TECHNICAL SPECIFICATION FOR ASBESTOS ABATEMENTBLACK MOUNTAIN ELEMENTARY SCHOOLROOFS - BUILDINGS 100, 200, 300, 400 & 500
Cave Creek Unified School District (CCUSD) has contracted with Dominion Environmental Consultants, Inc.(Dominion), to provide a general Technical Specification (Specifications) for the Asbestos AbatementOversight, Ambient Air Monitoring, Clearance Air Monitoring and Close-Out Documentation for the asbestosabatement project to take place on the roofs of Buildings 100, 200, 300, 400 and 500 at Black MountainElementary School in Scottsdale, AZ.
The following Specifications are written with the intention that a Dominion representative will continuallymonitor the entire abatement process for the asbestos-containing materials (ACM). Specificationmodifications are subject to preliminary approval by a representative of Dominion and final approval byCCUSD. Only under the direction and approval of CCUSD, Dominion will have the ability to stop jobactivities that, in their judgement, are not being performed satisfactorily, or in accordance with applicableasbestos regulations or the requirements of these Specifications. Such a work stoppage shall not result in anycosts to CCUSD or Dominion, nor will it relieve the Asbestos Abatement Contractor (Contractor) from anyliability for the violation of these regulations, contractual agreements or the requirements of theseSpecifications.
The designated Abatement Contractor (Contractor) shall be responsible for his own work and properprotection of his workers along with the proper removal and proper disposal of any abated ACM from theCCUSD, Black Mountain Elementary School - Buildings 100, 200, 300, 400 and 500 which are scheduledfor future renovation. The currently identified ACM are outlined in the CCUSD Asbestos Survey dated May16, 17, 22, 2018 (see Attachment A - Black Mountain Elementary School Roof Asbestos Survey). A summaryof the ACM (>1% Asbestos Content) and Unclassified Asbestos Materials (<1% Asbestos Content) identifiedto date are as follows:

CCUSD, through Dominion, shall have the authority to request that Contractor personnel (i.e., supervisors,foremen, or workers) be removed from the site and replaced if, in their judgement, those individuals’activities are not satisfactory, safe, or in accordance with applicable regulatory or other asbestos relatedguidelines and the requirements of these Specifications.
The work schedule will be regular work days Monday-Friday with no overtime, unless otherwise mandatedand approved in writing by CCUSD.
At a minimum, the selected Contractor shall utilize proper engineering controls to protect the properlycertified and experienced workers along with the occupants in and around the surrounding areas fromexposure to any on-site contaminates as it relates to the roofing abatement activities, as outlined below:
Regulatory Compliance:
The abatement contractor shall comply with all applicable Federal, State and Local Regulatory Guidelinesincluding but not limited to the following:
• OSHA 29 CFR 1926.1101 - Construction Industry • OSHA 29 CFR 1910.20 - Access to Employee Exposure and Medical Records• OSHA 29 CFR 1910.1200 - Hazard Communication • OSHA 29 CFR 1910.134 - Respirator Protection • EPA 40 CFR 61 - NESHAP Renovation and Demolition Standard• Maricopa County Rule 370 - Hazardous Air Pollutant Program• U.S. Department of Transportation 49 CFR 171 and 172 - Hazardous Substances
As stated the Contractor shall assume full responsibility and liability for compliance with all applicableFederal, State, and Local Regulations pertaining to work practices, hauling, disposal, and protection ofworkers, visitors to the site, and persons occupying areas adjacent to the site.
Contractor Qualifications:
• The Contractor shall be a pre-approved “State of Arizona” Abatement Contractor and shall providea current State of Arizona Contract number.
• The Contractor shall have a current valid Contractors License applicable to asbestos abatementactivities.
• The Contractor shall have current A+ rated or better, Occurrence-Based Liability Insurance withcoverage limits of $1,000,000 per Occurrence and $1,000,000 in Aggregate or as otherwise requiredby CCUSD.
• The Contractor’s asbestos workers and supervisors shall all possess either a current AHERA workercertification, or a supervisor’s certification from an approved EPA training facility. The workers orsupervisors shall also have a valid color photo identification from an EPA approved training facilityor a government issued photo identification card. All shall be kept on-site at all times.
• The Contractor’s workers and supervisors shall have current pulmonary medical examinations andfit tests for all employees required to use negative pressure respirators as required by OSHA. All shallbe kept on-site at all times.

Contractors Pre-Job Submittals:
The selected Contractor shall provide to CCUSD’s representative (Dominion) proof of the previouslyreferenced qualifications within twenty-four (24) hours prior to the commencement of the abatementactivities. In addition, the contractor will also be required to provide the following information prior to thecommencement of the abatement activities:
• Any historical data for the work being performed. In the event no historical data is available, anInitial Exposure Assessment will be provided and shall be followed by a Negative ExposureAssessment, as air sample results become available.
• A copy of the NESHAP notification (if necessary) shall be provided to the project oversightconsultant and CCUSD for review.
• The Contractor shall provide to CCUSD an abatement schedule to include preparation of regulatedwork areas, abatement, final cleaning and completion of the abatement activities.
• The Contractor shall submit Safety Data Sheets (SDS) for any materials proposed to be brought on-site, for approval by CCUSD and Dominion.
• The Contractor shall submit proof of insurance acceptable to the CCUSD, for approval.
Minimum Contractor Responsibilities & Requirements:
• Conditions affecting the work, including but not limited to, the physical conditions of the site,availability of water and utilities, accessibility, storage and handling of equipment and materials, orother factors that may affect the performance of abatement activities.
• The Contractor shall ensure that all asbestos work is performed within regulated work areas and issupervised by a competent person.
• The Contractor shall use hand methods, unless the Contractor deems mechanical removal will savetime and money. It is important to note that the time schedule is of the utmost importance. If theContractor elects to introduce mechanical means then the Contractor will need to file a NESHAPNotification and pay all applicable fees with the County.
• The verification of the location(s) and quantities of ACMs and Unclassified Asbestos Materials, aswell as the location and quantities of other surface and subsurface materials or obstacles that may beencountered are the full responsibility of the Contractor to verify prior to project start.
• The Contractor shall also consider any specific requirements or work procedures developed byCCUSD or Dominion and include provisions as necessary for compliance. Any failure by theContractor to acquaint himself with available information does not relieve the Contractor from theresponsibility of properly estimating the cost and degree of difficulty of successfully performing theabatement work. Information regarding material type, location, or quantity that is made available byCCUSD or its Consultant(s) is informational only. Neither CCUSD nor its Consultant(s) isresponsible for any conclusions or interpretations made by the Contractor on the basis of thisinformation. Any concerns, questions or clarifications should be addressed by the Contractor priorto project start.

• Once abatement activities have commenced, all workers or supervisors in the regulated work area shall at a minimum have a half-face HEPA filtered respirator. It is the Contractors full and totalresponsibility to ensure that their employees have the proper Personal Protective Equipment (PPE).
• The Contractor shall furnish all labor, materials, services and equipment necessary for the abatementof all ACM in the designated areas.
• All vacuums utilized by the Contractor shall be HEPA filtered.
• The Contractor shall provide a Manometer for all Negative Pressure Enclosures (NPE) in order tomaintain a pressure of -0.02 column inches of water pressure differential (If Needed).
• The Contractor shall furnish the site with power and water that is needed to support the abatementactivities, as well as supply the Consultant with any power that is needed to run ambient or clearanceair samples.
• The Contractor shall provide hygiene facilities (i.e.. J Johns, wash stations, etc.) in accordance withthe regulatory guidelines.
• If removing Regulated Asbestos-Containing Materials (RACM) as defined by the EPA, amendedwater shall always be used to adequately wet the RACM.
• When removing Category I and II ACMs, wet removal methods shall be utilized.
• The Contractor shall post all notices, notifications, permits and proper proof of training required bythe applicable Federal, State, and Local Regulations, and maintain one (1) copy of the applicableasbestos survey on-site at all times.
• The Contractor shall regularly inspect regulated work areas during abatement to ensure that allapplicable regulations and work procedures are being followed at all times. The Contractor shallinspect the regulated work areas prior to the completion of abatement to ensure all visible debris andcontaminated materials have been cleaned and/or removed.
• The Contractor will be responsible for site security of the work areas 24/7 until completion and finalclearance has been given.
• All routine communications, change requests to scope/schedule and procedures shall be submittedto the Consultant for initial consideration and final approval by CCUSD.
• The Contractor shall continually coordinate with the General Contractor on a daily basis for dryingin any abated areas. The Contractor shall not abate any more than what the General Contractor candry in on a daily basis. Any ACM removed without having written arrangements with GeneralContractor, is at the Contractor’s own risk and the Contractor will therefore be held financiallyresponsible for any related water damages.
• Work and services not part of the established scope shall not be initiated without prior scope and costapproval by CCUSD. Work and services undertaken by the Contractor without prior approval ofCCUSD shall be done at the risk and expense of the Contractor. Any labor or materials necessary toreverse unauthorized work shall be provided at no charge by the Contractor and without change tothe schedule.

Minimum Work Area Requirements:
• For all Class II work performed outside of the building, the Contractor shall:
Regulate the work area by constructing appropriate barriers in order to limit accessby unauthorized personnel and install appropriate signage.
Utilize impermeable drop cloths or coverings in all areas that have the potential ofbecoming contaminated due to abatement activities (i.e. soil, sidewalks, and othersurfaces), to protect those areas from contamination.
Promptly clean-up and containerize all generated asbestos-containing waste in leak-tight containers and carefully lower the containers to the ground, not dropping,throwing, sliding, or otherwise damaging or disturbing the material or the wrappingthat contains it. The Contractor shall not use chutes to transport ACM from anelevated surface to the ground.
• Provide an area of sufficient size as to accommodate cleaning of equipmentand removal of personal protective equipment without spreadingcontamination beyond the work area.
• At the end of the day’s work or before removing respirator protection, at aminimum each worker shall perform the following decontaminationprocedures:
HEPA vacuum hands, hair, face, and respirator, giving special attention tothe respirator seal area on the face.
Remove the respirator, and HEPA vacuum face at the respirator seal and allsurfaces of the respirator. HEPA vacuum any parts of hair or head coveredby the respirator’s straps.
Wet clean the respirator in accordance with manufacturer’s instructions,taking care to protect battery packs and moving parts, if any.
Thoroughly wash face and hands with soap and water.
Lock-out and tag-out all power supplies to the work area and provide power froma source outside the work area that is ground fault protected.
Note: Prior to the commencement of any abatement activities, Dominion’s field supervisor willconduct a complimentary work area inspection of the regulated work area.
Ambient Air Monitoring:
• Pursuant to the requirements of OSHA Regulation 29 CFR 1926.1101, the Contractor shall monitorairborne fiber counts and worker exposure levels inside the regulated area throughout the asbestosabatement and cleaning operations.
• Copies of all air monitoring records and sample results collected by the Contractor shall be providedto Dominion within 24 hours of collection, for review.

• The method of air monitoring and analysis by the Contractor during abatement operations shall beconducted in accordance with OSHA Regulations 29 CFR 1926.1101, using the NIOSH 7400 methodfor Phase Contrast Microscopy (PCM).
• The laboratory used to analyze asbestos samples shall be a participant in the AIHA/NIOSH PATprogram for asbestos fiber counting and accredited by the National Voluntary LaboratoryAccreditation Program (NVLAP) for asbestos analysis by PCM.
• Personal air monitoring of Contractor Personnel is for purposes of the Contractor. It shall beperformed by the Contractor at no additional cost to CCUSD.
• Throughout the asbestos abatement and cleaning operations, area air monitoring shall be conductedby the CCUSD representative (Dominion) both inside and outside the work areas. The airborne fiberlevel outside the work area shall not exceed 0.01 fibers per cubic centimeter of air (0.01 f/cc). Theairborne fiber level inside the work area shall not exceed 0.10 f/cc over an eight hour TWA or 1.0f/cc over a 30 minute period. If any of these levels are exceeded, removal activities will stop until theair levels have been reduced and the abatement procedures have been modified. Time lost due toexcessive air concentrations is the responsibility of the Contractor. Time delays will not be grantedfor reducing fibers levels in the work areas.
• The laboratory used by Dominion to analyze asbestos samples shall be a participant in theAIHA/NIOSH PAT program for asbestos fiber counting and accredited by the National VoluntaryLaboratory Accreditation Program (NVLAP) for asbestos analysis by PCM and PLM.
• All air samples collected by the Contractor or Dominion shall be analyzed within 24 hours ofcollection.
• “Airborne Fibers” in these provisions, include all fibers regardless of composition as counted byPCM, unless additional analysis by Transmission Electron Microscopy (TEM) demonstrates to thesatisfaction of Dominion, that non-asbestos fibers are being counted.
Waste Disposal:
• Asbestos-containing and/or contaminated materials shall be provided with proper disposal at alandfill operated in accordance with the requirements of NESHAP Asbestos Regulations (40 CFRPart 61, Subpart M), and approved by EPA for asbestos wastes.
• All wastes are to be hauled by a waste hauler with all required licenses from all State and localauthorities with jurisdiction.
• All containerized waste shall be carefully loaded into fully enclosed dumpsters, trucks, or otherappropriate vehicles for transport. Care must be exercised before and during transport to ensure thatno unauthorized person(s) have access to the material. The interior of the transport vehicle shouldbe protected with two (2) layers of polyethylene plastic sheeting, each layer 6-mil thick.
• All asbestos waste shall be at a minium doubled bagged or “buritto wrapped” with two (2) layers of6-mil thick re-enforced polyethylene plastic sheeting, adequately wet, proper warning labels andgenerator labels.
• Containerized waste materials must NOT be stored outside of the work area. Take containers fromthe work area directly to a sealed truck or dumpster.

• Do NOT transport bagged materials on open trucks. Transport only properly sealed containersmarked with the OSHA and DOT labels in closed-bed vehicles protected with sheet plastic.
• At the disposal site, carefully unload containerized asbestos waste from the transport vehicle.Wrapped asbestos waste may require special unloading procedures and equipment. The Contractorshall ensure that all special procedures are followed, and special equipment is provided as needed.
• Retain landfill receipts and waste manifests as proof of proper disposal. Copies of all of thesedocuments shall be provided to the Owner’s Representative upon completion of the work.
• The Contractor is responsible for obtaining all required signatures on landfill disposal documents andwaste manifests. Arrangements for obtaining the Owner’s signature on these types of documents mustbe made by the Contractor sufficiently prior to scheduled transportation of the asbestos waste toensure that no delay in transporting or disposal of the waste will occur.
Clearance Sampling:
• Because the abatement is being performed outside, a visual inspection of the abated areas will beperformed by CCUSD’s representative (Dominion). No final clearance air sampling will be required.
Contractor’s Post Job Submittals:
A complete and detailed copy of all of the Contractor’s post project documentation shall be submitted toCCUSD’s representative before the project will be considered for completion and release of the Contractor’sfinal payment. This will include but not be limited to the following:
• Copy of the Certificate of Insurance
• NESHAP Notification (If Required)
• Daily Field Reports
• Daily Sign-in Sheet
• Daily Regulated Work Area Log
• Air Sample Laboratory Reports
• Laboratory Certifications
• Historical Data/Negative Exposure Assessments
• Workers and Supervisors AHERA Certification, Medicals and Respirator Fit Test

LIMITATIONS
The conclusions and results contained herein are based solely on the information presented in thisreport. The limited sampling, testing, and observations described in the report represent conditionsonly at the specified times and locations. Additional materials or information which were hidden,undiscovered, inaccessible, or not a part of the findings presented herein, would result in themodification of the conclusions and recommendations made herein. Any response action guidelinesare based solely on the findings contained herein. The response action guidelines are presented asa courtesy and are not to be considered a complete or detailed set of specifications. Dominionperformed this survey as directed by the client and in compliance with applicable regulations.
Please note, with regard to laboratory analysis of suspect bulk asbestos-containing materials: TheEnvironmental Protection Agency (EPA) Method 600/R-93-116, Polarized Light Microscopy (PLM)is used for the testing of bulk building materials for asbestos by performing a visual estimation. Thismethod is the most widely used method for estimating asbestos in bulk building materials and workswell for most sample types. However, it might require a more detailed method such as pointcounting for accurate estimation of asbestos in samples with low asbestos concentration. Thismethod is also not applicable for samples containing large amounts of fine fibers below theresolution of the PLM (<0.3 microns). Typically, the asbestos fibers present in non-friableorganically bound (NOB) materials are often less than 0.1 microns in diameter and therefore maycause the PLM method to yield low estimates or even false negative results.
Dominion suggests that Transmission Electron Microscopy (TEM) Chatfield Semi-Quantitative EPAMethod 600/R-93-116 be used to confirm all negative results of NOB materials. This method isparticularly useful to reduce the possibility of false negatives in NOBs and to provide additionalquantitative data for samples which contain low levels of asbestos. The TEM method is availableat additional cost and is done only at the client’s request.
Dominion’s scope of service did not include the inspection for, or identification of, fungi, PCB’s,lead paint or any other hazardous or controlled substance not specifically identified herein.
Dominion is not responsible for the accuracy of information or any services provided by others, orfor conditions or consequences arising from relevant facts that were withheld, concealed,undiscovered, or not fully disclosed. EXECUTIVE SUMMARY
INTRODUCTION
This report presents the results of a limited asbestos inspection of the roofs of Buildings 100, 200,300, 400 and 500, on the Black Mountain Elementary School campus located at 33606 North 60th
Street, Scottsdale, Arizona. Pursuant to the information provided by the Cave Creek Unified SchoolDistrict #93 (CCUSD), the roofs of Buildings 100, 200, 300, 400 and 500 are scheduled forrenovation. Dominion was instructed to perform a limited asbestos inspection of the roofs ofBuildings 100, 200, 300, 400 and 500 scheduled for replacement in accordance with AHERA,OSHA, and NESHAP guidelines. Dominion was authorized to perform the asbestos inspection byverbal authorization from Mr. Mike Santina on May 11, 2018.
ASBESTOS SAMPLING
The limited asbestos inspection of Buildings 100, 200, 300, 400 and 500, was conducted on May16, 17 and May 22, 2018, by Mr. Basilio Marcos, an EPA-accredited AHERA Asbestos BuildingInspector.
A total of one hundred eight (108) bulk samples were collected from forty-three (43) homogeneousbuilding materials throughout Buildings 100, 200, 300, 400 and 500. The suspect bulk samples weresubmitted to EMLab P&K (EMLab) for analysis using Polarized Light Microscopy (PLM) methods.EMLab used the methods prescribed in Method 40 CFR, Part 763, in the Code of Federal Registerin analyzing bulk samples. EMLab is certified by the National Institute of Standards and Technologyas a National Voluntary Laboratory Accreditation Program (NVLAP), Number 500031-0. Thesuspect asbestos bulk samples were collected and submitted to the laboratory using establishedchain-of-custody procedures. Bulk samples were collected utilizing safety and health practices asrequired by the Occupational Safety and Health Administration (OSHA).
Identified in the following list, separated by building, are the forty-three (43) suspect homogeneousmaterials that were sampled during Dominion’s asbestos inspections.

RECOMMENDATIONS
Based on the previously referenced sampling data, Dominion recommends that the above listedACM identified by laboratory analysis to contain greater than one percent (>1%) asbestos-content,be properly abated by a qualified asbestos abatement contractor. The contractor should comply withall governing regulatory agencies guidelines. If any un-sampled suspect building materials arediscovered during the demolition activities, those building materials should be properly sampled bya certified inspector prior to their disturbance.
The EPA does not regulate materials reported to contain less than one percent (<1%) asbestos,however OSHA regulates all materials that contain asbestos regardless of the amount of asbestosin the material. Therefore, based on the prior stated analytical results, Dominion also recommendsthat OSHA unclassified work practices be followed for any building materials containing less thanone percent (<1%) asbestos-content that will be affected during the roof replacement activities.

C Mr. Mike Santina, with the Cave Creek Unified School District #93 (CCUSD) outlined theareas and materials that are scheduled to be disturbed during the upcoming roof replacementactivities, instructed Dominion to only collect bulk samples of the outlined homogeneousmaterials, and have the samples analyzed by a qualified laboratory for the presence ofasbestos fibers.
C If any un-sampled suspect building materials are discovered during the roof replacementactivities, those materials should be properly sampled by a certified inspector prior to theirdisturbance
