Part I: To be completed by the Student. Return the completed application package, including the application form and the two required attachments, to the Director of Undergraduate Studies in your department of concentration.

For F1 Visa Students: Once approved in Part II, below, the Office of the Registrar will forward notice of the Pending Milestone Credit directly to the Davis International Center. Students are responsible for submitting all other CPT application materials directly to the Davis Center separately. Please ensure that the offer letter that you submit with this application includes all of the details as listed on the Davis International Center website.

Once approved, this completed form should be forwarded to the Office of the Registrar, so that the pending Milestone Credit can be attached to the student’s academic record. Forms should be scanned and emailed to ugmilestonecredit@princeton.edu. Only the form, with the approval of the department, is required. Please do not include attachments.

Departments will maintain a copy of the approved application form in order to certify completion of the internship, as described in Part III, below. Departmental academic requirements for internship milestone credit, if applicable (e.g. paper, project, etc.):

Part III: To be completed by both the student and the department at the conclusion of the internship.

The student must provide the department with a letter or other evidence that the internship was completed. This letter must be on Internship Organization letterhead and signed by the sponsor from that organization. After receiving that confirmation, the department should complete this certification and return the form to the Office of the Registrar. Forms may be scanned and sent via email to ugmilestonecredit@princeton.edu.
