FEDERAL & STATE REGU LA TIONS:Acceptance of Federal equip ment and supplies by users who have established eligibility to participate in the AZSASP is subject to cer ti fi ca tions and agreements identified on the back side of the standard invoice.

1. Federal equipment and supplies are restricted to in sti tu tional use only, not personal use, and are to be used for a minimum of one year.

2. All vehicles and items with an ac-quisition cost of $5,000.00 or more shall be used for the purpose(s) for which acquired within one year from the date the property was invoiced and used for 18 months there af ter.

3. During the restriction pe riod, the property shall not be sold, traded, leased, lent, bailed, cannibalized, en cum bered, or otherwise dis posed of or re moved from the State of Arizona without the express approval of the AZSASP. The recipients of Federal property are urged to contact the AZSASP prior to taking any actions which might be interpreted as modi fy ing any of the cer ti fi ca tions and agreements stated on the back side of the notice.

TERMS: Net 30 days to all users unless previous ar range ments have been made. Payment must be by check issued against an eligible organization. No personal checks or cash transactions are permitted.
