The user can rest assured that all products by theKARL STORZ group have been designed and constructed not only in compliance with strict internal quali-ty guidelines, but also with international standards. All data relevant for safe use, such as viewing direc-tion, sizes and diameters, or notes regarding sterilization of telescopes, are applied to the instruments,have been formulated according to international standards, and therefore provide reliable information.

As we constantly seek to improve and modify our products, we reserve the right to make changes indesign that vary from catalog descriptions.

Original or Counterfeit

KARL STORZ products are name brand articles renowned around the world and represent the state of theart in important areas of healthcare. A large number of “copy cat” products are currently being offered inmany markets. These products are designed intentionally to resemble KARL STORZ products and usemarketing strategies that at least point out their compatibility with KARL STORZ products. These productsare by no means genuine products, since genuine KARL STORZ products are sold worldwide exclusivelyunder the name of KARL STORZ, which appears on the packaging and the product. In the absence ofsuch labeling, the product is not from KARL STORZ.

KARL STORZ, therefore, is unable to ensure that such products are actually compatible with genuineKARL STORZ products or can be used with them without injury to the patient.

The whole is greater than the sum of its parts – with theIMAGE1 S FULL HD camera platform, KARL STORZhas created a single system for all applications. As amodular camera platform, IMAGE1 S combines varioustechnologies (e.g., rigid, flexible and 3D endoscopy) inone system and can therefore be adapted to individualcustomer needs.The modular design allows individual modules to beselected according to user requirements. Furthermore,the camera system provides new and innovativevisualisization possibilities.

Camera Control UnitsThe IMAGE1 S CONNECT camera control unit is thecore of the IMAGE1 S system.Various link modules can be connected for rigid,flexible and 3D endoscopy. For rigid endoscopy with three-chip FULL HD cameraheads, the user requires the H3-LINK in addition toIMAGE1 S CONNECT. In order to use the camera system for flexible videoendoscopy as well as for rigid endoscopy with one-chip FULL HD camera heads, the X-LINK must beconnected to IMAGE1 S CONNECT.

Visualization Modes

In addition to brilliant standard images with a naturalcolor rendition, the system offers the user new tech-nologies for illumination, contrast enhancement andhue shifting in endoscopic imaging.

Color shifting in the CLARA, CHROMA and SPECTRAvisualization modes provides enhanced tissue differen-tiation. The visualization modes do not require a speciallight source.

Why should a method of mother-baby endoscopy inuse since the 1970s now require a new version?

The reason is that only a few centers make use of theconventional method due to its technical limitations.Furthermore, mother-baby endoscopy is not used asoften as it should considering the potential offered bythis method. Direct visualization and endoscopic treat-ment of the pancreobiliary system is just as desirableas endoscopy in the gastrointestinal tract, which haslargely replaced non-direct radiological techniques.

Mother-baby endoscopy has expanded the spectrumof diagnostic and therapeutic possibilities in ERCP.However, it still does not enjoy widespread use despitean evolutionary phase of three decades. Consideringthe wide range of diagnostic and therapeutic appli-cations this method offers, it is not employed as oftenas would be expected in those clinics where it isavailable.

Why is conventional mother-baby endoscopy soseldom used?Mother-Baby Endoscopy (MBE) is an option for patientswho are not suited for conventional ERCP. The reasonMBE is rarely used is that technical difficulties apparentfrom the beginning have not been resolved satis-factorily. This is mainly due to the excessive length ofthe conventional baby-endoscope. It was not possibleto avoid this problem until now as the baby-endoscopehad to be inserted through the entire working channelof the duodenoscope (“mother-endoscope”) in order toaccess the pancreobiliary system.

The excess length of the baby-endoscope results inawkward handling during cleaning and MBE. Inaddition, its restricted mobility in the pancreobiliarysystem is a major disadvantage. Consequently, suspectlesions are often inaccessible or difficult to accessduring biopsy. During laser or electrohydraulic litho-tripsy of large stones that cannot be removed withother methods, the probe position on the concrement isoften suboptimal. This results in laser or electrohy-draulic procedures that are difficult or cumbersome. Aparticular disadvantage is that the side branches of thebilary system can only be intubated if they do notdeviate too far from the direction of the main bile duct.Furthermore, access is frequently achieved throughcomplex positioning with a guide wire beforehand. Themobility of the baby-endoscope in a duct intubated inthis manner is practically non-existent.

The New FRIMBERGER Mother-Baby SystemThese problems can be minimized or prevented in asurprisingly simple way: by reducing the size of thebaby-endoscope! The new short baby-endoscopeversion only measures 98 cm. Due to the reduced size,the baby-endoscope can no longer be introduced tothe pancreobiliary system via the duodenoscope.Instead it is positioned through a side port (Fig. 1) in themother-endoscope sheath. The side port is mountedon the duodenoscope, approx. 70 cm from the distal tipand is simply an additional opening in the workingchannel. The side port can be covered with a cap (Fig. 5).

When the position of the duodenoscope is adjustedafter reaching the papilla, the side port is locatedoutside the patient, approx 5 – 15 cm extraoral. If notrequired during ERCP, it remains covered and theduodenoscope offers all the options of conventionalERCP. Should a problem arise during diagnostic ortherapeutic ERCP, the cap can be removed with aspecial tool (Fig. 5) and the baby-endoscope can bepassed through the side port. The short size of thebaby-endoscope makes it easy and convenient tohandle and offers more agility in the pancreobiliarysystem than was possible up to now. Furthermore,previously inaccessible side branches can now beintubated.

Experiences with the mother-baby systemExperience with the new mother-baby system, primarilyin the pancreobiliary system, has been extremely posi-tive. The baby-endoscope could be introduced into theductal system of all patients where passage was nothindered by a high-grade stricture. Handling of the newshort baby-endoscope was much easier than the extra-long “old” baby-endoscope. Its outstanding agility inthe pancreobiliary system offered the user a greaterdegree of freedom in the pancreobiliary system thanthe “old” extralong scope, making examination mucheasier. The unproblematic handling and agility hasresulted in the new MBE becoming a standard proce-dure in day-to-day clinical routine so that, for example,diagnostic ambiguity in ERCP can be resolved muchfaster than ever before. This procedure should benefitquite a few patients.

Which instruments can be introduced through theshort baby-endoscope?The working channel of the new baby-endoscope is1.5 mm larger than the previous baby-endoscope, allo-wing the use of larger-caliber instruments. The biopsyforceps (Fig. 3) is used for tissue diagnostics. Electro-hydraulic or laser probes are used for the fragmentationof large stones (Fig. 4).

Proctoscopy and Rectoscopy

Approximately 50% of adults suffer from an anorectaldisease and related symptoms at least once in theirlives. The disease can be identified by reviewing thepatient’s medical history thoroughly, thereby allowing atentative diagnosis to be made.

An inspection and rectal digital exploration are theobligatory preliminary medical examinations, which arethen followed by instrumental diagnostic procedures.

The correct indication for a method of treatment andoperation can only be given once a careful and preciseproctological investigation has been carried out.

A wide range of improvements in the techniques forinpatient and outpatient proctological operations in thelast 2 decades, not to mention the innovations inaccompanying diagnostic examinations, have led tomuch greater acceptance and satisfaction amongst thepatients concerned.

The most significant indications for a proctologicalexamination are:● Anal bleeding● Skin changes in the anal region● Pruritus● Anal pain

Many diagnoses can be reached upon inspection of theanal region. In this case, the detection of fissures, scarchanges or fistula openings indicates a requirement forfurther examination using an ARNOLD fistula hook.After a digital examination, rectoscopy and procto-scopy are performed using rigid tubes. Depending onthe findings and symptoms, various tube sizes can beused for proctoscopy and rectoscopy.

A significant aspect of the day-to-day work in proc-tology includes diagnosing and treating hemorrhoidalcomplaints. Sclerotherapy and rubber band ligation canbe implemented successfully in the treatment ofhemorrhoidal diseases which are still in the earlystages. A variety of ligation devices can be used for thispurpose. Many of the instruments used in ligationtreatment require the aid of an assistant, although thisis generally not the case when suction ligationinstruments with an integrated suction pump is applied.After use, the suction ligation instrument can bedismantled easily and the hygienic preparationperformed efficiently.

The hand instruments presented here for the diagnosisand treatment of proctological diseases are highlyrobust and easy to take care of. This guarantees a longservice life. The metal and plastic parts can beprepared hygienically in the thermal disinfection deviceand sterilized in the autoclave.

For optimal illumination in every situation, the XENON100 SCB is the logical component in your KARL STORZgastroenterology system.

With its 100 Watt Xenon lamp, this light source isperfectly coordinated to the endoscopes of the SILVERSCOPE® range and IMAGE1 S. The lamp’s colorspectrum correlates with the requirements of the CCDchip and the GI CCU in a unique way.

The integrated, efficient insufflation pump ensures thenecessary unfolding of the stomach or colon in order tocreate optimal conditions for diagnostics.

The digital control and setting options are presentedclearly on the front of the Xenon 100 SCB. Here, pumpcapacity and light intensity can be adjusted to currentrequirements at the touch of a button. Easily under-standable displays allow safe control of the settings andthereby simplify daily operation. After approx. 500 hours,the lamp service life display indicates the need tochange the lamp. The lamp can of course be changedwithout the help of a technician.

The CO2mbi LED is a unique cold light fountain thatfeatures the latest LED technology and integrated CO2

or air insufflation.To provide the best possible performance and cost-effectiveness, three different insufflation modes areavailable.The first mode “air” offers the user a pure room airinsufflation mode. A CO2 supply is not required in thiscase. The second mode “CO2/air” offers the advantage ofthe economical use of CO2 and increased safety levelsfor personnel present in the examination room. If thedelay time set in the service menu is exceeded afteractivation of the air/water valve, the light sourceautomatically switches the insufflation mode to roomair. As soon as the user activates the air/water valve,the system reverts to the CO2 mode. This is madepossible by the sensor technology integrated in theCO2mbi LED system.If a time delay of 10 seconds is set, for example, and abiopsy is required but the procedure, including setupand insertion of accessories as well as the biopsy itselflasts 60 seconds, room air instead of CO2 will besupplied for 50 seconds if the examiner does notrequire insufflation during this period. This system,therefore, actively saves CO2. When reverting to CO2,there is a slight increase in the amount of room airpresent in the air channel of the supply tube in thepatient. A further advantage of this mode is that itprevents an accumulation of gas in the examinationroom. In everyday clinical and outpatient practice,delays in the examination procedure often occur due tooverloading and tight schedules. Not infrequently, asignificant amount of time may elapse between testingthe endoscope for proper functioning and the start ofthe examination. In addition to the economical use of

CO2, this mode and the possibility to switch over toroom air also prevents gas accumulation in theexamination room, which can cause fatigue and a lackof concentration.The third mode “CO2 100%” is pure CO2 insufflation.When this mode is selected, the unit will not switchover to air insufflation. This mode is particularlysuitable for invasive interventions such as, for example,endoscopic mucosa resection (EMR), endoscopicsubmucosal dissection (ESD) or peroral endoscopicmyotomy (POEM). A switchover from the CO2 to the airmode is not possible during a procedure or vice versa.The CO2mbi LED is a stellar example for the innovativeproducts offered by KARL STORZ – naturally Made inGermany.

ZeroWireWireless FULL HD Video Transmission System

The ZeroWire FULL HD video transmission systemfrom KARL STORZ enables the wireless integration ofsecondary monitors in the existing FULL HD imagechain.

Developed specifically for applications in endoscopy,ZeroWire replaces the DVI cable between the CameraControl Unit (CCU) and the secondary monitor.

The unit therefore simplifies the installation of additionalmonitors.

The ZeroWire system consists of a transmitter that isattached to the primary monitor and a receiver that ismounted on the secondary monitor.

This enables the wireless transmission of FULL HDvideo signals from IMAGE1 S CONNECT to a secondarymonitor without any need for costly and sensitive lightfibers or DVI cables.

ZeroWire delivers a 1080p FULL HD image. The razor-sharp image quality is virtually indistinguishable from aconventional DVI connection.

Data Management and DocumentationMULTISWITCH®

A compact solution enables the easy and efficientdistribution and individual routing of several high-resolution video signals, even without an integratedOR. MULTISWITCH® offers the possibility to manageand distribute up to 4 DVI sources (camera systems,

PC etc.) to four independent outputs (monitors,recording devices). The MULTISWITCH® unit can beplaced directly on the videocart and can be upgradedwith all FULL HD endoscopy towers from KARL STORZ.

1. Mode A (Switcher)

Enables the simple selection of a source and simultaneous distribution to all four outputs. This mode is usedwhen, during live transmission, an image is switched from various sources and has to be distributed to allconnected monitors.

2. Mode B (Router)

Distribution of any four sources to any four outputs (monitors, documentation systems)

This mode is always useful if a flexible distribution of individual DVI sources to the outputs is necessary.

Units for Gastroenterology

The units manufactured by KARL STORZ combine long-lasting precision mechanics with state-of-the-art micro-electronic programmable controls. At KARL STORZ, thegreatest emphasis is placed on user and patient safety.The quality assurance system of KARL STORZ is certi-fied in accordance with the requirements of ISO9001/EN 46001. It guarantees constant quality testingin the selection of materials and components. At theend of each manufacturing process, tests are carriedout with automatic measuring and testing systemsdeveloped specially for this purpose. The results arerecorded and logged. That gives each device a distinct“fingerprint” that can be checked at any time beforeand after it is delivered to the customer.

The standardized, modular design of KARL STORZunits was developed based on extensive ergonomicstudies and is conceived for ease of care and cleaningand user-friendly practice, as well as to meet thedemands of the special hygienic standards required insurgery. Clearly laid out adjacent function keys anddisplays guarantee efficient operation and make iteasier to constantly monitor actual and set parameters.Acoustic and visual warning signals also assist theuser. The settings can be changed manually at anytime. Reprocessing and Careof Video Endoscopes

All flexible KARL STORZ endoscopes can be connectedto washers/disinfectors for semi- or fully automaticcleaning and disinfection. Applicable national regulationsfor cleaning and disinfection must be observed. First aleakage test is performed as well as manual pre-cleaning and rinsing with microbiologically pure water.The choice of reprocessing programs must be agreedwith the machine manufacturer. In most cases, specialprograms for flexible endoscopes are already availablethat are suitable for decontamination and more gentleon materials. For machine cleaning, it should be notedthat flexible endoscopes should not be heated over60 °C.

The video endoscope should be checked for leaksbefore every decontamination procedure in semi- orfully automatic equipment.

Only chemicals approved by KARL STORZ for repro-cessing should be used.

The endoscope should be carefully placed in the trayor slot. Irrigation tubes from the Cleaning Adaptor Set13991 CK or irrigation tubes from washer/disinfectorshould be connected to the endoscope.

Cleaning Tube Set Gastro 13991 CK is used for thevalidated reprocessing of KARL STORZ video endo-scopes from the series 13xxx NKS/PKS (acc. to ISO15883 part 4). The cleaning adaptor set is not intendedfor any other use as this may lead to damage.

Reprocessing

When reprocessing flexible endoscopes used for gas-troenterological purposes, two topics are of particularrelevance: Infection prevention of future patients andpreservation of instruments or avoiding unnecessarydamage to endoscopes.

To ensure adequate reprocessing, please observe thedetailed description given in the KARL STORZ in-struction manual for endoscopes.

The described cleaning, disinfection and sterilizationprocedures alone do not guarantee successful re-processing of the endoscopes. This can only beachieved if personnel follow recognized and validatedreprocessing methods.

The local occupational safety guidelines must be ob-served when cleaning and disinfecting contaminatedendoscopes and instruments. A list of KARL STORZapproved products for reprocessing video endoscopescan be found on the Internet at “www.karlstorz.com” or“www.karlstorz.de” and the respective instructionmanual.

When preparing and using cleaning and disinfectionsolutions, the chemical manufacturer’s instructions mustbe strictly observed, paying close attention to properconcentration and exposure times. Disregarding theinstructions can lead to damage.

Please note that decontamination can only beperformed successfully if● the instruments are free of all organic materials and

chemical residue● recognized and validated procedures are used● any deviation from recommended procedures is

validated and checked by the user● the recommended procedure parameters are

maintained. These are only valid withwashers/disinfectors or sterilization equipment thatare properly maintained and calibrated.
