอง 4. เอกสารยนยนเหตการณจากสายการบนหรอทาอากาศยานท�ออกโดยสายการบนหรอทาอากาศยานท�ระบสาเหตและระยะเวลาของการลาชาของเท�ยวบน 5. เอกสารหรอหลกฐานตามท�บรษทตองการตามความจาเปน (ถาม) การไมสงเอกสารหรอหลกฐานภายในระยะเวลาดงกลาวไมทาใหสทธการเรยกรองเสยไป หากแสดงใหเหนไดวามเหตอนสมควรท�ไมสามารถสงเอกสารหรอหลกฐานไดภายในระยะเวลาท�กาหนด แตไดสงโดยเรวท�สดเทาท�สามารถจะกระทาไดแลว ขอยกเวน การประกนภยในขอตกลงคมครองน>ไมคมครองการลาชาของการเดนทาง ซ"งมสาเหตมาจาก 1. การกระทาอนมเจตนารายเพ"อผลทางการเมอง ศาสนา ลทธนยม หรอการกอความวนวายทางการเมอง

ขอตกลงคมครอง ผลประโยชนความสญหายหรอความเสยหายของกระเปาเดนทางและทรพยสนสวนตวท"อยภายในกระเปา

เดนทาง คาจากดความ “ทรพยสนสวนตว” หมายถง ทรพยสนสวนตวของผเอาประกนภย เส(อผา เคร�องแตงกาย ซ� งนาตดตวไปหรอซ(อมาในระหวางการเดนทาง และทรพยสนน(นจะตองบรรจอยในกระเปาเดนทางของผเอาประกนภย และมไดระบไวในขอยกเวน “ตอรายการ” หมายถง ตอจานวน 1 ช(น หรอ 1 ค หรอ 1 ชด ความคมครอง การประกนภยน( ใหความคมครองความสญหายหรอความเสยหายของกระเปาเดนทางและทรพยสนสวนตวภายในกระเปาเดนทาง ซ� งรวมถงกระเปาเปท�บรรจทรพยสนสวนตวท�ใชในการเดนทางของผเอาประกนภย ซ� งเกดข(นภายในระยะเวลาเอาประกนภย จากเหตการณดงตอไปน( 1. การชงทรพย การปลนทรพย หรอการดาเนนการใดๆ ในลกษณะท�ใชความรนแรง 2. ความผดพลาดในการขนกระเปาเดนทางของผขนสง หรอการขนยายของกระเปาเดนทางท�อยภายใตการควบคมดแลของพนกงานโรงแรม 3. การลกทรพยโดยปรากฏรองรอยงดแงะภายในหองพกของผเอาประกนภย ท�ถกลอคไว โดยบรษทจะจายคาทดแทนสาหรบผลประโยชนภายใตขอตกลงคมครองน(ตามความสญเสยหรอความเสยหายท�เกดข(นจรงแตไมเกนจานวนเงนเอาประกนภยตอรายการและชดเชยรวมกนไมเกนจานวนเงนเอาประกนภยรวมตามท�ระบไวในตารางกรมธรรมประกนภย การประกนภายใตขอตกลงคมครองน( ผเอาประกนภย ไมอาจเรยกรองคาสนไหมทดแทนจากขอตกลงคมครองผลประโยชนการลาชาของกระเปาเดนทาง (ถาม) ในเหตการณเดยวกนได

เง"อนไขเฉพาะหมวด 1. การรบชวงสทธ ในกรณท�บรษทไดจายคาสนไหมทดแทนตามขอตกลงคมครองน( ใหแกผเอาประกนภยแลว บรษทจะรบชวงสทธของผเอาประกนภย เพ�อเรยกรองตอบคคลหรอองคกรใดท�รบผดชอบตอความสญเสยหรอความเสยหายน(นเฉพาะในสวนท�บรษทไดจายคาสนไหมทดแทนไป ผเอาประกนภยตองใหความรวมมอกบบรษทในการสงมอบเอกสารตางๆ พรอมดาเนนการท�จาเปนเพ�อปองกนสทธเชนวาน(น และไมกระทาการใดๆ อนเปนท�เสยหายตอบรษท 2. การเฉล"ยความเสยหาย ถาในขณะท�เกดความสญเสยหรอความเสยหายข(นตอทรพยสนท�เอาประกนภย และปรากฏวาทรพยสนน(นไดเอาประกนภยไวกบบรษทอ�น บรษทจะรบผดจายคาสนไหมทดแทนไมเกนสวนเฉล�ยตามจานวนเงนท�บรษทไดรบประกนภยตอจานวนเงนเอาประกนภยท(งส(นและไมเกนกวาจานวนเงนท�บรษทไดรบประกนภยไว 3. หนาท"ของผเอาประกนภยในการรกษาทรพยสน ผเอาประกนภยตองใชความระมดระวงในการดแลรกษาและปองกนทรพยสนตามสมควรเพ�อความปลอดภยของทรพยสนน(น การเรยกรองผลประโยชนความสญหายหรอความเสยหายของกระเปาเดนทางและทรพยสนสวนตวท"อยภายในกระเปาเดนทาง ผเอาประกนภย จะตองสงหลกฐานดงตอไปน( ใหแกบรษทภายใน 30 วน นบจากวนท�เกดเหตกระเปาเดนทางและทรพยสนสวนตวสญเสยหรอเสยหาย โดยคาใชจายของตนเอง

1) แบบฟอรมการเรยกรองคาสนไหมทดแทนท�กาหนดโดยบรษท 2) หนงสอรบรองเปนลายลกษณอกษรจากผบรหารของโรงแรมท�เกดเหต กรณความสญเสยหรอความเสยหายน(นเกดข(นในขณะท�กระเปาเดนทางและทรพยสนในกระเปาเดนทางของผเอาประกนภยอยในความดแลของพนกงานโรงแรมหรอผขนสง 3) สาเนาบนทกประจาวนของตารวจในทองท�เกดเหตในกรณความสญเสยหรอความเสยหายน(นเกดจากการใชกาลงประทษรายหรอโดยการขเขญจากบคคลอ�น 4) สาเนาบตรประชาชน และ/หรอหนงสอเดนทางของผเอาประกนภย 5) หลกฐานอ�นๆ ท�บรษทรองขอตามความจาเปน

การไมสงหลกฐานภายในระยะเวลาดงกลาวไมทาใหสทธในการเรยกรองเสยไป หากแสดงใหเหนไดวา มเหตอนสมควรท�ไมสามารถสงหลกฐานดงกลาวไดภายในระยะเวลาท�กาหนด แตไดสงโดยเรวท�สดเทาท� จะกระทาไดแลว ขอยกเวน 1. การประกนภยตามขอตกลงคมครองน>ไมคมครองความสญเสยหรอความเสยหายของทรพยสน ดงตอไปน>

1.1 กระเปาท"มลกษณะไมใชกระเปาเดนทาง ไดแก กระเปาสตางค กระเปาถอ กระเปาท"มลกษณะการใชท"มใชอยางกระเปาเดนทาง กระเปาใสคอมพวเตอร 1.2 สตวเล>ยง 1.3 ยานพาหนะท"เดนดวยเคร"องยนตและอปกรณสวนควบ จกรยาน 1.4 สกหมะ เคร"องใชภายในบาน เคร"องดนตร โบราณวตถ เคร"องครว 1.5 ของท"ระลก สนคาหรอสนคาตวอยาง 1.6 สราหรอเคร"องด"มท"มสวนผสมของแอลกอฮอล 1.7 โทรศพทเคล"อนท" อปกรณส"อสาร อปกรณอเลคทรอนคสทกชนด 1.8 เคร"องคอมพวเตอร (รวมอปกรณสวนควบและโปรแกรมขอมล) ขอมลท"บนทกอยในเทป โปรแกรม แผนดสก บตรบนทกขอมลหรอส"งอ"นใดในลกษณะเดยวกน 1.9 อปกรณท"เชาหรอใหเชา 1.10 ทองแทง เงนแทง เงนรปพรรณ เคร" องเงน เคร" องทองท> งปวง เงนสด เหรยญกษาปณ เคร"องประดบมคา เชน เพชร อญมณ 1.11 ใบหน เอกสารตางๆ ตcวเงน เชคเดนทาง เอกสารการเดนทาง 1.12 คอนแทคเลนส ฟนปลอม ขาแขนปลอม 1.13 ทรพยสนท"งายตอการแตกหก (Fragile articles) เวนแตเปนความเสยหายท"เกดจากไฟไหม ยานพาหนะท"ผเอาประกนภยโดยสารอย 1.14 กระเปาเดนทางของผเอาประกนภยท"สงไปลวงหนา หรอของท"ระลก หรอทรพยสนท"สงทาง ไปรษณย หรอแยกขนสงไปตางหาก โดยมไดไปดวยกนกบผเอาประกนภย 1.15 ทรพยสนท"ไดรบการชดใชจากโรงแรมหรอผขนสงแลว

2. การประกนภยตามขอตกลงคมครองน> ไมคมครองความสญเสยหรอความเสยหายอนเกดจากหรอสบเน"องจากสาเหต ดงตอไปน>

1.1 การสกหรอหรอการสกกรอน การเส"อมสภาพทละนอย แมลงกดกน สตวทาลาย การเส"อมสภาพในตวเอง หรอความเสยหายท"เกดข>นเน"องจากกระบวนการดาเนนการโดยผ เอาประกนภยเพ"อการซอมแซม การทาความสะอาด หรอการดดแปลงแกไขทรพยสน 1.2 การร"วหรอการซมของทรพยสนท"เปนผงหรอของเหลวท"เกบหรอบรรจอยในกระเปาเดนทาง 1.3 การถกหนวงเหน"ยว การถกยด การถกรบ หรอการถกทาลายทรพยสนโดยคาส"งเจาหนาท"ศลกากรหรอพนกงานผมอานาจตามกฎหมาย การขนสงสนคาผดกฎหมาย หรอการทาการอ"นใดท"ขดตอกฎหมาย

ผลประโยชนการลาชาของกระเปาเดนทาง

ความคมครอง การประกนภยน( ใหความคมครองในกรณกระเปาเดนทางของผเอาประกนภย ซ� งอยในความดแลของผ ขนสงมาถงลาชา หลงจากท�ผเอาประกนภยเดนทางไปถงจดหมายปลายทางตามกาหนดการเดนทาง บรษทจะจายคาทดแทนคาใชจายใหในกรณท�การเดนทางของผเอาประกนภยถกเล�อนหรอลาชาออกไปต(งแต 6 ช�วโมงตดตอกนจากกาหนดการ โดยบรษทจะจายคาทดแทนใหไมเกนจานวนเงนจากดความรบผดท�ระบในตารางกรมธรรมประกนภยแกผเอาประกนภย สาหรบทก 6 ช�วโมงเตมตอเน�องของการลาชาของกระเปาเดนทาง บรษทจะจายคาทดแทนสาหรบผลประโยชนภายใตขอตกลงคมครองน( สงสดไมเกนจานวนเงนเอาประกนภยตามท�ระบไวในตารางกรมธรรมประกนภย และผเอาประกนภยไมอาจเรยกรองคาสนไหมทดแทนจากขอตกลงคมครองผลประโยชนการสญหายของกระเปาเดนทางและทรพยสนสวนตวท�อยภายในกระเปาเดนทาง (ถาม) ในเหตการณเดยวกนได

เง"อนไขเฉพาะหมวด ในกรณท�บรษทไดจายคาสนไหมทดแทนตามขอตกลงคมครองน( ใหแกผเอาประกนภยแลว บรษทจะรบชวงสทธของผเอาประกนภยเพ�อเรยกรองตอบคคลหรอองคกรท�รบผดชอบตอเหตการณดงกลาว เฉพาะในสวนท�บรษทไดจายคาสนไหมทดแทนไป ผเอาประกนภยจะตองใหความรวมมอกบบรษทในการสงมอบเอกสารตางๆ พรอมดาเนนการท�จาเปนเพ�อปองกนสทธเชนวาน(น และจะไมกระทาการใดๆ อนเปนท�เสยหายตอบรษท การเรยกรองผลประโยชนการลาชาของกระเปาเดนทาง ผเอาประกนภยจะตองสงหลกฐานดงตอไปน( ใหแกบรษทภายใน 30 วน นบจากวนท�เกดเหตลาชาของกระเปาเดนทาง โดยคาใชจายของตนเอง

1) แบบฟอรมการเรยกรองคาสนไหมทดแทนท�กาหนดโดยบรษท 2) หนงสอยนยนการลาชาจากผทาการขนสงหรอสายการบน 3) ใบเสรจรบเงนตนฉบบจากการซ(อเส(อผาหรอของใชสวนตวท�จาเปน 4) สาเนาหนงสอเดนทางของผเอาประกนภย

การไมสงหลกฐานภายในระยะเวลาดงกลาวไมทาใหสทธในการเรยกรองเสยไป หากแสดงใหเหนไดวามเหตอนสมควรท�ไมสามารถสงหลกฐานดงกลาวไดภายในระยะเวลาท�กาหนด แตไดสงโดยเรวท�สดเทาท�จะกระทาไดแลว

ขอตกลงคมครอง ผลประโยชนการจ>เคร"องบน

คาจากดความ การจ>เคร"องบน หมายถง การเขายดครองหรอเขาควบคมเคร�องบนโดยการใชกาลงบงคบหรอใชความรนแรง หรอขวาจะใชกาลงหรอความรนแรงดวยเจตนาราย ความคมครอง เปนท�ตกลงกนวา ในระหวางท�กรมธรรมประกนภยมผลบงคบ หากผเอาประกนภยตกเปนตวประกนในเหตการณจ( เคร�องบน และการจ( เคร�องบนน(นเกดข(นเปนระยะเวลาอยางนอย 6 ช�วโมงตดตอกน บรษทจะจายคาชดเชยสาหรบทกๆ6 ช�วโมงเตมตอเน�องของการตกเปนตวประกน ใหไมเกนจานวนเงนจากดความรบผดท�ไดระบไวในตารางกรมธรรมประกนภยแกผเอาประกนภยหรอหนงสอรบรองการประกนภย การเรยกรองผลประโยชนการจ>เคร"องบน 1. ตzวเคร�องบน แสดงวาผเอาประกนภยไดโดยสารไปกบเคร�องบนท�ถกจ(น(น 2. เอกสารยนยนการถกจ( เคร�องบน (ถาม)

ขอตกลงคมครอง ผลประโยชนความรบผดตามกฎหมายตอบคคลภายนอก

คาจากดความ บคคลภายนอก หมายความถง บคคลใด ๆ นอกเหนอจากบคคลในครอบครว หรอบคคลท�อยดวยกนกบผเอาประกนภย ลกจางของผเอาประกนภยขณะอยในระหวางทางการท�จาง และบคคลผรวมเดนทางในคณะของผเอาประกนภย ความคมครอง การประกนภยน( ใหการคมครองความรบผดตามกฎหมายตอบคคลภายนอกของผเอาประกนภย ท�เกดข(นโดยอบตเหตในระหวางระยะเวลาการเดนทาง ซ� งผเอาประกนภยจะตองรบผดตามกฎหมาย ตามจานวนท�เสยหายจรงท(งน( เน�องจาก 1. การเสยชวต หรอการบาดเจบโดยอบตเหตของบคคลภายนอก 2. การสญเสย หรอความเสยหายตอทรพยสน โดยอบตเหตของบคคลภายนอก ท(งน(บรษทจะจายทดแทนใหไมเกนจานวนเงนจากดความรบผดท�ไดระบไวในตารางกรมธรรมประกนภย การเรยกรองผลประโยชนความรบผดตามกฎหมายตอบคคลภายนอก 1. กรณท�บคคลภายนอกเกดการบาดเจบ

ขอยกเวน การประกนภยในขอตกลงคมครองน>ไมคมครอง 1. ความเสยหายตอทรพยสนของบคคลใดๆ ซ"งเก"ยวของเปนญาตและบคคลผรวมเดนทางดวยหรอลกจาง หรอตามกฎหมายใหถอวาเปนลกจางของผเอาประกนภย 2. ความเสยหายตอทรพยสนซ"งเปนของผเอาประกนภยเองหรอท"อยในการครอบครองหรอการควบคมตามกฎหมายของผเอาประกนภย 3. ความรบผดทางการคาหรอวชาชพ หรอการงานของผเอาประกนภยหรอของครอบครว เง"อนไขเฉพาะหมวด ผเอาประกนภยตองไมกระทาการอนเปนการตกลงชดใชหรอยอมรบผดตอบคคลภายนอกหรอบคคลอ�นหรอผเสยหาย หรอการกระทาอนกอใหเกดการฟองรองหรอตอสคดโดยไมไดรบการยนยอมเปนหนงสอจากบรษท ท(งน(บรษทจะแตงต(งตวแทน เพ�อดาเนนการเจรจากบผเสยหายดงกลาว เม�อไดรบแจงจากผเอาประกนภยถงการรบผดชดใชตามกรมธรรมประกนภยน(

ผลประโยชนการเคล"อนยายทางการแพทยฉกเฉนหรอการเคล"อนยายกลบภมลาเนา ความคมครอง

การประกนภยน( ใหความคมครองการบาดเจบหรอการเจบปวยท�เกดข(นอยางกะทนหนและไมสามารถคาดการณได ซ� งเกดข( นในระหวางการเดนทาง จนเปนเหตใหผ เอาประกนภยตองเคล�อนยายเพ�อการรกษาพยาบาลไปอกสถานท�หน�ง หรอตองสงตวผเอาประกนภยกลบสภมลาเนา ตามความจาเปนทางการแพทยและมาตรฐานทางการแพทย

บรษทท�ไดรบมอบอานาจจะเปนผตดสนใจท(งหมดเก�ยวกบการดาเนนการจดหาและกาหนดวธการหรอรปแบบของการเคล�อนยาย และจดหมายปลายทาง เพ�อการเคล�อนยายผเอาประกนภยโดยวธการท�เหมาะสมท�สดโดยพจารณาจากความรนแรงของอาการของผเอาประกนภยตามความจาเปนทางการแพทยเพยงอยางเดยว ซ� งอาจรวมถงคาพาหนะเพ�อใชขนสงคนเจบทางอากาศ ทางเรอ ทางบก ทางรถไฟ หรอวธการขนสงอยางอ�นท�เหมาะสม และอยบนพ(นฐานของการรกษาพยาบาลท�จาเปน โดยบรษทจะจายคาสนไหมทดแทน เพ�อเปนคาใชจายในการใหบรการและ/หรอการดาเนนการดานการเดนทาง การบรการทางการแพทย และคายาท�จาเปนอนเกดจากการเคล�อนยายการทางแพทยฉกเฉน เปนตน ใหแกบรษทท�ไดรบมอบอานาจโดยตรง รวมกนแลวไมเกนจานวนเงนเอาประกนภยตามท�ระบไวในตารางกรมธรรมประกนภยหรอหนงสอรบรองการประกนภย

ท(งน(ผเอาประกนภย ญาตหรอผท�เก�ยวของตองรบแจงบรษทท�ไดรบมอบอานาจ ทราบโดยไมชกชา ในกรณผเอาประกนภยบาดเจบหรอเจบปวยในถ�นทรกนดาร ผเอาประกนภย ควรตดตอแพทยทองถ�นเพ�อทาการรกษาพยาบาลเบ(องตน (First Aid) หลงจากน(น ทางบรษทท�ไดรบมอบอานาจจะเปนผพจารณาวธการเคล�อนยายและประสานงานกบแพทยสาหรบการรกษาคร( งตอไป

แตหากผเอาประกนภยหรอผรวมเดนทางไมสามารถแจงใหบรษทท�ไดรบมอบอานาจทราบ และมเหตผลอนสมควรสาหรบคาใชจายเกนสมควรท�เกดข(นและไมสามารถควบคมไดในระหวางการรกษาพยาบาลฉกเฉนในท�ใดท�หน� ง ในกรณน( บรษทจะชดใชเงนท�ผเอาประกนภยไดสารองจายไปกอนเฉพาะในสวนคาใชจายท�เกดข(นสาหรบการบรการท(งหลายเหลาน(น โดยใหเปนไปตามคาใชจายเชนเดยวกนกบในสภาวการณท�บรษทท�ไดรบมอบอานาจไดกาหนดไว

ความคมครอง การประกนภยน( ใหความคมครองถงการบาดเจบหรอการเจบปวยท�เกดข(นอยางกะทนหนและไมสามารถคาดการณไดในระหวางการเดนทาง ซ� งมผลใหผเอาประกนภยเสยชวตภายใน 30 วนนบจากวนท�ผเอาประกนภยไดรบการบาดเจบหรอการเจบปวยน(น บรษทท�ไดรบมอบอานาจจะดาเนนการจดเตรยมการนาศพหรออฐของผเอาประกนภยกลบสภมลาเนาหรอการจดงานศพตามประเพณทองถ�นท�เปนสถานท�ท�ผเอาประกนภยเสยชวต ในกรณท�ครอบครวของผเอาประกนภยรองขอ โดยบรษทจะจายคาใชจายท�เกดข(นใหแกบรษทท�ไดรบมอบอานาจโดยตรง ตามจานวนเงนท�ตองจายจรง สงสดไมเกนจานวนเงนเอาประกนภยตามท�ระบไวในตารางกรมธรรมประกนภยหรอหนงสอรบรองการประกนภย ในกรณท�มคาใชจายในการขนสงศพ หรออฐ หรอคาใชจายในการจดงานศพตามประเพณทองถ�นท�ไดมการสารองจายไปกอน บรษทจะชดใชคนโดยเขาเปนกองมรดกของผเอาประกนภย ตามคาใชจายท�เกดข(นจรง สาหรบการบรการตางๆ และการเตรยมการของผจดการศพ ท(งน( ใหรวมถงการเตรยมการของผจดการศพหรอสปเหรอ คาหบศพ การดองหรออาบน( ายาศพ การเผา และคาใชจายอ�นๆ ในลกษณะเดยวกนท�ไดดาเนนการไปแลว

บรษทท�ไดรบมอบอานาจจะเปนผดาเนนการจดหาตzวเดนทาง โดยทางเคร�องบน ทางบก หรอทางเรอ (คาโดยสารช(นประหยดในกรณท�เปนไปได) และคาท�พกท�จาเปนและสมควรของสมาชกในครอบครวท�บรรลนตภาวะของผเอาประกนภยจานวน 1 คน ในการเดนทางไปเย�ยมและอยกบผเอาประกนภยจนกวาผเอาประกนภยจะสามารถเดนทางกลบสภมลาเนา โดยบรษทจะจายคาใชจายท�เกดข(นใหแกบรษทท�ไดรบมอบอานาจโดยตรง ท(งน( รวมกนสงสดไมเกนจานวนเงนเอาประกนภยตามท�ระบไวในตารางกรมธรรมประกนภยหรอหนงสอรบรองการประกนภย

DOMESTIC TRAVEL ACCIDENT INSURANCE (GROUP)

In reliance upon the statement made in the proposal for insurance which is considered as part

of this Insurance Policy, and in consideration of the premium paid by the insured, and subjected

to the terms, conditions, general insuring agreement, coverage, exclusions and endorsement of

this insurance, the Company agrees to the Insured as follows:

SECTION 1 – DEFINITION

Wordings and explanations with the specific meaning herewith in any part of insurance policy

1.10 Nurse refers to Person who is authorized for the license as a nurse

practitioner legally.

1.11 Hospital refers to Any clinics which provide medical services by providing an

overnight accommodation for patients and fully equipped

place and service management with enough medical

professions. Especially the major surgery room facility and

authorized from clinic registration of according to the law of

clinic in those territory

1.12 Medical Center refers to Any clinic who provide the medical service with overnight

accommodation and authorized from clinic registration of

according to the law of clinic in those territory.

1.13 Clinic refers to A conventional clinic who authorized legally under the

operation of medical profession for medical treatment and

diagnosis without overnight accommodation.

1.14 Transporter refers to Commercial airline, train, ocean liner, ferryboat, coach, bus

which the Insured has used for his/her travel.

1.15 AIDS refers to Acquired Immune Deficiency Syndrome (AIDS) caused by

HIV virus and include infected from Opportunistic Infectious

Microorganisms, Malignant Neoplasm, infections, or any

sickness resulted from the red blood test shown as positive

Human Immuno Deficiency Virus. The Opportunistic

Infectious Microorganisms included but not limited to

disease which caused pneumonia or Pneumocystis Carinii

Pneumonia, disease cause Organism or Chronic Enteritis,

Virus, and/or Disseminated Fungi Infection, Malignant

Neoplasm. It includes but not limited to only Kaposi’s

Sarcoma, Central Nervous System Lymphoma, and/or any

pestilential where the present known as Acquired Immune

Deficiency Syndrome or any cause of death immediately,

sickness, or disablement. AIS include Human Immuno

Deficiency Virus (HIV), or Encephalopathy Dementia, and

virus epidemic.

SECTION 2 - GENERAL CONDITION

2.1 INSURANCE AGREEMENT

This insurance agreement is arising from the trusting of company in the declaration by

the insured under insurance application and additional declaration (if there is any) wherein the

insured signed for an evidence in insuring agreement, thereby the company had issued this

insurance policy.

In the event that the insured had already known, but declared false statements in the

declaration upon paragraph 1 or had known in any representation but nondisclosed of that

representation by not informing to the company. If the company had known that representation,

it may influence the company to increase the premium or to reject for the agreement. This

insurance contract is voidable according to the clause 865 of the Civil and Commercial Code.

The company has a rights in avoidance of the contract, and the company shall not deny for the

liability by using any declaration other than the one where the insured declared in the document

according to paragraph 1

2.2 COMPLETION OF THE CONTRACT AND AMENDMENTS IN INSURANCE CONTRACT

This insurance policy together with the insuring agreements and endorsement are

forming as part of insurance contract. Any amendments of statements in the contract must be

approved by the company and recorded in this insurance policy and endorsement before such

changes became valid.

2.3 TRAVEL DURATION AND PERIOD OF INSURANCE

2.3.1 This insurance policy covers any loss or damage of the Insured which occurred

within the period of the voyage, but limited to the travel duration of not exceeding

to 30 consecutive days. In case of medical emergency, hijack, delay, or

inoperative plane where the insured is the passenger which made the insured to

not able to travel back within the specified period of insurance, this insurance

policy will automatically extend the coverage until the insured had arrived to the

destination as per the coverage under this insurance policy.

2.3.2 Travel duration of the Insured shall be started and ended within the period of

insurance.

2.3.3 In case of one-way coverage as stated in insurance schedule and/or the

certificate of insurance, the coverage shall start when the insured checks-in at the

airline counter service or the harbor (where applicable) and continuously until the

insured arrived to the arrival terminal building, bus terminal, or harbor at the

destination (where applicable).

2.3.4 In case of round trip coverage as stated in the insurance policy schedule and/or

the certificate of insurance, the coverage shall start when the insured checks-in at

the airline counter service or the harbor (where applicable) and continuously until

the insured arrived to the arrival terminal building, bus terminal, or harbor (where

applicable) for the return trip.

2.4 CHANGE OF VEHICLE DURING THE TRIP

If the insured needs to change the vehicle during the trip as part of the covered trip

caused by uncontrollable factor of the insured, the coverage still be effective as if there is no

change of the vehicle.

2.5 NOTIFICATION AND CLAIMS

The policyholder, the insured, the beneficiary or the representative of those

aforementioned in each case, must inform the company in case of injury or sickness as soon. In

case of loss of life, it has to be informed immediately, except there is any provable appropriate

reasons for that inability to inform the company and the effort to do so as soon as possible.

2.6 DUTY IN KEEPING THE RIGHT OF THE COMPANY FOR THE SUBROGATION

Under the company’s expenses, the insured must do anything which is necessary or as it

is a reasonable request from the company either before or after claim payment from the

company in order to keep the right of the company in claiming from third party on behalf of the

insured.

2.7 DUTY TO MAINTAIN FOR SALVAGE OF THE DAMAGED PROPERTY

Once the damage incurred to the property insured, the insured must not leave those

properties and must deliver those damage properties to the company, in case the company

requires the damage properties for claim consideration.

2.8 CLAIM COMPENSATION

The company will make a claim payment in case of death to the beneficiary and for other

compensations to the insured within 15 days since the company receives the completed and

corrected documents.

In case that there is any reasonable doubt for company to make claim compensation as

aforementioned which not followed in the coverage agreement of insurance policy, the period of

insurance maybe shall be expanded as necessary, but not exceed 90 days since the company

receives all completed documents.

In case the company cannot make the claim payment within the specified period of

insurance above, the company shall pay an additional 15% interests per year of the total

compensation amount since payment due as per paragraphn1 or paragraph 2 depend on each

case.

2.9 DISPUTE BY ARBITRATOR

In case of any argument, dispute or appeal under this insurance policy between the

person who is entitled for compensation versus the company and if so desired by that person to

settle the disputed claim by use of arbitration, the company must conform and allow the case to

be judged by the arbitrator according to the arbitrating regulation governed by the Office of

Insurance Commission.

2.10 AUTOMATIC TERMINATION OF THE CONTRACT

This insurance policy shall be automatically terminated should the insured is committing a

felony or while the insured is arrested, under arrest or escaping from the arrest.

2.11 ANTECEDENT CONDITION

The company shall not be liable to compensate under this insurance policy unless the

policyholder, the insured, the beneficiary or representative on each case has complied with the

insurance contract and the conditions under this policy.

2.12 PREMIUM PAYMENT

The premium must be paid immediately or before the insurance policy starts to cover.

2.13 INSURANCE POLICY CANCELLATION

Both the company and the insured can cancel the policy as follows:

2.13.1 The cancellation must be informed to the company within 48 hours before the

departure time. The company will allow a refund premium to the insured, less the

operating expenses.

2.13.2 The cancellation which is less than 48 hours before the departure time, the

company will not allow for a refund premium except the transporter cancels the

insured’s ticket, then the company shall allow a refund premium to the insured,

less the operating expenses.

SECTION 3 - GENERAL EXCLUSION

This insurance policy will not covered for:

1. Deductible where the insured shall liable by him/herself as per specified on the

policy schedule (if applicable).

2. Any loss or damage incurred from or in consequential of the following causes:

2.1 War, invasion, act of foreign enemies, any malicious acts with an aim

similar to war whether announced or not, civil war which means war

between people in the same country, mutiny, rebellion, insurrection,

revolution, civil commotion, announcement of martial laws, or any events

which may cause to announce or remain on the martial laws.

2.2 Nuclear weapons, radiation or radioactivity from any nuclear fuel or nuclear

waste arising from the combustion of nuclear fuel and any process of self-

sustaining nuclear fission.

3. Terrorism (not apply to the insuring agreement “Hijacking”).

4. While the insured is boarding or travelling in an aircraft which has no license for

carrying passengers or does not operated as a commercial aircraft.

5. While the insured pilots or works as a crew in any aircraft.

6. While the insured is committing a felony or while the insured is being arrested or

escaping from the arrest.

7. While the insured serves as a soldier, police, or a volunteer and participates in war

or crime suppression.

INSURING AGREEMENT

BENEFITS FOR DEATH, DISMEMBERMENT, LOSS OF SIGHT, OR PERMANENT

DISABILITY FROM ACCIDENT

DEFINITION

Dismemberment refers to The loss of body organ from the wrist joint or the ankle joint

and also the loss of use in that organ as per aforementioned by

the medical indication for never be able to function at any time

in the future.

Loss of sight refers to Complete blindness which is permanently incurable.

Total Permanent

Disability

refers to Disability to the extent of being unable to perform the normal

duty in the insured’s regular occupation or any other

occupation totally and permanently.

COVERAGE

This insurance cover for loss or damage arising from bodily insured of the insured from accident

which caused the insured to death, dismemberment, loss of sight or permanent disability within

180 days from the date of the accident or the injury causes the insured to receive continuous

medical treatment as an inpatient in the hospital and loss of life occurred later because of such

injury, the company shall compensate in accordance with the sum insured as specified in the

Schedule as follows:

1. 100% of the sum insured for loss of life.

2. 100% of the sum insured for permanent disability which continue consecutively for not

less than 12 months after the date of accident or if there is any medical indication that

the insured suffers a permanent disability.

3. 100% of the sum insured for loss of both hands from the wrist joint or both feet from the

ankle joint or loss of sight for both eyes.

4. 100% of the sum insured for loss of one hand from the wrist joint and one foot from the

ankle joint.

5. 100% of the sum insured for loss of one hand from the wrist joint and loss of sight for

one eye.

6. 100% of the sum insured for loss of one foot from the ankle joint and loss of sight for one

7. 60% of the sum insured for loss of one hand from the ankle joint.

8. 60% of the sum insured for loss of one foot from the ankle joint.

9. 60% of the sum insured for loss of sight for one eye.

The company shall compensate only one item of loss which has the highest amount through the

period of insurance. The company shall make a claim payment for the consequent of what

happened under this insuring agreement where the total payment will not over than the amount

as specified on insurance schedule. If the company had paid for the compensation herewith less

than full amount of the sum insured, the company still cover the remaining amount until the end

of insurance period.

CLAIM PROCEDURE FOR DEATH

The policyholder, the beneficiary or the representative of aforementioned depend on each case

have to submit the following evidences at their own expenses to the company within 30 days

from the date of insured’s death:

1. Claim form as required by the company.

2. Death Certificate.

3. Copy of perform postmodern report certified by the policeman or organization that issued

the report.

4. Copy of daily police record certified by the official on duty who is an owner of the case or

organization that issued the report.

5. Copy of identification card and census registration stamped “death” of the insured.

6. Copy of identification card and census registration of the beneficiary.

CLAIM PROCEDURE FOR DISMEMBERMENT, LOSS OF SIGHT OR TOTAL PERMANENT

DISABILITY

The policyholder, the insured and/or the beneficiary or the representative of aforementioned

depend on each case have to submit the following evidences at their own expenses to the

company within 30 days from the commencement date of the disability.


2. Medical report with a certification on the dismemberment, loss of sight or total permanent

disability.

Failure to submit the evidences as required above within the time limit prescribed will not

invalidate the right to claim if it can be shown, to the company’s satisfaction, that it was not

reasonably possible to submit such proof within the prescribed time limit but it was done as

soon as practicable for an otherwise legitimate claim.

ADDITIONAL EXCLUSION

This insuring agreement will not covered for:

1.1 Any loss or damage arising from or in consequence of the following causes:

1. Action of the insured while under the influence of alcohol, addictive drugs,

narcotic drugs to the extent of being unable to control one’s mind.

The term “under the influence of alcohol” in case of having a blood test

refers to a blood/alcohol level of 150 mg percent and over.

2. Suicide or attempted to commit suicide or self-inflicted injury.

3. Infections except pyogenic infections, tetanus, or rabies from a wound or

cut suffered as a result of an accident.

4. Medical treatment or surgical treatment except the necessary treatment for

the injury which is covered under this insurance policy and occurring

within the period as per specified in this insurance policy.

5. Miscarriage and abortion.

6. Food toxic.

1.2 Loss or Injury which occurs in the following time:

1. While the insured is on the racing of all kinds of cars or boat, horse racing,

all kinds of skiing including jet skiing, skate racing, boxing, parachuting

(except for the purpose of life saving), while boarding or travelling on the

balloon or glider, bungee jumping, diving with oxygen tank and breathing

equipment under water.

2. While the insured is riding or travelling on a motorcycle.

3. While the insured is taking part in a brawl or taking part in inciting a brawl.

The company reserves the right to ask the insured to get a physical check-up during the

compensation’s consideration including perform autopsy if necessary.

INSURING AGREEMENT

MEDICAL EXPENSES ARISING FROM ACCIDENT

DEFINITION

Inpatient Room

Expense

refers to Room, food, nursing expense, and any other service of the

hospital as daily provided in the hospital or medical center.

Inpatient refers to The insured person, who has admitted for treatment in the

hospital or medical center for more than 6 consecutive hours,

and must be registered as an inpatient. By obtaining the

diagnosis or advice from the doctor in accordance with the

medical standard for the appropriate period of medical

treatment for such injury, and herein includes any person who

dies later within 6 consecutive hours after the confinement.

Alternative Medicine refers to A diagnosis, medical treatment or prevention of disease by

traditional Thai or Chinese medicine or any other medicine

which is not classified as modern medicine.

COVERAGE

In the event that the insured receive medical treatment by doctor or nurse as a direct result of

accidental injury within 52 weeks from the accident date, the company will reimburse to the

insured for necessary and reasonable expenses, in accordance with medical necessary and

standard for actual inpatient room, observation room, medical, and nursing service expenses,

provided that, up to the maximum amount as specified in the Schedule or Certificate of

Insurance, less deductible (if applicable).

In the event that the insured already reimbursed the portion of medical expenses from

government benefit, any other benefit, or any other insurance, the company will only pay the

insured for the rest of medical expenses which are unable to reimburse from such benefit or

insurance.

CLAIM PROCEDURE

The insured have to submit the following evidences at their own expenses to the company

within 30 days from the date of discharging from hospital or medical center, or the date

obtaining medical treatment from clinic:

1. Medical report with specific detail including the diagnosis and treatment.

2. Original receipt showing list of medical expenses or summary sheet showing the closing

statement of finance with the receipt.

The receipt for list of medical expenses must be the original document and the company shall

return the original receipt where the insured had already paid for. So the covered person can

reimburse the rest of medical expenses from other insurance companies. In the event that the

insured already reimbursed the portion of medical expenses from government benefit, any other

benefit, or any other insurance, the insured must submit a copy of the receipt to reimburse the

rest of medical expenses from the company.


This insuring agreement will not covered for

INSURING AGREEMENT

TRIP CANCELLATION

DEFINITION

“Transporter” refers to Commercial airline, train, ocean liner, ferryboat, coach, bus which

insured has used for his/her travel.

COVERAGE

It is agreed that the company will indemnify for expenses incurred from cancellation of the trip

before a journey date where the insured had paid for in advance whether partially or wholly for

deposits, advance booking for the trip, or accommodation cost that had not been used, including

penalty fee arising from the trip cancellation, in the event that the insured was forced to cancel

any part of the trip as the direct and necessary result of the following causes:

1. The death of the insured, serious injury, or sickness certified by the doctor for being able

to travel.

2. The death of the insured’s family member, or serious injury or sickness of such person

which the doctor certified as being life-threatening.

3. The transporter has reported for the trip cancellation from adverse weather conditions

until cannot commencing the trip.

4. Strike or riot which effected the trip of the insured

Provided that, the company will reimburse to the insured for an actual expenses up to the limit

of liability amount as specified in the Schedule or the Certificate of Insurance.

CLAIM PROCEDURE

1. Protest or strike before the start date of insurance.

and mother, spouse, children of the insured, and father and mother of the

THE INSURING AGREEMENT

BENEFIT FOR EXPENSES INCURRED FROM THE TRIP CURTAILMENT

DEFINITION

Natural Disaster refers to any disasters occurred naturally and brought loss or damage to life

and property such as wildfire, flood, storm, earthquake, for example

COVERAGE

This insuring agreement provides cover for advance expenses of the insured for the travel and

accommodation expenses while the insured was on a trip and/or penalty fee or any other

expenses in the event that the insured was forced to return to their home country before the

planned trip as the direct and necessary result of the following:

1. The death, serious injury or sickness, of the insured or the insured’s family member

which the doctor certified as being life-threatening if continuing with the trip, it may force

the insured to return to their home country directly while still on that trip.

2. Natural disaster which prevents the insured from continuing in the trip according to the

plan.

3. Strike, riot, or civil commotion which occurred beyond the insured’s awareness and

control.

4. Quarantine due to the opinion of a doctor.

The company will reimburse for the actual expenses, up to the sum insured amount as specified

in the Schedule or Certificate of Insurance, for advance payments for travelling by the aircraft,

land or ferry boat (economy class fare if applicable) or hotel accommodation which has not been

used and will not be used, or for penalty fee or any other expenses arising from the return to the

insured’s home country before the planned trip, or for loss of frequent flyer points or similar

reward points which have been redeemed for air ticket and the air ticket is subsequently

cancelled.

Provided that the reimbursement will also include any other expenses as a result of quarantine

due to the opinion of a doctor where the insured is unable to reimburse for such expenses

incurred as specified above from that service providers.

This insuring agreement is effective only if this Policy is purchased before the insured became

aware of any circumstances which could lead to the curtailment of the trip.

CLAIM PROCEDURE FOR TRIP CURTAILMENT

The policyholder, the insured, the beneficiary or the representative of insured person depend on

the case; have to submit the following documents or evidences at their own expenses to the

company within 30 days from the date of accident:

3. Copy of the insured’s identification card and/or passport with certified true copy.

9. Any other evidences to verify the causes of trip curtailment.

10. Any other documents or evidences as required by the company (if applicable).

INSURING AGREEMENT

BENEFIT FROM THE TRIP DELAY

COVERAGE

The company shall indemnify for expenses incurred in case that the insured’s trip had been

postponed or delayed for 6 consecutive hours from the time specified in the travel itinerary. The

company shall pay to the insured for the compensation which does not exceed to the limit of

liability as specified in insurance schedule for every full 6 consecutive hours as a result of the

delay, due to the following causes:

1. Adverse weather conditions which affect the transporter to postpone the trip.

2. Mechanical breakdown/derangement of the transporter.

3. Strike or riot occurring after the company had agreed for the insuring of this insurance

policy.

CLAIM PROCEDURE

The policyholder, the insured, or the representative of those aforementioned depend on each

DEFINITION

“Personal Property” refers to personal goods belonging to the insured, cloths, accessories,

which are taken on the trip or acquired by the insured, and those goods must be kept in the

insured’s baggage, and not defined in the exclusion.

“Each item” refers to 1 piece, 1 pair or 1 set.

COVERAGE

This insuring agreement provides cover for loss or damage of baggage and personal property,

which are kept in the baggage, including but not limited to backpack for keeping the insured’s

personal property while the insured is on a trip, during the period of insurance, due to the

following events:

1. Robbery, burglary, or any attempt thereof.

2. Loss of damage of property whilst in the custody of an airline or a hotel.

3. Theft which appears the sign of breaking into the insured’s locked hotel room.

Provided that, the company will indemnify the insured in respect of such actual loss or damage

up to the sum insured amount of each item and in the aggregate as specified in the Schedule

under this insuring agreement.

Under this insuring agreement, the insured are not entitled to make a claim from baggage delay

benefit (if applicable) in the same circumstance.


1. SUBROGATION

In the event of any payment are made by the company to the insured under this insuring

agreement, the company will subrogate all the insured’s rights in recovery against any person or

the organization that responsible for that loss or damage, only in the part that the company had

already paid for. The insured shall cooperate with the company in providing all related

documents including any necessary execution in order to protect that right and will take no

actions which may prejudice to the company.

2. AVERAGE CLAUSE

In the occurrence of loss or damage to the property insured and it appeared that the

property had been insured with other company, the company will liable for claim settlement for

not more than the average amount of money which the company insured toward the total sum

insured and not exceed for the money company had insured for.

3. DUE DILIGENCE

The insured will exercise due diligence and precaution in doing all things for the purpose

of safety of such property.

CLAIMS PROCEDURE FOR BENEFIT TOWARD LOSS OR DAMAGE TO THE BAGGAGE

AND PERSONAL PROPERTY

The insured must submit the following documents to the company within 30 days from the date

of loss or damage to baggage and personal property at their own expense:


2. Written confirmation letter which obtained from an executives of the hotel where such

loss or damage taking place in the event that such loss or damage to baggage and

personal property occurred whilst in the custody of the hotel or transporter.

3. Copy of a police report which obtained from the local police evidencing such loss or

damage arising from physical force or intimidation by other person.

4. Copy of the insured’s identification card and/or passport.

5. Any other documentary evidence as required by the company.

1. This insuring agreement will not cover for any loss or damage to baggage and

personal property if they are:

1.1 Baggage which are not intended for travelling purpose such as wallet,

handbag or laptop bag.

1.2 Pets.

1.3 Motor vehicles and other equipment or bicycles.

1.4 Skiing equipment, indoor equipment, musical instruments, antiques or

kitchenware.

1.5 Souvenirs, goods or samples.

1.6 Liquor or alcoholic beverage.

1.7 Mobile phone, communication equipment or all kinds of electronic

equipment.

1.8 Computers (including hardware and software), data recorded on tapes,

cards, discs or otherwise.

1.9 Hired or leased equipment.

1.10 Gold, bullion, jewelry, silver, cash, coin or precious stones such as

diamonds or gems.

1.11 Stocks and bonds, manuscripts, bank notes, travelers’ cheques or travel

documents.

1.12 Contact lenses, false teeth, or artificial limbs.

1.13 Fragile articles except damage arising from fire to vehicles in which the

insured is boarding.

1.14 Travel baggage of the insured which shipped in advance, souvenir or

property shipped as freight, or separate shipping with the insured.

1.15 Property that had reimbursed from the hotel or transporter already.

2. This insuring agreement will not covered for any loss or damage arising from, or

in consequence of the following causes:

2.1 Wear and tear, gradual deterioration, moths, vermin, inherent vice, damage

sustained while being worked upon, or damage sustained due to any

process initiated by the insured to repair, clean or modify that property.

2.2 Leakage or penetration of property which is in the form of powder or liquid

and stored in the baggage.

2.3 Retention, confiscation or destruction of property by orders of the customs

or other officials, illegal goods transportation or any other acts against the

law.

INSURING AGREEMENT

BAGGAGE DELAY

COVERAGE

This insuring agreement will provide cover in the event that the insured’s baggage which is in

the custody of the transporter is delayed after the insured had arrived to the scheduled

destination.

The company will reimburse for an expenses in case that the trip of the insured had been

postponed or delayed for 6 consecutive hours from the schedule. The company shall

compensate to the insured for not more than the limit of liability as specified on insurance

schedule for every full 6 consecutive hours of postponement or delay of such baggage.

The company shall compensate for the benefit under this insuring agreement up to the

maximum benefit amount as specified in the Schedule. The insured will not be entitled to make

a claim from the benefit of loss or damage to baggage or personal property (if applicable) in the

same circumstance.


In the event of any payment made by the company to the insured under this insuring

agreement, the company will subrogate all the insured’s rights of recovery against any person or

organization who is responsible for such circumstance. The insured shall cooperate with the

company in providing all related documents including any necessary execution in order to

protect that rights and will take no actions which may prejudice to the company.

CLAIM PROCEDURE

The insured must submit the following documents to the company within 30 days from the date

of baggage delay at their own expense:

1. Baggage delay occurring after the end of the insured’s trip as specified in the

INSURING AGREEMENT

BENEFIT IN CASE OF HIJACK

DEFINITION

Hijack refers to any unlawful seizure or exercise of control by forcing, violence, threating for

violence, or violence and with wrongful intent of an aircraft.

COVERAGE

It is hereby agreed that, if, during the Period of Insurance, the insured is detained on the aircraft

due to it being hijacked for at least 6 consecutive hours, the company will compensate to the

insured for every full 6 hours period that insured had been detained, up to the maximum Benefit

amount specified in the Schedule or Certificate of Insurance.

CLAIM PROCEDURE

1. Air ticket to verify that the insured had been the passenger on the hijacked aircraft.

2. Any documents to confirmed the hijack (if applicable).

INSURING AGREEMENT

BENEFIT FOR LEGAL LIABILITY TO THIRD PARTY

DEFINITION

Third Party refers to any person other than family member, companion who lives with the

insured, employee under a contract of service of the insured and any person who is travelling

with the insured’s group.

COVERAGE

This insuring agreement provides cover for legal liability to third party of the insured arising from

any accident during the trip which the insured legally liable to pay compensation from the actual

damage from:

1. Death or Accidental Injury to any third party.

2. Accidental property loss or damage to any third party.

The company will indemnify the insured up to the limit of liability as specified in the Schedule.

CLAIM PROCEDURE

1. In case of accidental injury to any third party:

1.1 Medical certificate in connection with such injury.

1.2 Invoice of medical expenses.

1.3 Copy of identification card of such injured third party with certified true copy.

2. In case of accidental property loss or damage to any third party:

2.1 Receipt of repair of such damaged property or receipt in case of necessarily

purchasing such property from the owner of that property.

2.2 Photo indicating such loss or damage (if applicable).

This insuring agreement will not covered for:

1. Damage to the property of any person who is the insured’s relative or any person

who is travelling with the insured or employee or deemed by law to be the

insured’s employee.

2. Damage to property belonging to or held in trust by the insured, or while in the

insured’s custody or control.

3. Liability arising out of trade or professional service of the insured or the insured’s

family.


The insured will not do anything to admit liability for or settle any claim to any third party, others,

injured person, do things which may incur any litigation without the company’s prior written

consent. The company will appoint legal representative to negotiate with injured third party

when notice has been given by the insured to pay claim under this Policy.

INSURING AGREEMENT

BENEFIT FOR EMERGENCY MEDICAL EVACUATION OR DIRECT REPATRIATION TO

HOME COUNTRY

COVERAGE

This insuring agreement provides the cover for accidental injury or sickness which is emergency

or unpredictable of the insured during the trip, it may cause the insured for emergency medical

evacuation to other places, or direct repatriation to home country upon the requirement

accordance with medical necessary and standard.

All decisions as to the means, methods, format of the transportation and its final destination will

be made by the company’s authorized representative in order to transfer the insured suitably by

considering of the intensity level of the insured’s symptom upon medical necessity, including

expenses incurred to transfer the injured patient which arising from air ambulance, surface

ambulance, ferry boat, railroad or any other appropriate means for the necessary medical

purpose. The company will pay directly to authorized representative in any expenses for the

transportation, medical services and medical supplies necessarily incurred as a result of the

insured’s evacuation, up to the sum insured as specified in the Schedule or Certificate of

Insurance.

Provided that, the insured’s relative or related person must notify the company’s authorized

representative as soon as practicable if the insured is injured or ill in the remote area. The

insured must contact the local hospital to get a first aid. After that, the authorized representative

will then make the necessary evacuation and future medical treatment arrangements at their

own discretion.

If the insured is unable to notify the authorized representative to make the necessary evacuation

arrangements due to reasons beyond reasonably incurred expenses and the insured’s control

during emergency medical treatment at anywhere, the company will reimburse the insured for

advance expenses incurred in relation to the evacuation, up to the amount which the authorized

representative would have incurred for services provided under the same circumstances.

Benefit amount under this insuring agreement will also include additional expenses for

transportation by aircraft, land vehicle or ferry boat (economy class fare if applicable) of the

family member or other person travelled with the insured for 1 person. In the event that such

person is unable to return to his home country within scheduled timeframe due to the insured’s

evacuation.

CLAIM PROCEDURE

The policyholder, the insured or the representative of the insured depend on each case, must

submit the following documents or evidences at their own expenses to the company within 30

days from the date of accident:

5. Copy of receipt of showing list of medical expenses or summary sheet showing the

closing statement of finance with the receipt.

Except for general exclusion, this insuring agreement does not cover any expenses for

any of the following:

1. Any expenses incurred for services provided by another party for which the

insured are not liable to pay, or any expenses already included in the cost of a

scheduled trip where the trip manager or transporter must responsible for.

2. Strike, riot, or malicious acts for benefits in connection with political, religious,

ideological or civil commotion.

INSURING AGREEMENT

BENEFIT FOR THE REPATRIATION OF MORTAL REMAINS OR CREMAINS

COVERAGE

The company’s authorized representative will make the necessary arrangements for the return

of the insured’s mortal remains to their home country or the traditional cremation at the place

where the insured suffer death as required by the insured’s family. The company will pay such

actual expenses directly to the authorized representative up to the actual amount with the

maximum not higher than the sum insured amount as specified in the Schedule or Certificate of

Insurance.

In addition to expenses for the transportation of the remains or traditional cremation, the

company will reimburse to the insured’s estate expenses actually incurred for services and

supplies by a mortician, including but not limited to the preparation cost of mortician or

undertaker, cost of a casket, the embalming, the enamel of body, cremation, and other

expenses in the same type which had already performed.

Benefit amount under this insuring agreement as mentioned above will include additional

expenses for transportation by aircraft, land vehicle or ferry boat (economy class fare if

applicable) of the family member or other person travelled with the insured for 1 person in the

event that such person is unable to return to his home country within scheduled timeframe due

to the repatriation of mortal remains or cremains of the insured in this time.

Provided that the beneficiary, relative or the representative of the insured must notify the

company’s authorized representative as soon as practicable, the authorized representative will

then make the best arrangements, at their own discretion, for repatriation of mortal remains.

BENEFIT FOR THE COMPASSIONATE VISIT EXPENSES AT NON-LOCAL HOSPITAL

COVERAGE

This insuring agreement provides the cover for the event that the insured is confined in a non-

local hospital for more than 7 consecutive days as a result of accidental injury or sickness which

is emergency or unpredictable of the insured during the trip and their medical condition forbids

evacuation and no family member (who is over 20 years old) is with the insured.

The company’s authorized representative will make the necessary arrangements of the ticket by

air, land or ferry boat transportation (economy class fare if applicable) and hotel accommodation

expenses necessarily incurred for 1 relative, who is required to visit and stay with the insured

INSURING AGREEMENT

BENEFIT FOR TRIP MISCONNECTION

COVERAGE

This insuring agreement provides the cover for missing the flight which already confirmed for the

route according to the flight schedule while the insured commuted on the trip. At the point

needed to have a transferring plane from the flight delay of the commuting flight which arrived at

that connecting point, and no other flight or other method of the trip to be replaced within the

period at least 6 hours from the time that the present plane arrived at the transferring point
