Systems Integration
• Create a web-based no wrong door system of access to services that includes:– On line public access to ESP data– On line chat capabilities– Self-service features – Use of platform independent mobile applications
• Statewide Marketing of the system
Streamlined Access• Establish data sharing agreement with DCH• Develop process for Gateway/ ADRC and CM staff to
track client Medicaid eligibility status through DCH portal
• Develop protocol for Gateway/ ADRC staff to assist clients with completing Medicaid application to include electronic signature
• Information gathered by the Aging Services portal will be passed on to the integrated eligibility system

Wellness Evidence Based Initiatives
• Wellness Initiatives to bring evidence based programming to the Georgia Aging Network– CDSMP expansion over three years
• Trainings and supplies– A Matter of Balance (year one)
• Trainings and supplies– Tai Chi Moving for Better Balance (year two)

Nutrition Initiatives
• Nutrition Initiatives to help with food insecurity and evidence based education.– EBT Machine Partnerships with Farmers’ Markets
• Will help incentivize signing up for SNAP for older adults.• Will benefit local farmers and farmers’ markets• Will benefit local communities and increase access to healthy
foods.– Stanford Nutrition Action Program
• An evidence based nutrition education program to help unify and strengthen nutrition education across the aging network.
Expansion of innovative approaches• Expand the use of consumer direction - CLP:
– Movement to align CCSP consumer direction with non-Medicaid model
– Provide TA to AAAs to implement with traditional services using FA
• Expand person-centered care transitions and consumer directed services:– Provide Statewide training for Gateway/ ADRC, Options
Counselor, APS staff and CCSP and Non Medicaid CM

RCI TA and Training for EBPs• Rosalynn Carter Institute will implement a dementia
capable comprehensive training program for AAA, APS, and Alzheimer’s Association staff– First step will be to conduct a statewide assessment of
training needs– Feedback received from all AAAs last June at Early
Stage AD Work Team indicated that training on managing problem behaviors, competency, financial management issues, and hoarding would be helpful
Caregiver Evidence Based Programs
• Expand the availability of evidence-based and evidence-informed programs and services: –Funding for:
• PTC• Savvy Caregiver
Dementia Capable Network
• Comprehensive training program to increase competencies of Gateway/ ADRC, Case Management staff and APS including dementia-capable assessments and service planning
Telephone Reassurance for Memory Impaired Clients
• Participants at Early Stage AD Work Team noted Telephone Reassurance as way to serve:– People who live alone and are at risk– Those on waiting lists
• Alzheimer’s Association will implement statewide program, receiving referrals from Gateway/ ADRC, Case Managers, APS etc..– Second model implemented by River Valley AAA– Statewide training on both models in Year three of grant
Create a sustainable service delivery system
•All AAAs will receive TA on business plans and private pay service systems
– Templates with tax and other rules specific for:• AAA/ RC• 501(c)3• County or municipality
– Web portal opportunities for private pay
