Many production companies call SKM regarding children for various television shows, feature films, commercials, music videos and some print-work. SKM helps guide you appropriately for your new adventure in the entertainment industry with you and your family. And help protect your family standards and values in future projects that may come your way. SKM has been in operation for many years by Lisa Santillan, a Christian mother of five (5) children, including a set of twin boys of her own. WHAT DOES IT TAKE TO BECOME AN EXTRA? A great attitude and an eagerness to want to learn about the entertainment industry. Since there is no speaking involved – NO classes are needed. You would need a great picture of the child, like a current snapshot from your own camera or from other sources and you will need a current Entertainment Work Permit. An “Extra” can be anyone! We are always looking for all different types of kids. When working as an “Extra” we need all ethnicities, shapes, ages, sizes, and unique character looking kids. Sometimes we will need Mom & Dad’s too! TWINS, TRIPLETS AND QUADRUPLETS! Yes, we also specialize in multiples. We have represented a lot of multiples on many amazing projects. Heard of…Scandal, Annabelle, Guys with Kids, Bones, Up All Night, Parenthood, Shameless, Dexter, Friends, Scrubs, Desperate Housewives, ER, and much more show but those babies came from SKM. Since Lisa, the owner has twins she has a tender approach when working with the most fragile babies and kids. Studios love multiples because of the “hour” issue. Identical twins, triplets and quadruplets are all welcomed. WHAT WILL HAPPEN IF YOU SIGN WITH SKM? Studio Kids Management is not a Talent Agent or Talent Agency. We are a Talent Listing Management Company! You and your child are about to embark on a new and exciting adventure. We will educate and advise you of your child’s career and get them all that they need to start their career as a new young actor. SKM will keep all of your documents needed for your child’s career will be in order with the help of the parent or guardian. SKM will keep all documents current and available for production companies who wish to hire you. Studio Kids will take a 20% Management fee. WHAT WILL I NEED TO GET STARTED? First – You will need to obtain an Entertainment Work Permit (Free) and must renew them every six months. A current copy will be needed in our office every time. SKM will help you obtain these documents. You can get it online or by mail. Second – You will need to send us a photo (current). We require you send (email or mail) us a least 1 or 2# - 4x6 color photos with the child behind a white background when registering. This is the great part…just send a simple cell phone photo. OR if you have professional ones we can use those too. Send or email with your SKM Registration Packet. LET’S TALK ABOUT SCHOOL AS AN EXTRA- Your child will be educated on every set when hired. It is mandatory by law to have all school-aged children in school during a calendar school year. A licensed state certified School Teacher will be hired by the production company who hires your child to have on set. They will ask each child to bring their appropriate schoolwork with them to set. HOW MUCH WILL I MAKE AS AN EXTRA? When a production company hires a non-union & union minor as an Extra they will pay them $178.00 minimum for each day they work. If a minor works on a Union background commercial as an Extra they will pay $388.40 minimum per day. As an Extra they can be hired by production to be a Photo Double or Stand-In, since there is no speaking involved. And that pays a little more. At the discretion of some Directors extras can be upgraded on any project they wish. All rates are never negotiated by Studio Kids. HOW DO I GET MY CHILD REGISTERED WITH STUDIO KIDS? …Everything is done by mail

CHILDREN – 15 days to 17 years old -

1. Download the ‘Studio Kids Packet’ on our website www.studiokidsmanagement.com for your child(ren) and all other pertinent documents to get started.

2. Send/Email two (2) current 4x6 photos of the same look and mail with the packet to our office by email or mail. 3. Obtain your Entertainment Work Permit and make a copy for SKM to make your child ‘active’. 4. Once the Entertainment Work Permit is received we will add your child to our system. 5. Take your minor to get I9’d (making sure they have the proper documents to work in California) in La Mirada. 6. Send everything to our office…we will send an email confirmation once we receive all or some of your documents. 7. You will then receive in the mail an additional packet containing multitudes of information about the entertainment industry to

guide you and an email link will be sent to you with our information. 8. Once your child has booked their first booking then a one-time processing fee of $30.00 for each minor child will be charged

after they have received their paycheck. 9. Remember: All documents needed to complete your child’s folder will be necessary to consider your child “Active”. Which

includes a current work permit, photo and all SKM documents. 10. A final confirmation letter will be sent by email to client when all documents are received.

As an already experienced mother of twins in the business and a Christian mother of five (5) children, I can relate to the family issues we all come across of wanting to try it all. SKM is a unique Talent Listing Management company that will carefully guide you and your family in opportunities that you have never experienced. If your life is busy with sports, church, and keeping good grades up in school, SKM will encourage you to make the best possible decision for your child and family. Studio Kids Management is not a Talent Agency or Talent Agent. Get your map books out for an awesome adventure. EXPLANATION OF DOCUMENTS YOU WILL RECEIVE FROM STUDIO KIDS:

TELL ME ABOUT STUDIO KIDS Letter

DEAR NEW/PROSPECTIVE CLIENT – Checklist of procedures of Signing with STUDIO KIDS Management.

CLIENT INFORMATION SHEET – (2 pages) – Filled out and return back to STUDIO KIDS.

TALENT LISTING MANAGEMENT AGREEMENT – (2 pages) – Filled out and return back to STUDIO KIDS.

Explains that we will represent your child at a management level. It states that SKM is not a Talent Agency/Agent. It will explain the duties and requirements of our company and the talent. Two (2) signatures are needed!!

Signing: 1. Parent Signature Section Signing: 2. Acknowledging your Right to Cancel Section (using today’s date)

CHECK AUTHORIZATION SHEET/Change of Address - (1 page) – Filled out and return back to STUDIO KIDS.

This allows the production companies’ payroll department to discuss & send the talent’s check to SKM. Most checks come directly to SKM. Commissions will be deducted and remainder sent to minor within 10 days of receiving check. Checks can take up to 30 days to receive from payroll company. The Talent will receive one check for each project they have done. Taxes are taken out of the child’s check by the payroll department. It is the responsibility of the parent to handle their taxes at the end of the year. In January, look for the W2’s for all the projects they have done. If you don’t receive them call the payroll department on their voucher or paycheck. Also allows payroll companies to have conversation with Studio Kids about adjustments, reissues and problems pertaining to paycheck.

SKILLS – (1 page) – Filled out and return back to STUDIO KIDS. List of what your child’s hobbies, sports, acting abilities, languages, etc.

OPT-IN on COOGAN TRUST FOR MINORS FORM - (1 page) – Parent sign your name only – and return to STUDIO KIDS.

As of January 1, 2014 Coogan Trust Account is no longer required for Background Extras. However, if you are interested in participating with the Coogan Trust procedures you are welcome to obtain, but not obligated. If you are Principal Actor (someone who receives a speaking part) you will be required by Law to obtain the Coogan Trust Account. The employer/payroll company will deduct 15% of the gross amount of your child’s check and hold it until YOU show proof of the account you establish. The document must have written somewhere by the bank “Coogan Trust” on it. The money will be held in the account until the child is 18 years old. At no time are you allowed to withdrawal this money. You may establish a Coogan anywhere of your choosing. Please make sure you send SKM a copy of the Coogan document by mail or fax. We will forward the information onto this form once we receive your bank name & info. Coogan Trust holders prior to December 31, 2013 it is the parent’s responsibility to maintain and continue with the Coogan Trust Account.

Temporary Postponed/FILLING OUT I9’S – AFTER COVID All NEW Studio Kids clients will need to make an appointment, bring the minor and their original documents to fill out the I9 form (we will supply them). This form is to verify the identity and employment authorization of each new employee (both citizen & noncitizen). You will need to bring Original documents from the I9 List of Acceptable chart included in this packet. Minor must be present when filling out the documents. They must have their OWN EMAIL ADDRESS. Some production companies will not potentially hire your minor if this procedure is not complete. This is mandatory for all clients. Once COVID is over we can resume the I9 procedure.

POSTPONED UNTIL AFTER COVID – I9’s FORM I-9, EMPLOYMENT ELIGIBILITY VERIFICATION FORM – COPY ONLY - You will fill this out at Postal Stop in La

Mirada. Postal Stop will give you the documents. It is to prove your minor is a United States Citizen. FORM I-9 LIST OF ACCEPTABLE DOCUMENTS: List of usable documents. All must be unexpired.

Signing: Employee’s Signature (parent you sign this) Signing: Preparer Certification (parent you sign this) Send your documents to SKM from List A or List B & C

ENTERTAINMENT WORK PERMIT FORMS – See Instructions – Online service & Mail in obtions. This is mandatory for all children – Issued FREE of charge. No Social Security numbers are required. Application CANNOT be faxed. Any grades listed unsatisfactory will NOT be accepted by the Labor Board. You may try again to obtain a WP when the grades are satisfactory. NON-SCHOOL AGE CHILDREN (1st Grade and under) – the TOP portion should be filled out by the parent and attach a copy of either the child’s Certified Birth Certificate, copy of Baptismal Certificate, Official letter from hospital where born, passport (all listed on the back of the WP application). Obtain by mail or online. INFANTS – (15 days to 30 days old) – the TOP portion should be filled out by the parent. The infant must be at full gestation (due date) and must add 15 days from their DOB in order to be ready to work and must weigh (5) five pounds and above. In addition, to the official document included the LOWER portion of the WP application must be filled out by their doctor for approval of good health in order to work. Obtain by mail or online. SCHOOL AGED CHILDREN – WHEN SCHOOL IS IN SESSION - (1st Grade to 12th Grade) – The TOP portion must be completely filled out by parent and the MIDDLE section is to be given to their school and signed by an authorized school official. Make sure the signature is NOT a stamp signature and make sure the authorized signer includes their title. Have them stamp the document with their official school seal/stamp. VERY IMPORTANT! SUMMERTIME/HOLIDAYS – the application must be filled out according to their appropriate category. However, if your child is on Summertime break a copy of their most recent report card needs to be attached to the application. The Work Permit will only be issued until approximately Mid-September when school normally resumes. The process then has to been done all over again…but, with the school’s signatures. Same applies for Holidays.

WHERE TO GET YOUR ENTERTAINMENT WORK PERMIT: List of all Labor Boards & Addresses – Online services are available. HOW TO OBTAIN BY MAIL: You can mail the application with a self-addressed stamped envelope. Typically takes 4 weeks to return. HOW TO OBTAIN BY ONLINE: Go to the website follow instructions. www.dir.ca.gov/dlse/entertainment-work-permit.htm Addresses/hours are in your SKM packet. They are only valid for (6) six months. Mark your calendar one month before it expires to remind yourself for the next one. Make sure SKM gets a faxed or mailed copy of every new work permit. Otherwise, your child will be considered inactive. We must have a current copy in office at all times. Take a photo of your work permit as a photo so you always have a copy! Effective 3/2/20 No Same Day Walk In’s unless with production letter requesting.

ENTERTAINMENT INDUSTRY SEXUAL HARASSMENT PREVENTION TRAINING REQUIREMENTS: Applicants for entertainment work permits between 14-17 years of age (age-eligible minors) must complete training (free & takes about 1 hour) in sexual harassment prevention, retaliation, and reporting resources using the online training course made available on the internet website of the Department of Fair Employment and Housing (DFEH) prior to obtaining an entertainment work permit.

PHOTO SAMPLES will need at least 1-2# 4x6 color photos for our files EVERY 6 months. IF you do it yourself just…send by

regular mail to the office or email using a .jpg format only! Remember do not spend a lot of money they are going to change in a few months! Make sure the photo is a similar pose if you send more than one if you can not decide. Have the child against a white wall background. Bright primary color clothing is suggested (no logos on the shirts, no black, or white, stay away from pony-tails it looks like they are bald). Example: Just like a passport or school photo without the busy background, full face and smiling.

TALENT LISTING MANAGEMENT AGREEMENT

I, hereby retain Studio Kids Management to act as manager from the date of this Agreement, and I agree to perform as and when requested by you. I agree to devote my full time and effort, under the direction of Studio Kids Management toward the development of my “Minor-Client/Artist”.

PER LABOR CODE 1701g, IT IS CLASSIFIED AS A TALENT LISTING SERVICE. THIS IS NOT A TALENT AGENCY CONTRACT. ONLY A TALENT AGENT LICENSED PURSUANT TO SECTION 1700.5 OF THE LABOR CODE MAY ENGAGE IN THE OCCUPATION OF PROCURING, OFFERING, PROMISING OR ATTEMPTING TO PROCURE EMPLOYMENT OR ENGAGEMENTS FOR AN ARTIST. STUDIO KIDS MANAGEMENT IS PROHIBITED BY LAW FROM OFFERING OR ATTEMPTING TO OBTAIN AUDITIONS OR EMPLOYMENT FOR YOU. FOR MORE INFORMATION, CONSULT CHAPTER 4.5 (COMMENCING WITH SECTION 1701) OF PART 6 OF DIVISION 2 OF THE LABOR CODE. A DISPUTE ARISING OUT OF THE PERFORMANCE OF THE CONTRACT BY THE TALENT SERVICE THAT IS NOT RESOLVED TO THE SATISFACTION OF THE ARTIST SHOULD BE REFERRED TO LOCAL CONSUMER AFFAIRS DEPARTMENT OR LOCAL LAW ENFORCEMENT, AS APPROPRIATE.

I agree to pay Studio Kids Management, as when monies are received by the “Minor-Client/Artist” and/or “Parent/Guardian”, and during and throughout the term this Agreement, to receive a management commission sum equal to twenty percent (20%) of any and all gross monies, which are related to Studio Kids Management. Let it be said that it may be necessary for Studio Kids Management to collect and receive sums in your child’s name. Whereas monies shall be deposited, any and all checks payable to the minor for Studio Kids Management’s service, and retain there from all sums owing to Studio Kids, and discharge any and all checks to the “Minor-Client/Artist”.

I agree to pay Studio Kids Management within thirty (30) days of receiving payment from the payroll/production company. I will send a copy of each check stub to process and all checks will be made to Studio Kids Management. There will be a one-time processing fee upon the first job of thirty U.S. dollars ($30.00).

Let it be known that Studio Kids Management does not accept any other funds from Production or outside services regarding compensation. Commission payments only comes from the client and/or parent/guardian.

I agree that should the said “Minor-Client/Artist”, at the discretion of the production company, become upgraded to a principal talent, it is the said, “Parent/Guardian’s”, responsibility to obtain a Talent Agent to help further any negotiations and/or such future contracts. In the event the client may need Studio Kids Management to assign a licensed SAG franchised talent agency to the job for any and all possible negotiations. If a SAG Agency is required for a job it would be your responsibility to pay their legal standard commission and all said payments to be made direct to the agency. In addition, let it be said that should such situation occur Studio Kids Management will continue to receive a management sum equal to ten percent (10%) on all gross monies. All commissions are due under this agreement for work up to the time of discontinuance, including all subsequently earned residuals, which may accrue after the discontinuance of this agreement, remain due and payable to Studio Kids Management.

In the event “Minor-Client/Artist” cancels or does not show up for a job, and we cannot find a replacement, the lost commission will be billed to the “Minor-Client/Artist”. I will verbalize with Studio Kids to cancel my child.

I agree to send a mandatory current 4x6 color photo of the “Minor-Client/Artist” in front of a white background every six (6) months.

It is my responsibility to obtain an Entertainment Work Permit every six (6) months through the State of California Division of Labor Standard Enforcement office in order for the “Minor-Client/Artist”, and a Xerox copy is needed in the office of Studio Kids Management to consider my “Minor-Client/Artist” active.

It is my responsibility to obtain a Minor Coogan Blocked Trust Account per Labor Code 1308.9 within seven (7) business days after a minor’s employment contract is signed if they become a Principal actor, and to provide the minor’s employer with a copy of a trustee’s statement (evidencing proof of the account) within ten (10) business days after the start of employment. I understand background minors do not need to obtain a Coogan Trust Account as of 1/1/2014.

It is understood that Studio Kids Management is working in the entertainment industry covering, but not limited to, movies, television, commercials, print-work and modeling.

I accept that since the nature and extent of the success or failure of my “Minor-Client/Artist” career cannot be predetermined the failure or success is not Studio Kids Management’s responsibility. This agreement is a formal binding agreement, can be dissolved at any time by either party, if either party so deems.

How to Obtain Photo ID’s for your minors

ALL MINORS are now required to have I9’s completed to be considered for submission, interview, fitting, or work. This procedure is mandatory and needs to be completed by all Studio Kids clients.

ALL minor’s must carry a PHOTO ID it’s a New Law as of January 22, 2017 from The Department of Homeland Security. This includes infants (15 days old), toddlers, minors, teenagers. Everyone must have a Photo Id (List A & B), in addition to holding a Birth Certificate or Social Security Card (option List C). Attached is the List of Acceptable Items for EEV-I9’s. Here are some OPTIONS to obtain Photo Id’s ASAP:

1. California Department of Motor Vehicles – California ID card - $29.00 any age. Requirements: Using BC/SS & 2# items proof of parent’s residency (originals) Requirements: Infants (able to hold head up for photo) – parent signs for minor Requirements: Toddler & up – minor signs name or can write ‘X’ or ‘O’ Make appointment and then takes about 2 weeks to return.

Making sure that all employees have the correct documents for employment. By doing this it will verify & identify the minor's US documents. This process will be valid until the minor's 18th birthday. Some companies will not hire anybody from any outside company to work for them. It is that important to have this process done prior to working with them. They will not consider to hire ANY kids thru SKM anymore unless you have gone down to have the I9 processed. Once you have completed the process (going to Postal Stop in La Mirada) they will notify SKM. This version requires minors (individuals 17 or under) to meet the same requirements as adults in regards to the documents they can present for form I-9. Per the Department of Homeland Security, a list C document alone is no longer valid. Adults and minors alike are required to either present one original unexpired List A document, or, original unexpired List B and List C document combination. If presenting a List B document to an E-Verify employer (like Central Casting), the employer can only see List B documents that include a photo. Per Federal Law, companies like Central Casting are not permitted to register anyone, minor or adult, who does not have the required documentation.

Once you have worked on a project you will receive a Studio Kids Invoice/Statement (by email) it is just a reminder that you owe SKM for commission due from a recent project you worked on. There are no amounts on this Statement (you can fill in that part) it will have the Date Worked & Name of the Project & SKM Invoice # as reference. You can print and hold until the payment arrives as a reminder to you! IF PAYROLL CHECKS WERE SENT TO STUDIO KIDS: Effective immediately, all checks coming from Entertainment Partners Payroll/Central will be directly sent to Studio Kids for processing. It can take up to 2 weeks to receive the payroll check from the payroll companies to SKM. IF any payroll checks are sent to SKM we will cash and process our agreed commission and/or fee’s and send the remainder monies to the client within one week of receiving the check along with any original documentation. Once we have processed the check you will receive a check from CHASE BANK and SKM will send the original documentation in a ‘separate’ envelope. It can take up to 5-10 days from the date the check was processed by Chase Bank to get to you. This process is approximate 30-days from the work date. Once you receive the Chase Bank check you do not need to worry about paying SKM, since we have taken our commission already. Chase Bank checks expire after 90 days…be sure to cash as soon as you receive it. This is an easier process! IF THE PAYROLL CHECKS WERE SENT TO YOUR HOME: EFFECTIVE IMMEDIATELY – We would like ALL CHECKS to be sent to SKM first BUT…IF the payroll check was sent to your home address it is your responsibility to pay agreed commission to Studio Kids on all checks including interview & wardrobe fitting’s. In this case we do use the 'Honor System' and know that you will send your payments to us in a timely fashion (about 2 weeks from receiving your check). You can wait up to 2-4 weeks to receive your original check from the payroll company. Once you receive the check you will need to make a copy of the 'check-stub' for SKM and you will send it along with your payment. We do not need the copy of the check itself - just the portion with all the details called the ‘check-stub’. CHECKS/MONEY ORDER: You may send a check or money order and mail to our office with a copy of the check-stub. If you have multiple invoices, you may combine the Gross amounts and send ONE payment – however, be sure to send each check-stub for reference. PAY USING DEBIT/CREDIT CARDS: You can also pay on our website www.studiokidsmanagment.com go to the tab – "Make a Payment" and using your Debit/Credit Card with PayPal services. Here is the link www.PayPal.com ONE-TIME PROCESSING FEES: You will be charged a One-Time Processing Fee at your 1st project of $30.00, under the terms of the Studio Kids Agreement. Payment is due AFTER you have been paid. NOTE: You DO NOT need to wait for the SKM Statement to be mailed to you to pay Studio Kids - they are simple just reminders! IF you did not receive a copy you may request a replacement. LOST/NEVER RECEIVED CHECKS: IF you have not received a check within (1) one month of working, please notify the office Ext 3# or email at [email protected] so we can locate and reissue the check. EXPIRED CHASE BANK CHECKS: Chase Bank checks expire after 90 days be sure to cash them as soon as you receive them. IF you received a Chase Bank check and you did not cash the check and it’s expired, there is a $10.00 Research Fee on each check needing for reissue. Please email or mail a copy of the expired check to our office for reissue. Research Fee will be applied to the returned reissued check. COOGAN TRUST ACCOUNTS: Background minors are not required by Law to deposit into the Coogan Trust. Upgrades/Principal actors are made mandatory by Law to deposit into Coogan Trust Account. PARENTS/GUARDIANS WHO WORK: Studio Kids will take a 10% commission from the gross amount on all parents and/or guardians if they should work thru Studio Kids. IF you have received an SKM Invoice and it’s already been paid or SKM has already deducted from the check, please ignore this email and thank you for your payment!
